Mechanisms and clinical consequences of acute lung injury.

Acute respiratory distress syndrome (ARDS) was first described in 1967, and since then there have been a large number of studies addressing its pathogenesis and therapies. Despite intense research efforts, very few therapies for ARDS have been shown to be effective other than the use of lung protection strategies. The scarcity of therapeutic choices is related to the intricate pathogenesis of the syndrome and to insensitive and aspecific criteria to diagnose this profound acute respiratory failure. The aim of this paper is to summarize advances of new ARDS definitions and provide an overview o

Angiogenic potential of microvessel fragments is independent of the tissue of origin and can be influenced by the cellular composition of the implants.

UNLABELLED: We have demonstrated that MFs isolated from adipose retain angiogenic potential in vitro and form a mature, perfused network when implanted. However, adipose-derived microvessels are rich in provascularizing cells that could uniquely drive neovascularization in adipose-derived MFs implants. OBJECTIVE: Investigate the ability of MFs from a different vascular bed to recapitulate adipose-derived microvessel angiogenesis and network formation and analyze adipose-derived vessel plasticity by assessing whether vessel function could be modulated by astrocyte-like cells. METHODS: MFs wer

Evidence of increased endometrial vascular permeability at the time of implantation in the short-nosed fruit bat, Cyanopterus sphinx.

Early embryonic development and implantation were studied in tropical short-nosed fruit bat Cyanopterus sphinx. We report preimplantation development and embryo implantation. Different stages of cleavage were observed in embryo by direct microscopic examination of fresh embryos after retrieving them either from the oviduct or the uterus at different days, up to the day of implantation. Generally, the embryos enter the uterus at the 8-cell stage. Embryonic development continued without any delay and blastocyst were formed showing attachment to the uterine epithelium at the mesometrial side of t

Gastric antiulcer and anti-inflammatory activities of the essential oil from Casearia sylvestris Sw.

To investigate the antiulcer and antiinflammatory activities of the essential oil from Casearia sylvestris leaves (EOCS) the following tests were used: rat paw edema, granulomatous tissue test, vascular permeability, writhing test, gastric ulcer stress-induced and evaluation of gastric secretion (pylorus ligation test). The total yield of EOCS was 2.5% with LD50 of 1100 mg/kg in mouse. The major compounds identified using gas chromatography were caryophyllene, thujopsene, alfa-humulene, beta-acoradiene, germacrene-d, bicyclogermacrene, calamenene, germacrene B, spathulenol and globulol. The EO

H1-receptors: localization and role in airway physiology and in immune functions.

Histamine H(1)-receptors are involved in the pathologic processes of allergy. Clinical trials of H(1)-receptor antagonists have demonstrated the efficacy of these agents in reducing the sneezing, pruritus, and rhinorrhea associated with allergic rhinitis. In the lung, H(1)-receptors mediate the bronchoconstrictive effects of histamine and increase vascular permeability, which lead to plasma exudation. H(1)-receptors are present on T cells, B cells, monocytes, and lymphocytes, and stimulation of these receptors induces pro-inflammatory effects. It has been suggested that a signal from the H(1)-

Transient forebrain ischemia increases the blood-brain barrier permeability for albumin in stroke-prone spontaneously hypertensive rats.

1. The aim of the present study was to reveal the effect of transient forebrain ischemia on the regional and temporal changes in the permeability of the blood-brain barrier (BBB) permeability for sodium fluorescein (MW: 376 Da) and Evan's blue-labeled albumin (MW: 67 kDa) in stroke-prone spontaneously hypertensive rats (SHRSP). 2. BBB permeability was significantly higher in the brain regions of 16-week-old control SHRSP than those in age-matched normotensive Wistar-Kyoto rats. 3. Transient forebrain ischemia evoked by 10-min bilateral carotid occlusion increased the permeability of the BBB fo

Capillary filtration is reduced in lungs adapted to chronic heart failure: morphological and haemodynamic correlates.

OBJECTIVE: To determine pulmonary capillary filtration in experimental chronic heart failure and to investigate some morphological and haemodynamic mechanisms that could account for reduced filtration in lungs adapted to chronic heart failure. METHODS: We studied pulmonary capillary filtration, vascular resistances and morphology in lungs from guinea-pigs adapted to chronic heart failure. Heart failure was induced by banding of the ascending aorta (n=66) or sham control operation (n=78) in guinea-pigs which were studied at 150+/-8 days post-operation. RESULTS: Reduced cardiac output, increas

Microvascular coagulopathy and disseminated intravascular coagulation.

OBJECTIVE: To review the dual characteristics of disseminated intravascular coagulation (DIC), as both a contributor to multiple organ failure as well as a symptom of severe underlying disease associated with systemic vascular changes. DATA SOURCES: Published literature data and unpublished results from the authors. DATA SUMMARY: Clinical and experimental studies strongly suggest that DIC contributes to multiple organ failure and death in patients with severe systemic disorders such as sepsis. DIC is evoked by systemic cytokine activity, and the inflammatory response aggravates vascular perm

Experimental evaluation of the vascular effects and transport of an iodinated macromolecular contrast medium.

RATIONALE AND OBJECTIVES: For assessment of the tissue blood pool and overall vascularity, macromolecular contrast media have significant advantages over low molecular weight contrast agents. The authors evaluated the vascular effect and transport of a new macromolecular contrast media (MMCM), an iodinated dextran polymer of 32 kDa. METHODS: The new MMCM was obtained from dextran activated by carboxy methylation, followed by linkage with triiodinated aminophtalamid conjugates. To detect whether the tracer induces vascular leakage, MMCM (350 mg I/kg) was administered intravenously in 10 mice,

A review of effects of hypothyroidism on vascular transport in skeletal muscle during exercise.

