[Otto Lanz; colourful appendectomist and art collector].

Otto Lanz (1865-1935) was a Swiss surgeon trained by Theodore Kocher in Berne. In 1902, he was invited to become professor of surgery at the Binnengasthuis hospital in Amsterdam. He was an outstanding surgeon who was well known for his surgery of the thyroid gland and the appendix. He introduced the eponymous Lanz's point in acute appendicitis and also the girdle incision for appendectomy. He was a strong proponent of 'early' appendectomy, as opposed to the conservative approach, at that time the treatment of choice. He was a great connoisseur and passionate collector of Italian Renaissance ar…

Secondary appendicitis in an adult patient with an ascended testicle - case report.

Acute appendicitis is one of the most common acute abdomen cases. Although many causes have been described in literature, it is mostly primary affection of the appendix that leads to acute appendicitis. A 57-year-old patient was admitted to our department displaying all usual signs of acute appendicitis, both clinical and laboratory. During laparoscopic appendectomy, we found the inflammation of the appendix in this case appeared secondary, originating in a structure located nearby the appendix, strongly resembling a testicle. We performed appendectomy and removed the suspected structure altog…

Down the Rabbit Hole-Considerations for Ingested Foreign Bodies.

We report the case of a seven-year-old boy with an ingested foreign body, which was retained within the appendix for a known duration of ten months, ultimately requiring appendectomy. The ingested foreign body was incidentally discovered by abdominal x-ray at an emergency room visit for constipation. Despite four bowel cleanouts, subsequent x-rays showed persistence of the foreign body in the right lower quadrant. While the patient did not have signs or symptoms of acute appendicitis, laparoscopic appendectomy was performed due to the risk of this foreign body causing appendicitis in the futur…

Goblet cell carcinoid of the appendix accompanied by adenomatous polyp with high-grade dysplasia at the cecum.

Goblet cell carcinoid tumor of the appendix is an uncommon neoplasia that shares the histological attributes of both adenocarcinoma and carc … cell carcinoid tumor of the appendix was revealed incidentally during microscopic evaluation of the appendectomy specimen. The patient healed uneventfully and no recurrence was observed after the 12-month follow-up period. Careful microscopic examination of the appendectomy specimen is key for the diagnosis of appendiceal tumors such as goblet cell carcinoid, …

Carcinoid tumor of the appendix: A case report.

Carcinoid tumors are the most common neoplasm of the appendix. The clinical presentation of these lesions is often similar to that of acute appendicitis, or the tumors are asymptomatic. The carcinoids are commonly found incidentally during histopathological examination of the resected appendix following appendectomy or other abdominal procedures. Appendiceal carcinoids usually behave as benign tumors and appendicectomy alone is a sufficient treatment in the majority of cases, while for larger lesions, right colectomy should be performed. The prognosis of patients with local appendiceal carcino…

Variations in the position and length of the vermiform appendix in a black kenyan population.

Background. Topography of the appendix influences its mobility, degree of mobilization of the cecum, and need for additional muscle splitting during appendectomy. Although appendectomy is a common surgical procedure, there is a paucity of data on its topography in black Africans. Methods. The position and length of the appendix and relation of the appendicular base with spinoumbilical line were determined in 48 cadavers obtained from the Department of Human Anatomy, University of Nairobi, Kenya. Results. The commonest appendicular types in males were retrocecal 10 (27%) while in females was su…

Live birth after laparotomy for concurrent heterotopic pregnancy and appendicitis in a 6 weeks IVF pregnancy.

BACKGROUND: A term delivery after concurrent appendectomy and salpingectomy at 6 weeks in an appendicitis coexisting with a heterotopic pregnancy after in vitro fertilization (IVF) treatment is reported. METHODS: Case report. RESULTS: The patient presented with severe low abdominal pain at 6 weeks of pregnancy after IVF and had explorative laparotomy for appendicitis, during which she had appendectomy and salpingectomy for an undiagnosed concurrent heterotopic pregnancy. The patient delivered a 3,960 g healthy infant at 38 weeks. CONCLUSION: In patients with severe abdominal pain after IVF,…

[Iterative intestinal intussusception and appendiceal mucocele in an infant with mucoviscidosis].

UNLABELLED: Cystic fibrosis is a common and potentially life-threatening hereditary disease which can affect numerous organs, particularly the digestive tract. CASE REPORT: A 4.5-year-old boy exhibited two little known clinical manifestations: an appendiceal mucocele and repeated intussusceptions. In spite of an appendectomy, intussusception relapsed and an ileocolic resection was necessary 2 years later. DISCUSSION: Appendiceal diseases in cystic fibrosis represent a large spectrum, ie, distention on the appendiceal lumen, engorged with sticky mucous matter, which becomes an appendiceal muc…

[Primary adenocarcinoma of the appendix].

