Metallo beta-lactamase producing pseudomonas species--a major cause of concern among hospital associated urinary tract infection.

Multidrug resistant pseudomonas strains are isolated in high frequency from urinary samples in hospitalised patients. With the occurrence of extended spectrum beta-lactamases (ESBLs) and metallo beta-lactamase (MBL)-producing Pseudomonas aeruginosa being increasingly reported worldwide, this study evaluated the resistance pattern of pseudomonad strains to carbapenam and other antipseudomonal antibiotics isolated from patients with hospital associated urinary tract infections along with clinical usefulness of various MBL screening methods (combined disc diffusion test, E-test and modified Hodge

Bacterial vaginosis and trichomoniasis vaginitis are risk factors for cuff cellulitis after abdominal hysterectomy.

To assess the relationship between either bacterial vaginosis or trichomoniasis vaginitis and posthysterectomy infection, preoperative evaluation of the vaginal secretions was performed in 161 women undergoing abdominal hysterectomy. Thirty-two patients (19.9%) and 27 patients (16.8%), respectively, met the diagnostic criteria for bacterial vaginosis and trichomoniasis vaginitis. Patients with either bacterial vaginosis or trichomoniasis vaginitis were more likely than control subjects to have cuff cellulitis, cuff abscess, or both (relative risk 3.2, 95% confidence interval 1.5 to 6.7 for bac

Aminoglycosides in gynecologic infections.

Aminoglycosides have always had a major role in the treatment of gynecologic infections. This therapeutic emphasis has been based upon the bactericidal activity of the aminoglycosides against gram-negative aerobes. This group of organisms is important to the gynecologist because of their frequent isolation from soft tissue, urinary sites, and the bloodstream in women with nosocomial or community-acquired pelvic infections. In the past decade, there has been increasing clinical awareness of the multi-bacterial nature of these soft tissue pelvic infections. A major therapeutic change directed ag

Utilization of antibiotics for prevention of symptomatic postpartum infections.

This analysis determined the incidence, indications, and efficiency of postpartum antibiotic therapy. At Wilford Hall USAF Medical Center, 384 of 2000 consecutive delivered patients (19.2%) were treated with antibiotics during the postpartum period. The greatest utilization of antibiotics was prophylactic or for subclinical infection (290 patients; 75.5%) as compared to treatment for symptomatic infection. The most frequent indications for therapy were for patients at high risk to develop pelvic infection or to prevent urinary tract infection. The failure of antibiotic therapy and the developm

[Activity of antiseptic catheter lubricants against methicillin-resistant Staphylococcus aureus (MRSA)].

In recent years, general and urinary tract infections caused by methicillin-resistant Staphylococcus aureus (MRSA) have became an increasing problem worldwide. Catheterization of the urinary bladder is one of the most significant risk factors for MRSA contamination of the urine. In vitro tests have documented the antimicrobial activity of the catheter lubricants Instillagel and Endosgel against MRSA specimens within 5 min. These findings support recommending the use of these lubricants in high-risk patients in hospitals and old people's homes for prophylaxis of MRSA bacteriuria.

Is there a place for indwelling transurethral catheterization in women with febrile urinary tract infection? A prospective randomized trial.

PURPOSE: To assess the common practice to transiently place an indwelling transurethral catheter in case of hospitalization of women with febrile urinary tract infections. So far, this intervention has not been scientifically investigated. METHODS: Inclusion criteria were female gender, a leucocyte esterase-positive urine dipstick analysis (≥ 250) from urine obtained with a catheter and fever > 38 °C. Patients were randomized 1:1 to either receive an indwelling catheter French 16 or not. The catheter was removed after 24 h without fever (< 37.5 °C). Principal exclusion criteria were a post vo

[Inquiry among general practitioners' knowledge regarding clean intermittent self-catheterization].

INTRODUCTION: Clean intermittent self-catheterization is the recommended mode of bladder voiding in patients with urinary retention. Hence, this procedure should be well known and understood by the general practitioners (GP) who are responsible for monitoring such patients. OBJECTIVE: To assess GP' state of knowledge regarding clean intermittent self-catheterization and to make recommendations for dealing with problems that occur during patient follow-up. METHOD: A questionnaire focusing on the practice of clean intermittent catheterization was sent to 910 family GP in the Franche-Comté regi

[Development and validation of a diagnostic model of antimicrobial resistance by extended-spectrum beta-lactamases in community-acquired urinary tract infections. How to adjust the prediction in variable outcome prevalences? The usefulness of contracting predictors with LASSO regression].

BACKGROUND: In the case of community-acquired urinary tract infection, the identification of Enterobacteriaceae with extended spectrum beta-lactamases (ESBL) can optimize treatment, control and follow-up strategies, however the effect of variable prevalences of this resistance pattern has affected the external validity of this type of models. AIM: To develop a diagnostic predictive model that adjusts the prediction error in variable prevalences using the LASSO regression. METHODS: A diagnostic predictive model of community-acquired urinary tract infection by infection by ESBL producing Enter

In vitro killing of canine strains of Staphylococcus pseudintermedius and Escherichia coli by cefazolin, cefovecin, doxycycline and pradofloxacin over a range of bacterial densities.

BACKGROUND: Bacterial densities likely fluctuate during infection and may exceed the bacterial density used in susceptibility testing. As such, investigation of bacterial killing by antibiotics over a range of varying bacterial densities may provide important differences between compounds and could impact drug selection for therapy. HYPOTHESIS/OBJECTIVES: To measure killing of clinical isolates of Staphylococcus pseudintermedius and Escherichia coli by cefazolin, cefovecin, doxycycline and pradofloxacin at clinically relevant (minimum inhibitory, mutant prevention, maximum serum and maximum t

Activity of temocillin and comparators against urinary Escherichia coli and Klebsiella pneumoniae from Iran.

To evaluate the ground susceptibility of urinary Escherichia coli and Klebsiella pneumoniae to temocillin, the susceptibility of temocillin and comparators against 500 clinical isolates (231 E. coli and 269 K. pneumoniae) was tested. Temocillin was either found as the most active antibiotic (susceptibility rate of 92%) or the fourth most active antibiotic (65%), as per its urinary and systemic breakpoint, respectively. No difference of activity was observed in isolates expressing ESBL/AmpC enzymes. Considering the percentage of isolates expressing beta-lactamases and the need to spare broad-sp

Urinary tract infection in children after cardiac surgery: Incidence, causes, risk factors and outcomes in a single-center study.

Nosocomial urinary tract infection (UTI) increases hospitalization, cost and morbidity. In this cohort study, we aimed to determine the incidence, risk factors, etiology and outcomes of UTIs in post-operative cardiac children. To this end, we studied all post-operative patients admitted to the Pediatric Cardiac Intensive Care Unit (PCICU) in 2012, and we divided the patients into two groups: the UTI (UTI group) and the non-UTI (control group). We compared both groups for multiple peri-operative risk factors. We included 413 children in this study. Of these, 29 (7%) had UTIs after cardiac surge

Management of suspected bacterial urinary tract infection.

New guidelines on the management of suspected bacterial urinary tract infection in adults have just been released by the Scottish Intercollegiate Guidelines Network (SIGN). The guidance states that the presence of bacteriuria should lead to antibiotic treatment only when there is definitive evidence that eradicating the bacterial infection will result in a tangible health gain at a reasonable level of risk (SIGN, 2006).
