PURPOSE: Current ASE/EACVI echocardiography guidelines recommend estimating central venous pressure from inferior vena cava (IVC) diameter and collapsibility in biventricular circulations. Because altered blood volume, venous capacitance, and venous tone may weaken the relationship between IVC morphology and systemic venous pressure, it is uncertain whether these criteria apply in the Fontan circulation.

Fontan conduit stenosis can impair haemodynamics, necessitating stent implantation. We present a 16-year-old patient who developed platypnea-orthodeoxia syndrome due to a post-stenting baffle leak. Despite initial diagnostic challenges, a second catheterisation confirmed and successfully sealed the defect with a covered stent. This case highlights the need for close monitoring and prompt intervention to manage complications in Fontan patients.

Improved survival rates for patients with a Fontan circulation has allowed more women with this complex cardiac physiology to contemplate pregnancy. However, pregnancy in women with a Fontan circulation is associated with a high risk of adverse maternal and foetal outcomes, high rates of miscarriage, and preterm delivery. A 2-year-old girl underwent the Fontan operation for aortic valve stenosis, mitral valve stenosis, a hypo-plastic left ventricle, and a non-compacted right ventricle. The patient's cardiac function reduced gradually thereafter, mainly due to systemic ventricular dysfunction. A Berlin Heart EXCOR (BHE) ventricular assist device with a 10-mL pump was implanted at 4 years of age. After 465 days, BHE support without major complications, the patient underwent heart transplantation. A safe and long-term BHE support was achieved with proper case selection and repeated examinations.
