Pediatric acute kidney injury in Rwanda: awareness, early detection, and timely management to improve outcomes, a multi-center mixed method study.

BACKGROUND: Acute kidney injury in children is a serious but often overlooked condition in low-resource settings. In Rwanda, although referral hospitals provide advanced care, most children are managed at district hospitals, where limited diagnostic services and low provider awareness may delay diagnosis. We assessed healthcare providers' knowledge of pediatric AKI, audited real-world case management, and evaluated whether a brief training intervention could improve early detection and care. METHODS: We conducted a mixed-method study in six Rwandan hospitals affiliated with the University Tea

Provider Preferences, Perceptions, and Barriers to Colorectal Cancer Screening in Federally Qualified Health Centers: A Cross-Sectional Survey.

BACKGROUND: Colorectal cancer (CRC) screening reduces mortality, yet uptake varies by screening modality, provider type, and clinical setting. Federally Qualified Health Centers (FQHCs) care for underserved populations and face unique challenges, including limited resources, diverse care needs, and competing clinical priorities. Despite the importance of screening, provider preferences for CRC screening modalities are not well understood. METHODS: We conducted a cross-sectional survey of primary care providers (physicians, nurse practitioners, physician assistants) at four FQHCs in Wisconsin.

Shifts in Antipsychotic Prescribing by Clinician Type for Medicare Part D Beneficiaries, 2013-2023.

IMPORTANCE: Despite safety warnings, antipsychotics remain commonly prescribed to older adults, particularly those with dementia, and for off-label indications. However, little is known about how prescribing patterns vary by clinician type over time. OBJECTIVE: To examine trends in antipsychotic prescribing by clinician type among Medicare beneficiaries from 2013 to 2023. DESIGN, SETTING, AND PARTICIPANTS: This repeated cross-sectional study used the Medicare Part D Prescribers by Provider and Drug dataset from 2013 to 2023. Participants included Medicare beneficiaries enrolled in Medicare P

A multidisciplinary virtual (p)rehabilitation model of care for colorectal cancer surgery.

BACKGROUND: Colorectal cancer is a leading cause of cancer-related morbidity and mortality globally, with surgery the primary curative treatment. Despite advances in medical care, postoperative complications are common, resulting in prolonged hospital stays, delayed recovery, diminished quality of life, and increased healthcare costs. This study aimed to develop a standardised, evidence-based, multidisciplinary virtual model of care for (p)rehabilitation (i.e., preoperative and postoperative rehabilitation) in colorectal cancer surgery patients. METHODS: This study reports the development of

Ann Arbor Guide to Triaging Adults With Suspected Urinary Tract Infection for In-Person and Telehealth Settings.

IMPORTANCE: Urinary tract infection (UTI) is common in ambulatory care settings and the primary reason for antibiotic prescribing. Despite several guidelines focused on the type and duration of antibiotics prescribed for treating UTI, there is limited outpatient guidance on how to best triage patients with presumed UTI. OBJECTIVE: To assess the appropriateness of different triage and management recommendations involving empiric antibiotics, urine testing strategies, and visit types and how these recommendations vary by patient sex, age, presenting symptoms, and clinical history. Expert Consensus Recommendations on the Management of Treatment-emergent Adverse Events Among Men with Prostate Cancer Taking Poly-ADP Ribose Polymerase Inhibitor + Novel Hormonal Therapy Combination Therapy.

BACKGROUND AND OBJECTIVE: Recent clinical trials have shown improvement in progression-free survival in men with metastatic prostate cancer (mPC) treated with combination poly-ADP ribose polymerase (PARP) inhibitors (PARPi) and novel hormonal therapy (NHT). Regulatory bodies in the USA, Canada, Europe, and Japan have recently approved this combination therapy for mPC. Common adverse events (AEs) include fatigue, nausea and vomiting, and anemia. Nuanced AE management guidance for these combinations is lacking. The panel objective was to develop expert consensus on AE management in patients with

The Next Frontier of Remote Patient Monitoring: Hospital at Home.

