[Burden of bureaucracy in surgery : Results of a national survey].

BACKGROUND: Bureaucratic tasks in everyday surgical clinical practice are time-consuming. Due to the increasing shortage of skilled workers and the growing need for care, it is necessary to relieve surgeons of administrative tasks. OBJECTIVE: The analysis aims to make the nature and extent of administrative tasks transparent in order to derive options for action. MATERIAL AND METHODS: Data were collected as a survey using a digital questionnaire with 29 questions. RESULTS: 1632 BDC members participated in the survey with a response rate of 19%. The survey results illustrate that surgeons ma

Nobel Prizes and surgery.

The Nobel Prize is an international recognition awarded to individuals who have made significant contributions to humanity. The areas that are recognized are physics, chemistry, physiology or medicine, literature, and peace. With more than 100 years of existence, only 10 surgeons have been recipients of this recognition. Their contributions range from cardiac catheterization, the discovery of insulin to the beginning of the modern era of organ transplantation. These surgeons through their research have greatly contributed to modern medicine, saving, and improving quality of life of millions of

Surgical Training for a Disaster: Preparation of Surgical Trainees for Victims of Conflict.

INTRODUCTION: With increasing global unrest and military physician shortages potentially leading to a surgeon draft, we sought to evaluate the readiness of graduating general surgery residents to care for casualties of war. MATERIALS AND METHODS: We evaluated the National Data Reports of Surgery Case Logs for general surgery residents from 2009 to 2018 to quantify experience with key procedures that provide critical skills required for wartime surgery. Reported cases from the Accreditation Council for Graduate Medical Education for graduating residents from civilian and military residency pro…

Thyroid Surgery: Whose Domain Is It?

In the United States of America (US), most departments of otorhinolaryngology head and neck surgery have been performing thyroid surgery for many years. In contrast to the US, thyroid surgery is still dominated by general surgeons in most European countries. In numerous university centers, there continues to be friction regarding thyroid surgery. The focus of this editorial is to demonstrate that there is objective data in the literature to suggest that otorhinolaryngologists with appropriate training in head and neck surgery are well suited to perform the entire spectrum of thyroid surgery. Internist, anesthesiologist and surgeon use of ketogenic diet.

Ketogenic diet is being increasingly utilized in recent decades because of its success as an effective tool for short and intermediate-term weight loss. Promoting physiological ketosis from a drastically low carbohydrate diet is the fundamental basis for this diet regime. Though debated, these diets have been demonstrated to be effective, at least in the short- to medium terms, to manage excess weight, hyperlipidemia, and other cardiovascular risk factors. We reviewed the cardiovascular, metabolic, anesthetic, and postsurgical profiles in the literature and summarized technical issues of anest

New Trends at 25 Years: An American Board of Surgery Academic Certifying Examination Review Course.

BACKGROUND: In the 1980s, a small research group began identifying variables affecting applicant success on the American Board of Surgery (ABS) Certifying Examination (CE). We now report success and trends as we complete 25 years. We had multiple challenges as identified through faculty focus groups and participant feedback that needed to be addressed: increase the national optics of the program, integrate new innovative experiences, maintain the integrity of the collected data on excel files, incorporate national trends in surgery, attract experienced clinical volunteer faculty and staff, sec

Laparoscopic common bile duct exploration four-task training model: construct validity.

BACKGROUND: Training models in laparoscopic surgery allow the surgical team to practice procedures in a safe environment. We have proposed the use of a 4-task, low-cost inert model to practice critical steps of laparoscopic common bile duct exploration. METHODS: The performance of 3 groups with different levels of expertise in laparoscopic surgery, novices (A), intermediates (B), and experts (C), was evaluated using a low-cost inert model in the following tasks: (1) intraoperative cholangiography catheter insertion, (2) transcystic exploration, (3) T-tube placement, and (4) choledochoscope ma

Ninety and still active in research.

BACKGROUND: Here, we review Prof. Stelzner's career and scientific achievements, which include his personal attitude to genuine creative thinking and its application to surgery and research. DISCUSSION: The stations of his clinical career are highlighted together with his excursions into philosophical and historical topics. From the vast number of publications and research activities, we describe major contributions to morphology including recent contributions through functional examination techniques. We reflect on his research on perianal fistulas. Modern-day understanding of continence of

Clinical features and treatment of alveolar echinococcosis.

PURPOSE OF REVIEW: Human alveolar echinococcosis is caused by the larval stage of Echinococcus multilocularis, occurring in at least 42 countries of the northern hemisphere. Recent studies in Europe and Asia have shown that the endemic area of E. multilocularis is larger than previously known and the parasite has regionally expanded from rural to urban areas. Diagnosis of alveolar echinococcosis is supported by results from imaging studies, histopathology and/or nucleic acid detection, and serology. The present review summarizes current understanding of clinical features, knowledge on appropri

Interobserver variation in the assessment of appendiceal perforation.

BACKGROUND: Following an appendectomy, surgeons define appendicitis, for treatment and billing purposes, into one of four categories: normal … nt of defining an image as to whether it was perforated or not was 27% (R4 = 0.27). Completion of a general surgery residency did improve the interobserver agreement, when compared with trainees. CONCLUSION: There is poor agreement among surgeons in describing the severity of appendicitis. Treatment protocols based on more accurate assessment and categorization …

Neuromonitoring in the operating room: why, when, and how to monitor?

This review considers the main principles and indications of EEG and evoked potential (EP) neuromonitoring in the operating room. Neuromonitoring has a threefold purpose: to warn the surgeon that he has to adjust his strategy, to confirm his decision, and to help him improve subsequent procedures. The pathophysiology of intraoperative events liable to alter the EEG or the EPs is first considered. The usefulness of neuromonitoring in preventing neurological complication relies on its ability to detect neurological dysfunction at a reversible stage. This applies especially to ischemia and compre

My early experience with Hirschsprung's disease.

Prior to 1948, when the first distal colonic resection with preservation of continence was performed for Hirschsprung's disease, there was no clear understanding of the etiology of the disease. Early explanations for the cause of the disease included mechanical obstruction, "neurologic imbalance," and congenital malformation of the entire colon. No medical or surgical treatment had proved successful in treating the disease. It is generally assumed today that the absence of ganglion cells was widely known and accepted as the cause of Hirschsprung's disease prior to 1948 and that this led to the

The life and time of Guillaume Dupuytren.

Guillaume Dupuytren's good fortune was the time of his birth. He was a product of revolutionary change in France, and without it he would never have become a surgeon. From south central France he came to Paris to become a surgeon and reached his goal, literally, "on a crust of bread". Dupuytren is remembered principally for the hand condition which bears his name; his other numerous and more-important contributions have been ignored. Never happy outside the operating room--or perhaps inside it either, due to his mania for perfection--this arrogant master surgeon constantly clashed with his col
