Postoperative Surprises: A Carcinoid Tumor Within a Perforated Meckel's Diverticulum.

Meckel's diverticulum, a true diverticulum originating from the incomplete closure of the vitelline duct during embryologic development, rarely presents with carcinoid tumors. The coexistence of a Meckel's diverticulum and carcinoid tumor following laparoscopic sigmoid colectomy for diverticulitis is an uncommon phenomenon, with limited documented cases in the literature. We present a case of a 74-year-old male with a past medical history of hypertension and diverticulitis who underwent a laparoscopic sigmoid colectomy for dysplastic and cancerous changes of a polyp revealed during a screening…

Chicken-bone perforation of a sigmoid colon diverticulum into the right groin and subsequent phlegmonous inflammation of the abdominal wall.

Colonic diverticula are frequent in western countries and approximately 80% of patients who develop diverticulitis are elderly. The sigmoid and descending colon are most commonly involved. Clinical manifestations range from asymptomatic diverticula to life-threatening complications such as diverticulum perforation and subsequent peritonitis. In a small percentage of cases ingestion of foreign bodies can cause perforation of the sigmoid diverticulum. Ingestion of foreign bodies such as chicken bones, fish bones, toothpicks and dentures usually has no consequence and only 5% of such cases requir…

Appendicovesical fistula associated with papillovillous adenoma of the appendix.

Appendicovesical fistulae represent 5% of all intestinal vesical fistulae. Usually a known cause is appendicitis, cecal diverticulitis, cystadenocarcinoma of the appendix and carcinoid tumors. We report the 2nd case, to our knowledge, where a benign tumor is a possible reason for the formation of an appendicovesical fistula.

Actinomycosis at multiple colonic sites.

A case of actinomycosis affecting multiple sites in the colon, masquerading as diverticulitis, is presented. Final diagnosis was delayed until a tuboovarian abscess developed that required resection and was found to contain sulfur granules. Bacteriology, pathophysiology, and antibiotic treatment of abdominal actinomycosis is discussed. Difficulties in diagnosis are mentioned.

Small Intestinal Diverticulosis: A Rare Cause of Intestinal Perforation Revisited.

Jejunoileal diverticulosis (JID) is a rare and nonspecific symptomatic disease. It is usually an acquired condition associated with false diverticula and integrated with colonic diverticulosis which can be diagnosed incidentally or later with complications. A sixty-nine-year-old male presented with sudden onset generalized abdominal pain. Computed tomography (CT) imaging was suggestive of ileal diverticulitis with localized perforation. The patient was treated conservatively with IV fluids and antibiotics and kept nil per orem for three days and discharged after symptoms subsided. The patient …

Meckel Diverticulum Presenting as Abdominal Pain and Subsequent Bowel Perforation.

BACKGROUND: Abdominal pain is a common presenting symptom with a broad array of potential etiologies. Meckel diverticulum (MD), the most common congenital gastrointestinal malformation, classically presents with painless gastrointestinal bleeding. However, it can also lead to diverticulitis, intussusception, or obstruction, manifesting as abdominal pain. CASE REPORT: A 2-year-old boy presented to the emergency department with intermittent abdominal pain, vomiting, and loose stools. Abdominal ultrasound findings were consistent with ileitis and ileocolic intussusception, but no such intussusce…

[A Case of a Laparoscopically Resected Gastrointestinal Stromal Tumor of the Stomach].

During treatment for sigmoid colon diverticulitis, a 72-year-old man underwent abdominal computed tomography, which revealed a gastric tumor that was approximately 40mm in diameter. He then underwent a detailed examination after the remission of diverticulitis. The gastric mucosa was found to be unremarkable on gastrointestinal endoscopy. On ultrasonic endoscopy, however, a hypoechoic mass of approximately 40mm in diameter was detected in the fourth layer of the gastric fornix and was believed to be a gastric submucosal tumor(SMT). The tumor was laparoscopically resected, and histological anal…

Pylephlebitis: a Review of 95 Cases.

Pylephlebitis, or suppurative thrombophlebitis of the portal mesenteric venous system occurring in the setting of abdominal inflammatory processes, is a rare but deadly disease commonly associated with diverticulitis. We review our institutional experience in the management of patients with this condition. A retrospective review of medical records from 2002 to 2012 was performed. Patients with a portal mesenteric vein thrombosis (PMVT) within 30 days of an intra-abdominal inflammatory process were identified and evaluated. Ninety-five patients were included. The mean patient age at presentatio…

[Diverticulosis of the small intestine].