Remote patient monitoring (RPM) has shown promise in aiding safe and efficient remote care for chronic conditions; however, its use remains more limited within the hospital at home (HaH) model of care despite a significant opportunity to increase patient eligibility, improve safety, and decrease costs. HaH could achieve these goals by further adopting the 3 primary modalities of RPM (ie, vital sign, continuous single-lead electrocardiogram, and fall monitoring). With only 2 in-person vital sign checks required per day, HaH patient eligibility is currently often limited to lower-acuity cases. T

Pilot Trial of Homebound Hematopoietic Cell Transplantation.

For eligible patients with multiple myeloma (MM) and amyloid light chain (AL) amyloidosis, high-dose chemotherapy and autologous hematopoietic cell transplantation (HCT) is a standard and widely used consolidation therapy. Autologous HCT requires specialized care at a transplantation center and investment from patients and caregivers. We studied the safety and feasibility of delivering transplantation care in a homebound setting to decrease the burden of therapy and increase access to autologous HCT. Patients with MM and AL amyloidosis undergoing autologous HCT were eligible if they resided in

The SUMMIT ambulatory-ICU primary care model for medically and socially complex patients in an urban federally qualified health center: study design and rationale.

BACKGROUND: Medically complex urban patients experiencing homelessness comprise a disproportionate number of high-cost, high-need patients. There are few studies of interventions to improve care for these populations; their social complexity makes them difficult to study and requires clinical and research collaboration. We present a protocol for a trial of the streamlined unified meaningfully managed interdisciplinary team (SUMMIT) team, an ambulatory ICU (A-ICU) intervention to improve utilization and patient experience that uses control populations to address limitations of prior research.

Reducing moral distress through interdisciplinary collaboration: the impact of a weekly palliative care and neonatology conference.

BACKGROUND: Moral distress is the experience of knowing what ethically right action to take, but being unable to act accordingly, due to external factors. It is an experience common to providers working in the neonatal intensive care unit (NICU) where care for infants, often born at the edges of viability or with other life-limiting diagnoses, includes life and death medical decision-making in the context of uncertain prognoses. Palliative care, which aims to reduce suffering, can assist with staff moral distress by providing space for conversations regarding goals-of-care and end-of-life deci

Use of the Maslach Burnout Inventory Among Public Health Care Professionals: Scoping Review.

BACKGROUND: Work can be considered a source of living, well-being, and socioeconomic development. When the work environment negatively influences individuals, it may trigger emotional disturbances, behavioral problems, chronic stress conditions, and illnesses such as burnout syndrome (BS). Recently, studies on BS have increased and placed a special focus on health care professionals. The prevalence of BS among health professionals is associated with their chronic exposure to human hardship and long working hours without proper rest. These factors have contributed to greater stress and high phy

A placenta clinic approach to the diagnosis and management of fetal growth restriction.

Effective detection and management of fetal growth restriction is relevant to all obstetric care providers. Models of best practice to care for these patients and their families continue to evolve. Since much of the disease burden in fetal growth restriction originates in the placenta, the concept of a multidisciplinary placenta clinic program, managed primarily within a maternal-fetal medicine division, has gained popularity. In this context, fetal growth restriction is merely one of many placenta-related disorders that can benefit from an interdisciplinary approach, incorporating expertise f

Carriage, Clinical Microbiology and Transmission of Staphylococcus aureus.

Staphylococcus aureus is one of the most important bacterial pathogens in clinical practice and a major diagnostic focus for the routine microbiology laboratory. It is carried as a harmless commensal in up to two-thirds of the population at any one time predominantly not only in the anterior nares, but also in multiple other sites such as the groin, axilla, throat, perineum, vagina and rectum. It colonizes skin breach sites, such as ulcers and wounds, and causes superficial and deep skin and soft tissue infections and life-threatening deep seated infections particularly endocarditis and osteom
