BACKGROUND: In patients with chronic respiratory failure (CRF) nocturnal mechanical ventilation (NMV) confers increased exercise tolerance. The hypothesis tested in the present study was that the increased exercise performance is associated with increased quadriceps strength.
METHODS: In 28 patients with CRF due to chronic obstructive pulmonary disease and restrictive thoracic disease (post-tuberculosis-sequelae, scoliosis and obesity-hypoventilation) NMV was started. Before and after 2-month NMV the exercise tests, namely shuttle and 6-min walking distance, were performed. Furthermore, quadr…

We report a 10-year-old boy with cholinergic urticaria associated with epileptic seizure and abnormalities on electroencephalogram. In March 1999, many red wheals developed over the entire body during exercise and at increased body temperature. In April, systemic red wheals developed during exercise. Simultaneously, loss of consciousness was noted for 2 to 3 minutes. After the patient's body was cooled, consciousness improved, and these exanthemas disappeared. In the Department of Dermatology, his illness was diagnosed as cholinergic urticaria. Neurologically, a heat tolerance and mental strai

INTRODUCTION: Although Chronic Obstructive Pulmonary Disease (COPD) is generally considered a progressive condition with limited reversibility, recent data suggests potential for lung function improvement with exercise. Mechanistic insights into exercise-induced benefits remain limited, but in vitro models offer promise for understanding cellular and molecular changes. This study aims to elucidate the mechanisms behind lung tissue regeneration in COPD through adapted exercise training as well as validating advanced lung function tests, such as Impulse Oscillometry (IOS) and Airspace Dimension

INTRODUCTION: Cardiac rehabilitation (CR) improves cardiovascular health and quality of life after coronary heart disease (CHD). However, it remains uncertain whether patients with diabetes achieve similar gains in exercise capacity as those without diabetes.
METHODS: This cohort study included patients discharged from hospital following CHD between February 2019 and December 2022 who completed a 12-week CR programme at 18 community health centres. Exercise capacity was assessed using a symptom-limited bicycle ergometer test and reported as both absolute (mL O₂/min) and relative VO₂peak (mL O

INTRODUCTION: Cirrhosis is the main cause of morbidity and mortality globally, accounting for approximately 1.2 million deaths annually. Impaired aerobic capacity, muscle wasting and reduced muscle strength are significant complications in patients with cirrhosis. Preoperative exercise intervention "prehabilitation" has been recognised as a potential approach to optimise muscle strength, aerobic capacity and body composition as well as quality of life in patients awaiting abdominal surgery. However, there is little evidence on the effects of preoperative exercise on older adults with cirrhosis

OBJECTIVES: To compare cardiorespiratory fitness (CRF) expressed as maximal oxygen uptake (VO2max) between patients with long-term JDM and controls and between patients with active and inactive disease, as well as to explore exercise limiting factors and associations between CRF and disease variables.
METHODS: JDM patients (n = 45) and age- and gender-matched controls (n = 45) performed a cardiopulmonary exercise test (CPET) on a treadmill until exhaustion. Physical activity was measured by accelerometers. Disease activity, damage and muscle strength/function were assessed by validated tools.

OBJECTIVE: To 1) report the feasibility of an "all-out" 30-second cycling exercise test (Wingate Anaerobic Exercise Test [WAnT]) in juvenile-onset idiopathic inflammatory myopathy (JIIM) patients, 2) describe the anaerobic exercise capacity in juvenile dermatomyositis patients, and 3) determine if the anaerobic exercise capacity could be related to disease duration or disease phase.
METHODS: Twenty patients (age 14.13 +/- 5.4 years) with JIIM participated in this study. All patients were able to perform the WAnT without adverse events.
RESULTS: Comparison with healthy controls revealed a -29

OBJECTIVE: The purpose of this study was to compare the effects on exertional dyspnea and exercise capacity of square wave (bilevel) cycle e … walk test, which increased by 7% (P < 0.05).
CONCLUSIONS: This study suggests that the decline in exercise tolerance for male smokers can be reduced by intensive exercise training (SWEET) and comprehensive directed breathing but not by moderate training and traditional diaphragmatic breathing. Thus, some smokers can be helped despite continuing to smoke.