Small bowel diverticulosis is a rare disease of gastrointestinal tract that occurs most frequently in older patients. Since this disease can be asymptomatic, for a long time, the diagnosis rarely is made in the preoperative period. In 40% of cases there are acute or chronic complications, most frequently diverticulitis either with or without perforation, bowel obstruction and massive haemorrhage clinically manifested as bleeding from the lower part of the gastrointestinal tract. Abdominal pain, pseudoobstructive and malabsorption syndrome, expressive weight loss, steatorrhea and anemia are com…

CT in the diagnosis of enterovesical fistulae.

Enterovesical fistulae are difficult to demonstrate by conventional radiographic methods. Computed tomography (CT), a sensitive, noninvasive method of documenting the presence of such fistulae, is unique in its ability to outline the extravesical component of the primary disease process. Twenty enterovesical fistulae identified by CT were caused by diverticulitis (nine), carcinoma of the rectosigmoid (two), Crohn disease (three), gynecologic tumors (two), bladder cancer (one), cecal carcinoma (one), prostatic neoplasia (one), and appendiceal abscess (one). The CT findings included intravesical…

Synchronous Diagnosis of De Novo Acute Myeloid Leukemia with inv(16)(p13q22) and Chronic Lymphocytic Leukemia: A Case Report and Review of the Literature.

The occurrence of de novo acute myeloid leukemia (AML) with chronic lymphocytic leukemia (CLL) is rare. Most cases of hematologic malignancies such as AML occurring in patients with pre-existing CLL are therapy-related. In this report, we describe a 65-year-old male with no past history of a hematolymphoid malignancy, who presented with abdominal pain. He was evaluated for acute diverticulitis, and incidentally found to have 14% circulating blasts upon peripheral blood smear review for anemia and thrombocytopenia. Bone marrow biopsy revealed 30-40% blasts and lymphoid aggregates. In conjunctio…

Genetic polymorphisms in the tumour necrosis factor locus in childhood acute lymphoblastic leukaemia.

AIM: Diverticular disease (DD) is one of the most common benign gastrointestinal conditions in the Western world, with a range of clinical manifestations from uncomplicated symptoms to acute complications requiring surgery. Elective sigmoidectomy is indicated in cases of Complicated Diverticular Disease (CDD) after recovery, Symptomatic Uncomplicated Diverticular Disease (SUDD), and Recurrent Diverticulitis (RD). This study aims to assess the short- and long-term outcomes of a standardized surgical approach for DD. METHODS: A retrospective analysis was conducted on 442 patients who underwent …

Colosplenic fistula: a highly unusual colonic fistula.

Internal fistulization of the colon to other organs, such as the urinary bladder, vagina, or small bowel is a relatively common complication associated with inflammatory diseases such as diverticulitis, Crohn disease, as well as neoplasia such as colorectal cancer or lymphoma. However, fistulization of the colon to the spleen is an exceedingly rare condition described by few in the literature.

Colonic obstruction due to sigmoid muscular hyperplasia.

We report a case of acute colonic obstruction initially presumed to be secondary to acute diverticulitis, necessitating emergent surgical intervention. Pathologic examination failed to reveal evidence of inflammation, fibrosis or neoplasia. Marked hypertrophy of the sigmoid circular muscle layer was documented and thought to be the etiology of the colonic obstruction.

[Colonothoracic fistulas: presentation of 2 cases].

Two cases of colothoracic fistulae are reported. The first one is secondary to pulmonary actinomycosis, the second to complicated colic diverticulitis. The etiologies of colothoracic fistulae are multiple and can be divided in intrathoracic or intraabdominal origin. The treatment includes: effective intravenous antibiotherapy, colic resection of the concerned portion and thoracic drainage.

[Epiploic appendagitis: report of a case].

Primary epiploic appendagitis are considered to be a rare cause of acute abdomen. They are frequently misdiagnosed as either acute appendicitis or acute diverticulitis and the diagnosis is usually made during surgery. We report a case in which computed tomography (CT) suggested the diagnosis and helped in avoiding unnecessary surgery.