Hypothyroidism is a common thyroid disease characterized by exercise intolerance. Both exercise capacity and endurance are compromised in the hypothyroid state. Studies involving rats performing treadmill running have shown that blood flows during exercise to high oxidative, extensor-type muscles are lower in hypothyroid rats compared with those in euthyroid rats. Abnormal cardiac and vascular function appear to contribute to this hypoperfusion. Experiments involving isolated arterial vessel segments have demonstrated that potential for constriction is normal in vessels from hypothyroid animal

Changes in vascular permeability and deciduoma formation during the peri-implantation period of the rat in response to exogenous gonadotropins.

BACKGROUND: Permeability of thin-walled vessels of the endometrium could be a prerequisite for deciduoma formation. Exogenous gonadotropins alter endometrial morphology and inhibit vascular permeability on the day of implantation. The aim of this study was to investigate the effect of exogenous gonadotropins on vascular permeability in the peri-implantation period and relate this to deciduoma formation. METHODS: Female rats were hyperstimulated with follicle-stimulating hormone and human chorionic gonadotropin prior to mating. Control animals were not injected. Endometrial tissue was collecte

Reperfusion nerve injury: pathology due to reflow and prolonged ischaemia.

Ischaemia plays an important role in the development of pathological changes in various neuropathies. Nerve pathology in acute ischaemic injury has been delineated longitudinally in peripheral nerve and reperfusion injury could amplify ischaemic pathology. We examined ischaemia/reperfusion-induced pathological changes along the length of sciatic, tibial and peroneal nerves from pelvic to ankle levels. Pathological features were correlated with the degree of postischaemic nerve blood flow (NBF) restoration, measured by a laser Doppler flowmeter, and the blood-nerve barrier function by a horsera

Attenuating effect of antithrombin III on the fibrinolytic activation and microvascular derangement in rat gastric mucosa.

The roles for the fibrinolytic activation and disorder of coagulation in formation of gastric ulcer induced by microvascular derangement were investigated. The rat stomach was exposed and repeated electrical stimuli (RES) were applied on the small arterial wall close to the lesser curvature to induce mucosal microcirculatory disturbances. The level of tissue-type plasminogen activator (t-PA), a key enzyme for fibrinolytic activity, in the regional blood of the stomach was significantly elevated immediately after RES. At 5 min after RES, the leakage of FITC-labeled albumin and thrombus formatio

The problem of defining heart failure.

A succession of theories arising from last century has attempted to explain why patients with damaged hearts develop peripheral edema. Opposed to the original simple concepts of backward failure, a number of theories of forward failure have been proposed, the cardiac output being considered inadequate for capillary permeability, renal function, or the metabolic needs of the body. Any theory needs to take account of the neuroendocrine stimulation now known to occur under these conditions. This article presents evidence for the belief that the condition arises when the cardiac output becomes ins…

Hereditary angioneurotic oedema: characterization of plasma kinin and vascular permeability-enhancing activities.

The mediator(s) responsible for localized enhanced vascular permeability that characterizes an exacerbation of hereditary angioneurotic oedema (HAE) is thought to be a product of either contact or complement system activation. In contrast to normal individuals, plasma from these patients generates both kinin and vascular permeability-enhancing activity following incubation at 37 degrees C. Depletion of C1 inhibitor in both normal and C2-deficient plasma, but not in contact factor-deficient plasmas, resulted in generation of these activities. The kinin activity from incubated HAE plasma was sus

[Morphology of the minor salivary glands in Sjogren's disease].

Histochemical, immunohistochemical and histomorphometrical changes in sialadenitis have been studied on 33 biopsies of labial salivary glands (LSG) from patients with primary Sjogren's syndrome. The control group consisted of 10 biopsies from persons without oral cavity inflammatory diseases. A direct relation was established between xerostomia stage and the degree of LSG parenchyma substitution for the inflammatory cell infiltrate. Dependence of LSG tissue damage on local immunopathologic reaction was detected. With a highly active inflammatory reaction the changes were characterized by cellu

The rational administration of colloids.

The safe administration of i.v. fluids is one of the most significant advances in the care of critically ill patients this century. However, despite advances in the monitoring of cardiovascular variables, the questions of what? when? and how much? remain areas of enormous controversy. Ironically as the choice of i.v. fluids becomes greater and the monitoring more sophisticated the controversy grows. This article will concentrate on the diagnosis of hypovolaemia, the consequences of hypovolaemia and the rational use of i.v. fluids, particularly colloids, to treat it.

Mustard oil-induced cutaneous inflammation in the pig.

Recent findings indicate that chemical stimulation of the porcine skin with capsaicin evokes a flare response similar to that observed in man. The aim of the present study was to elucidate whether chemical stimulation of cutaneous capsaicin-sensitive nerve endings with mustard oil produces neurogenic inflammatory reactions in the pig. The application of mustard oil onto the abdominal skin of domestic pigs resulted in a pronounced flare response. After a previous intravenous injection of a solution of Evans blue, the skin area in contact with the irritant turned dark blue, indicating a marked e

Effect of smoking on pulmonary vascular permeability. A positron emission tomography study.

Positron emission tomography (PET) can be used to evaluate pulmonary vascular endothelial permeability by measuring the pulmonary transcapillary escape rate (PTCER) for radiolabeled transferrin. Because epithelial permeability, as evaluated by other techniques, is significantly affected by cigarette smoking, we used PET to compare the effects of smoking on extravascular lung density (EVD) and PTCER in seven normal chronic cigarette smokers within 30 min of smoking a cigarette and seven normal nonsmokers. We found no difference in PTCER and EVD between the two groups. We conclude that the inter