The authors published experience with the treatment of four patients with primary adenocarcinoma of the appendix and an analysis of 20 patients with primary adenocarcinoma of the appendix treated in the North Moravia region during a ten-year period (1980-1989). The incidence of the disease is rare. Usually it is manifested by symptoms imitating acute appendicitis. In exceptional instances there are other symptoms such as melaena, invagination, infiltration of neighbouring organs etc. Only rarely the disease is diagnosed during appendectomy. Most frequently an adenocarcinoma of the colonic type…

Increasing accuracy in the diagnosis of acute appendicitis with modern diagnostic techniques.

One hundred consecutive patients admitted to the hospital with the prospective diagnosis of appendicitis were evaluated retrospectively to determine the effect of the use of modern diagnostic techniques upon the accuracy of preoperative diagnosis. Appendectomy was performed on 48 patients. In three of these patients, the diagnostic process was assisted by the use of radiologic or ultrasound techniques. Of the 52 patients who did not undergo appendectomy, 13 patients clearly avoided surgery or were steered towards an other appropriate operative procedure due to findings on ultrasound or barium …

Serum immunoglobulins after surgical operation.

We measured immunoglobulins in the sera of 33 patients on days 1, 3, 6, 10, and 17 and three to four weeks after surgical operations (mostly hysterectomy or appendectomy) or (six patients) after spinal injury. In the absence of infection or blood transfusion, IgG usually decreased slightly and transiently after hysterectomy or appendectomy, as did IgA or IgM after hysterectomy. IgD concentrations showed no consistent changes, but in one patient after hysterectomy and with minimal infection IgD concentration decreased sharply, which contrasted with significant and early increases in IgG, IgA, a…

Clinical aspects and self-reported symptoms of sequelae of Yersinia enterocolitica infections in a population-based study, Germany 2009-2010.

BACKGROUND: Foodborne Yersinia enterocolitica infections continue to be a public health problem in many countries. Consumption of raw or undercooked pork is the main risk factor for yersiniosis in Germany. Small children are most frequently affected by yersiniosis. In older children and young adults, symptoms of disease may resemble those of appendicitis and may lead to hospitalization and potentially unnecessary appendectomies. Y. enterocolitica infections may also cause sequelae such as reactive arthritis (ReA), erythema nodosum (EN), and conjunctivitis. METHODS: We studied clinical aspects

Preoperative diagnosis of Amyand's hernia complicated by strangulated acute appendicitis: a case report and literature review.

Amyand's hernia (AH) is a rare clinical entity characterized by the presence of the appendix within an inguinal hernia sac, with or without associated appendicitis, and is rarely diagnosed preoperatively due to its nonspecific clinical presentation. We report the case of a 66-year-old woman who presented with an irreducible right inguinal swelling accompanied by severe right lower abdominal pain. Contrast-enhanced computed tomography (CT) revealed an inflamed appendix within the hernial sac, consistent with a strangulated AH. The patient underwent appendectomy along with hernia repair, with an…

Long-term outcomes of non-operative compared to operative management of acute uncomplicated appendicitis - a systematic review and meta-analysis.

BACKGROUND: We evaluated long-term outcomes of non-operative management (NOM) versus surgical management of acute uncomplicated appendicitis. METHODS: Systematic review of studies comparing NOM versus surgery with ≥2 years follow-up. Primary outcome was long-term failure rate. RESULTS: 9/1635 studies were included (3 RCTs; 6 non-RCT studies), involving 3883 patients; 5 were in pediatric populations. Median follow-up was 33.6 (range 24-312) months. NOM pooled long-term failure rate was 38.9% (95% CI: 31.1%-46.7%), increasing to 44.4% (95% CI: 41.4%-47.4%) in RCTs. Pooled appendectomy rate aft…

[Appendix somewhat different-Excerpts from the history, controversies of the present and view to the future].

If one could talk about a superstar in abdominal surgery, the appendix would definitely be qualified for this name. Much has already been said about the vermiform appendix. The potential to talk about the appendix is, however, by no means exhausted. In this article already published articles are selected and subsumed. Less known details on the discovery and research of the appendix as well as the diagnostics of appendicitis and establishment of appendectomy are also included.

Diverticulosis of the vermiform appendix.

Diverticulosis of the vermiform appendix is not a common entity. Due to possible complications resulting from this lesion the discovery of an appendiceal diverticulum on barium studies should not be overlooked and may be an indication for appendectomy.