INTRODUCTION: Breathlessness limits exercise training intensity in people with chronic lung disease (CLD). Stimulation of the trigeminal nerve via fan-to-face (F2F) therapy (facial airflow) can reduce exertional breathlessness and improve exercise endurance in CLD. This randomised controlled trial tested the hypothesis that adding F2F therapy to an exercise training programme could enhance the benefits of exercise training on exercise endurance time (EET) and exertional breathlessness in adults with CLD by allowing them to train at higher intensities.
METHODS: 23 participants with COPD (n=19)

Cardiac rehabilitation (CR) improves exercise capacity, but frequent cardiopulmonary exercise testing (CPET) is impractical. The AESCULON mini enables non-invasive hemodynamic monitoring, though its role in CR remains unclear. Eleven patients (6 myocardial infarction, 3 angina pectoris, 2 dilated cardiomyopathy) undergoing outpatient CR at the University of Tokyo Hospital were studied. Hemodynamics were measured using the AESCULON mini before and after 20 min of aerobic exercise at the anaerobic threshold. CPET and brain natriuretic peptide (BNP) were assessed within 2 weeks. Stroke volume, ca

In left heart failure, iron supplementation (IS) is a first-line treatment option, regardless of anemia. Pulmonary arterial hypertension (PAH), a rare disease leading to right heart failure, is also associated with iron deficiency. While it is a much debated topic, recent evidence demonstrate that restoration of iron stores results in improved right ventricular function and exercise tolerance. Hence, IS may also be considered as an option in the treatment of PAH.

A spayed female dog was evaluated because of edema of the ventral cervical region, lethargy, cough, and reduced exercise tolerance. Invasive thymoma and cranial vena cava syndrome were diagnosed by use of ultrasound-guided fine-needle biopsy and contrast-enhanced helical computed tomography. Resection of the cranial vena cava and an autogenous jugular vein graft were used for restoration of normal venous return to the right atrium and alleviation of the cranial vena cava syndrome.

A cyanotic, debilitated 66-year-old man was referred as an emergency because of embolic arterial occlusion of the left lower extremity. Subsequent investigation revealed total anomalous pulmonary venous connection. Complete surgical correction was accomplished. Despite a difficult postoperative hospital course, more than 3 years after operation he continues to experience improved quality of life with better exercise tolerance, freedom from further emboli and symptomatic arrhythmias, and no need for his previously routine phlebotomies.

A 35-year-old patient who presented with recurrent chest infection, pulmonary oedema and cardiac failure was found to be grossly hypocalcaemic owing to previously undiagnosed hypoparathyroidism. The cardiac failure was not easily relieved by digoxin and diuretics but it quickly responded when the plasma calcium was restored to normal with dihydrotachysterol. With dihydrotachysterol as sole treatment for more than 2.5 years he had normal exercise tolerance and no features of cardiac failure.

Short-term studies with an artificial endocrine pancreas have provided insights into the pathophysiology of diabetes and demonstrated the ability of such a system to restore and maintain glucose homeostasis in human subjects given either their usual diet, a 50 gm oral glucose tolerance test, or a moderate level of physical exercise. Animals were used to demonstrate that a peripheral route of insulin delivery by the artificial pancreas is equivalent to the portal route.

OBJECTIVE: Non-alcoholic fatty liver disease (NAFLD) covers a wide spectrum of liver pathology ranging from simple fatty liver to non-alcoholic steatohepatitis (NASH). Notably, immune cell-driven inflammation is a key mechanism in the transition from fatty liver to the more serious NASH. Although exercise training is effective in ameliorating obesity-related diseases, the underlying mechanisms of the beneficial effects of exercise remain unclear. It is unknown whether there is an optimal modality and intensity of exercise to treat NAFLD. The objective of this study was to determine whether hig

Exercise-induced left bundle branch block (EI-LBBB) is infrequent phenomenon. We present two patients with angina pectoris who developed EI-LBBB during exercise tolerance test. The first patient with typical angina pectoris had significant obstructive coronary artery disease (CAD) requiring percutaneous coronary intervention of multiple lesions including placement of drug eluting stents. The second patient had atypical chest pain without signs of CAD at all. EI-LBBB occurred at a heart rate of 80 bpm and 141 bpm in the first and second patient, respectively. EI-LBBB remained visible through th…

It is thought that mitral valve prolapse is the most commonly diagnosed valvular heart disease and affects 3 to 5% of the community. Mitral valve prolapse is commonly a benign disease and the prognosis is good. But it is understood that mitral valve prolapse causes sudden death rarely. As etiologies of their sudden deaths ventricular arrhythmia, mitral valve regurgitation et al. are known. Mitral valve prolapse is brought as the reason of sudden deaths in athletes not a little. Therefore, it is important to follow up them by exercise tolerance test, Holter ECG, ultrasoniccardiography, and ches…
