Estimates of the Health and Economic Burden of Pneumococcal Infections Attributable to the 15-Valent Pneumococcal Conjugate Vaccine Serotypes in the USA.

IntroductionDespite the availability of vaccines, pneumococcal disease (PD) is associated with high clinical and economic burden, mainly caused by non-vaccine serotypes and certain vaccine-type serotypes. V114 is a 15-valent pneumococcal conjugate vaccine (PCV) that contains two epidemiologically important serotypes, 22F and 33F, in addition to the 13 serotypes in 13-valent PCV (PCV13). This study quantified the epidemiologic and economic burden of PD attributable to V114 serotypes among adults in the USA.MethodsA Markov model was used to estimate the burden of V114 serotypes in a hypothetical

Aging and postural hypotension. An update.

Postural hypotension is a common clinical problem in elderly patients. Reported studies have investigated the epidemiology and mechanisms of this disorder. However, many of these data are based on subjects with diseases and medications also known to cause this disorder, which confuses the relative contributions of normal aging, disease, and medication effects. This review critically evaluates published studies on measurement, epidemiology, and mechanisms of postural hypotension in screened and unscreened elderly populations. Pertinent age-related changes in physiology and the relationship of p

The Influence of Epidemiologic Context on the Success of Partner Notification Programs: Analysis of Gonorrhea Transmission Dynamics.

BACKGROUND: Limited estimates exist on the population-level impact of partner notification (PN) for gonorrhea, with uncertainty in the influence of local epidemiology on PN effectiveness. An ecologic study in New York found a 6% reduction in diagnoses with a 10% increase in PN coverage. We estimated gonorrhea incidence reductions via PN across different epidemiologic conditions to compare the effects with the prior finding and to understand key determinants of variation. METHODS: We developed a stochastic network model of men who have sex with men and calibrated gonorrhea transmission dynamic

Geographic transmission hubs of the 2009 influenza pandemic in the United States.

A key issue in infectious disease epidemiology is to identify and predict geographic sites of epidemic establishment that contribute to onward spread, especially in the context of invasion waves of emerging pathogens. Conventional wisdom suggests that these sites are likely to be in densely-populated, well-connected areas. For pandemic influenza, however, epidemiological data have not been available at a fine enough geographic resolution to test this assumption. Here, we make use of fine-scale influenza-like illness incidence data derived from electronic medical claims records gathered from 83

Associations of prenatal exposure to bisphenols with infant anthropometry: A prospective cohort study.

BACKGROUND: Bisphenols (BPs) have been shown to exhibit developmental toxicities. Epidemiological evidence on prenatal BPs exposure and infant growth primarily confined scopes to specific BPs and birth outcomes, with few studies focusing on infant growth and reporting inconsistent findings. The joint effect of prenatal exposure to BPs mixture on infant growth was rarely studied. OBJECTIVE: This study examined associations of prenatal exposure to individual bisphenol A (BPA) and its analogues (bisphenol F [BPF], bisphenol S [BPS], bisphenol AF [BPAF], and tetrachlorobisphenol A [TCBPA]) and th

A mathematical modelling framework for linked within-host and between-host dynamics for infections with free-living pathogens in the environment.

In this study we develop a mathematical modelling framework for linking the within-host and between-host dynamics of infections with free-living pathogens in the environment. The resulting linked models are sometimes called immuno-epidemiological models. However, there is still no generalised framework for linking the within-host and between-host dynamics of infectious diseases. Furthermore, for infections with free-living pathogens in the environment, there is an additional stumbling block in that there is a gap in knowledge on how environmental factors (through water, air, soil, food, fomite…

Potential distributions of phlebotomine sandfly vectors of human visceral leishmaniasis caused by Leishmania infantum in Morocco.

Zoonotic visceral leishmaniasis is a common vector-borne systemic disease caused by Leishmania infantum (Kinetoplastida: Trypanosomatidae). In Morocco the situation is complex: many sandfly species have been collected in areas in which the disease is endemic, but only Phlebotomus ariasi, Phlebotomus perniciosus and Phlebotomus longicuspis (Diptera: Psychodidae) have been confirmed to have vectorial roles. The objective of the present study was to ascertain the potential distribution of L. infantum and its vectors in Morocco, using ecological niche modelling. Vector records were obtained from f

Epidemiological, clinical, and diagnostic characteristics of a large-scale upsurge of dengue in the rohingya refugee camps and host communities in Cox's Bazar, Bangladesh, 2021 to 2024: A retrospective study.

BACKGROUND: Dengue emerged as a new public health threat in the Rohingya refugee camps in Cox's Bazar, Bangladesh, in 2021 and expanded into large-scale upsurges in subsequent years. Evidence on dengue epidemiology and clinical presentation in protracted refugee settings remains limited, despite the need for stronger outbreak preparedness and case management in these contexts. OBJECTIVES: To describe the epidemiological, clinical, and diagnostic characteristics of the dengue upsurge among Rohingya refugees and surrounding host communities in Cox's Bazar, Bangladesh, and to identify predictors

Rate variation and recurrent sequence errors in pandemic-scale phylogenetics.

Phylogenetic analyses of genome sequences from infectious pathogens reveal essential information regarding their evolution and transmission, as seen during the coronavirus disease 2019 pandemic. Recently developed pandemic-scale phylogenetic inference methods reduce the computational demand of phylogenetic reconstruction from genomic epidemiological datasets, allowing the analysis of millions of closely related genomes. However, widespread homoplasies, due to recurrent mutations and sequence errors, cause phylogenetic uncertainty and biases. We present algorithms and models to substantially im

Response to "Colorectal cancer in Saudi Arabia: Turning epidemiological transition into prevention-oriented action".

