An estimated 5.1 million Americans aged 20 years and older have heart failure.(1) With therapies ranging from medication and physical therapy to implantable defibrillators and circulatory support, and possibly transplantation, accurate risk stratification of patient with heart failure and delivery of therapies appropriate to the level of their disease severity is becoming increasingly important. Determination of risk and associated treatment strategies is the subject of this brief review.

We encountered a unique pattern of cardiac dyssynchrony in a patient with complex congenital heart disease (heterotaxy syndrome) with a biventricular physiology and systemic left ventricle (LV). On speckle tracking echocardiography, dyssynchrony was not detected within the LV, but was noted in an interventricular fashion between the LV and right ventricle (RV). BACKGROUND: QRS duration is commonly used to select heart failure patients for cardiac resynchronization therapy (CRT). However, not all patients respond to CRT, and recent data suggest that direct assessments of mechanical dyssynchrony may better predict chronic response. Left bundle branch area pacing (LBBAP) can effectively enhance cardiac contraction by engaging the conduction system. LBBAP, compared with right ventricular apex pacing, can reduce QRS duration and enhance left ventricular function. Consequently, LBBAP has been proposed as a viable alternative to cardiac resynchronization therapy (CRT).Here, we present a case illustrating the efficacy of the LBBAP in a patient with a complete atrioventricular block and complete left bundle branch block (CLBBB).

Cardiac resynchronization therapy (CRT) is an established treatment for heart failure patients with left ventricular dysfunction and a left bundle branch block. However, its impact on right ventricular (RV) function remains uncertain. This cardiac magnetic resonance imaging study found that CRT did not improve RV volumes and function, and CRT-off during follow-up had an immediate detrimental effect on the RV, which may suggest potential unfavorable RV remodeling with RV pacing during CRT.

Cardiac resynchronization therapy is a non-pharmacological treatment for patients with dilated cardiomyophaty and congestive heart failure. The success of this therapy depends of permanent biventricular stimulation. We report an 84 year-old man, with intermittent loss of biventricular pacemaker stimulation despite having adequate sensing and stimulation thresholds in the right atrium and both ventricles. The problem was solved after correcting some programming parameters.
