Successful treatment of cardiac dysfunction due to left main trunk obstruction and severe acute aortic regurgitation secondary to acute type A aortic dissection using Impella: a case report.

BACKGROUND: Acute type A aortic dissection (A-AAD) with severe acute aortic regurgitation (AR) and coronary involvement is a potentially fatal condition that causes left ventricular volume overload and catastrophic acute myocardial infarction. We present the successful management of a patient using Impella 5.5 following cardiopulmonary arrest caused by A-AAD with severe acute AR and left main trunk (LMT) obstruction. CASE PRESENTATION: A 50-year-old man presented with acute anterior chest pain. The patient subsequently experienced a cardiac arrest, and veno-arterial extracorporeal membrane ox

Detection of Leptospira kirschneri in a short-beaked common dolphin (Delphinus delphis delphis) stranded off the coast of southern California, USA.

BACKGROUND: Pathogenic Leptospira species are globally important zoonotic pathogens capable of infecting a wide range of host species. In marine mammals, reports of Leptospira have predominantly been in pinnipeds, with isolated reports of infections in cetaceans. CASE PRESENTATION: On 28 June 2021, a 150.5 cm long female, short-beaked common dolphin (Delphinus delphis delphis) stranded alive on the coast of southern California and subsequently died. Gross necropsy revealed multifocal cortical pallor within the reniculi of the kidney, and lymphoplasmacytic tubulointerstitial nephritis was obse

Outcomes of symptomatic penetrating aortic ulcer and intramural hematoma in the endovascular era.

BACKGROUND: Although endovascular technology has resulted in a paradigm shift in treatment, medical management remains the standard of care for penetrating aortic ulcer (PAU) and intramural hematoma (IMH). This study aimed to detail the short- and long-term outcomes of symptomatic PAU/IMH. METHODS: Institutional data on symptomatic PAU/IMH were gathered (2005-2020). The primary outcome was the composite of recurrent symptoms, radiographic progression, intervention, rupture, and death from related or unknown cause. Factors associated with the primary outcome were determined using a Fine-Gray m

Clinical characteristics and risk factors for fatal outcome of patients receiving Sun's procedure after previous cardiac surgery.

BACKGROUND: Cardiac reoperation has always been a difficult problem in clinical practice. Because of the difficulty of operation, the incidence of complications and mortality rate is high. Secondary aortic surgery, especially the reoperation involving arch, has higher risk and is more difficult for patients with renal failure. Sun's operation (total arch replacement + stent elephant nose) has achieved good results in the treatment of diseases involving aortic arch, and occupies an important position in the treatment of patients with secondary arch lesions after cardiac surgery. METHODS: A tot

Outcomes of Antegrade Stent Graft Deployment During Hybrid Aortic Arch Repair.

BACKGROUND: Complex aortic arch disease can be a formidable challenge and is often treated with a two-stage elephant trunk technique. We examined our experience with hybrid arch repair with combined zone 0 stent graft deployment. METHODS: A retrospective review was conducted of all patients who underwent type 2 hybrid arch replacement and zone 0 antegrade endovascular stent graft deployments at a single university center from June 2010 to August 2015. RESULTS: The review included 48 patients, 25 (52%) elective and 23 (48%) nonelective, with a mean ± SD age of 64 ± 11 years. Overall in-hospit

Management and outcomes of carotid artery extension of aortic dissections.

BACKGROUND: Aortic dissection (AD) is the most common aortic catastrophe. Carotid artery dissection due to extension of AD (CAEAD) is one severe complication of this condition. Despite years of refinement in the techniques for repair of AD, the optimal management strategy for CAEAD remains yet to be described. We hypothesized that CAEAD eventually resolves on antiplatelet therapy with a low but not insignificant risk of cerebrovascular accident (CVA). METHODS: This was a single-institution retrospective review of patients admitted with nontraumatic coincident aortic and carotid dissection bet

Beyond the Aortic Root: Staged Open and Endovascular Repair of Arch and Descending Aorta in Patients With Connective Tissue Disorders.

BACKGROUND: Improvements in care have prolonged survival of patients with connective tissue disorders (CTDs), but their entire native aorta remains at risk. Little data are available to guide treatment. Objectives were to characterize patients, describe repair methods, and assess outcomes. METHODS: From 1996 to 2012, 527 patients with CTDs underwent cardiovascular operations. Beyond the root, arch and descending repair was performed in 121 patients (23%) for aneurysm (n = 17), acute complicated dissection (n= 5), or chronic dissection with aneurysmal degeneration (n = 99). CTD diagnoses inclu

Sex differences in outcomes of a two-stage approach to aortic arch surgery: 8-year experience of a national aortovascular centre.

BACKGROUND: Hybrid surgery for aortic arch pathology consisting of a combined frozen elephant trunk (FET) and endovascular (TEVAR) treatment, confers a good prognosis with excellent outcomes. It is known that females, who often have higher risk profiles and more advanced disease at presentation, confer a worse prognosis post cardiac surgery compared to their male counterparts. We compare our outcomes of male and female patients after a two-stage approach. METHODS: Demographic, survival and outcome data were retrospectively analysed in patients undergoing two-staged FET and endovascular surgery

Optimizing flexible and navigable ureteral access sheaths (FANS) manoeuvrability: the impact of sheath size, working channel occupancy and deflection technique - an EAU endourology in vitro analysis.

INTRODUCTION: Conventional ureteral access sheaths are placed below the ureteropelvic junction (UPJ), whereas the novel flexible and navigable suction ureteral access sheaths (FANS) are navigated across the UPJ with its distal tip deflected into target calyx under endoscopic guidance to facilitate stone fragments suctioning. We evaluate how FANS deflection angles are affected by sheath sizes, occupancy of working channel (WC) (empty, occupied by Olympus Soltive™ Thulium Laser Fiber 150 μm and 200 μm core diameter) and deflection techniques (ureteroscope deflection versus sheath advancement) ac

Innominate artery reimplantation as a strategic step in lifetime aortic management: a vascular surgeon's perspective.

The management of complex aortic disease has evolved toward a "lifetime management" strategy, where the initial surgical arch repair critically influences the feasibility of future staged interventions. While the left subclavian artery has received significant attention, the strategic role of the innominate artery (IA) as a primary endovascular gateway remains underappreciated. This paper explores how IA reimplantation geometry determines the success of subsequent endovascular procedures. We analyzed the technical requirements for optimal antegrade endovascular access during complex downstream

Effect of peri-conception high-fat diets on maternal ovarian function, foetal and placentome growth, and vascular umbilical development in goats.

The objective of this study was to determine whether a high-fat diet (HFD) fed to goats for a brief period during peri-conception would optimize reproductive and foetal responses. Thirty-four Anglo-Nubian crossbred adult goats were allocated into three groups: control (n = 11), fed with a total mixed ration (TMR) based on chopped elephant grass and concentrate; HFBM (n = 11), given TMR supplemented with soybean oil on a 0.5% dry matter basis for 11 days starting nine days before mating (BM); and HFAM (n = 12), fed with soybean oil included in the TMR for 15 days after mating (AM). The TMR diet