We thank Dr. Hu for the thoughtful comments on our study examining the burden and projected trends of colorectal cancer (CRC) in Saudi Arabia. In this response, we clarify that the increasing absolute CRC burden largely reflects population growth, aging, improved case detection, and enhanced survival rather than a worsening underlying disease risk. We further discuss the narrowing sex disparity in CRC incidence, emphasizing the need for additional research to elucidate sex-specific risk factors and screening uptake. We acknowledge the limitations of trend-based forecasting models and support t

Multitask deep learning for the emulation and calibration of an agent-based malaria transmission model.

Agent-based models of malaria transmission are useful tools for understanding disease dynamics and planning interventions, but they can be computationally intensive to calibrate. We present a multitask deep learning approach for emulating and calibrating a complex agent-based model of malaria transmission. Our neural network emulator was trained on a large suite of simulations from the EMOD malaria model, an agent-based model of malaria transmission dynamics, capturing relationships between immunological parameters and epidemiological outcomes such as age-stratified incidence and prevalence ac

Impact of carbon nanotubes on pulmonary disorders attributed to occupational and environmental exposures.

With widespread use of carbon nanotubes (CNTs) in manufacturing, the public is increasingly exposed to these materials being released into the environment, with concerns of potential adverse effects on respiratory health. Studies have demonstrated that exposure to CNTs initiates inflammatory cascades and oxidative stress. CNT inhalation challenge in rodents often produces granulomatous inflammation and lung fibrosis. CNT exposure causes TH2 asthmatic inflammation in animal models. CNTs are implicated in disrupting the delicate balance of extracellular matrix homeostasis, contributing to fibrot

Is Sexual Transmission of Chagas Disease Possible? Evaluating the Evidence and Future Directions.

Trypanosoma cruzi is the etiological agent of Chagas disease, traditionally transmitted by triatomine vectors. However, experimental and clinical evidence suggest a possibility that the parasite could also be transmitted sexually. In animal models, T. cruzi amastigotes and trypomastigotes have been identified in reproductive tissues and semen, and infection has been experimentally transmitted between mating partners. In humans, the presence of the parasite in menstrual blood, semen, and other genital secretions has been reported, raising the possibility of sexual transmission in both endemic a

Social Behavior in Animal Models of Autism Spectrum Disorder.

Autism spectrum disorder (ASD) is a complex neurodevelopmental disorder characterized by social deficits and stereotyped, repetitive patterns of behaviors, limited interests, and cognitive impairment. Especially, social deficit has been considered a core feature of ASD. Because of the limitations of the experimental approach in humans, valid animal models are essential in an effort to identify novel therapeutics for social deficits in ASD. The genetic and environmental factors are clinically relevant to the pathophysiology of ASD. Epidemiological studies demonstrate environmental interventions

Obesity and myeloma: Clinical and mechanistic contributions to disease progression.

Obesity and obesogenic behaviors are positively associated with both monoclonal gammopathy of unknown significance (MGUS) and multiple myeloma (MM). As the only known modifiable risk factor, this association has emerged as a new potential target for MM prevention, but little is known about the mechanistic relationship of body weight with MM progression. Here we summarize epidemiological correlations between weight, body composition, and the various stages of myeloma disease progression and treatments, as well as the current understanding of the molecular contributions of obesity-induced change

Introduction to Preclinical Evidence from Animal Models of Endometriosis.

Endometriosis, the presence and growth of uterine endometrial glandular epithelial and stroma cells outside the uterine cavity, causes pain and infertility in women and girls of reproductive age. As randomized, double-blinded, controlled studies of endometriosis in women are impractical and at times ethically prohibitive, animal models for endometriosis arose as an important adjunct to gain mechanistic insights into the etiology and pathophysiological mechanisms of this perplexing disorder. A more thorough understanding of endometriosis in women may help develop novel noninvasive diagnostics,

BCG-associated heterologous immunity, a historical perspective: intervention studies in animal models of infectious diseases.

The WHO Special Advisory Group of Experts (SAGE) review of the available epidemiological and trial evidence in humans concluded that bacillus Calmette-Guérin (BCG) vaccination leads to beneficial heterologous ('non-specific') effects, specifically on all-cause mortality. Randomized controlled trials showing this beneficial effect suggest improved survival is the result of enhanced protection against infection. This paper reviews the available evidence for the attenuating effects of BCG vaccine on experimental infections in animal models, including protection from bacteria, viruses, parasites a

Hypoxia at the heart of sudden infant death syndrome?

Sudden infant death syndrome (SIDS) is a significant clinical problem without an accepted pathological mechanism, but with multiple conflicting models. Mutations in a growing number of genes have been found postmortem in SIDS cases, notably genes encoding ion channels. This can only account for a minority of cases, however. Our recent work on a novel mouse model of SIDS suggests a potentially more widespread role for cardiac arrhythmia in SIDS without needing to invoke the inheritance of abnormal ion-channel genes. We propose a model for SIDS pathogenesis whereby postnatal hypoxia leads to del

From hepatic diseases and jaundice to viral hepatitis: the configuration of a kaleidoscope.

Viral hepatitis A, B, C, D and E--systemic hepatotropic viral infections--present as acute hepatitis that, depending on the etiological agent, viral load and host conditions, may evolve into chronic hepatitis, cirrhosis, liver cancer and acute fulminant disease. The ecological versatility of these viruses, their spectrum of transmission in time and space, potentialized by the sub-clinical course of a large proportion of infections, comprise an epidemiological challenge. This essay describes scenarios and tendencies in the socioepidemiologic profile, based on the history of these infections, an
