RATIONALE: Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital vascular anomaly that often remains undetected in asymptomatic patients. While most cases are hemodynamically insignificant, failure to recognize PAPVC preoperatively may lead to intraoperative misidentification and surgical complications. Coexistence of PAPVC and lung cancer, particularly on the ipsilateral side, is extremely rare and poses challenges for surgical decision-making.

Bilateral spontaneous pneumothorax is a rare condition that accounts for approximately 1% of all pneumothorax cases. BACKGROUNDS: With increase of patients with a small-sized lung cancer, there is an increasing need for minimally invasive lung segmentectomy that can preserve respiratory function. We perform S(9+)10 segmentectomy with retrograde dissection of the pulmonary vein, bronchus, pulmonary artery, in order, without interlober fissurelectomy and staple dissection of the peripheral lung parenchyma.

Diaphragm plication has recently become a standard operation for diaphragm eventration. The surgical experience for the patient has improved with the development of minimally invasive operations, but the operator's convenience has not improved significantly. We performed video-assisted thoracoscopic surgery for a diaphragm oblique resection using endostaplers in 10 patients. The total operation time was 39.5 minutes; all patients' symptoms disappeared postoperatively. This presentation will help more thoracic surgeons simplify this operation on the diaphragm.

Posterior basal (S10) segmentectomy is one of the most challenging (and uncommon) types of pulmonary segmentectomy. Here, we present two key tips for facilitating a uniportal operation. The first is a full understanding of the relative locations of the pulmonary vessels and bronchi (as revealed by preoperative three-dimensional computed tomography/broncho-angiography), and the other is the use of "suction-guided stapling" to dissect and divide the peripheral pulmonary vessels and bronchi. In the present study, influences of pleural adhesions on thoracoscopic lung surgeries were investigated. A total of 666 consecutive patients who had undergone thoracoscopic surgeries for lung malignant tumors were retrospectively analyzed. A 40 years-old woman with a previous history of a right pneumothorax in 2013 had a left pneumothorax with persistent air leak in 2020. The computerized tomography scan of the chest described diffuse emphysematous changes with multiple "small bullae" in the upper lobes. She was submitted to mechanical and chemical pleurodesis with a wedge apical resection by uniportal video-assisted thoracic surgery. Pathological findings confirm the diagnosis of pulmonary lymphangioleiomyomatosis.
