Diurnal and nocturnal cardiovascular variability and heart rate arousal response in idiopathic hypersomnia.

OBJECTIVES: Autonomic nervous system dysfunction has been described in narcolepsy with cataplexy affecting sympathetic functions. In this study we analyzed whether altered diurnal and nocturnal cardiovascular control is present in idiopathic hypersomnia (IH). METHODS: Fourteen drug-free patients aged 26.2 ± 7 years and 14 age-matched controls were examined. Clinical data, 24-h polysomnography, heart rate (HR) variability, and the HR response to spontaneous arousal were available. RESULTS: Sleep macrostructure was comparable between controls and patients, with the latter having significantly …

Heart Rate Variability in Adolescents - Normative Data Stratified by Sex and Physical Activity.

BACKGROUND: Heart rate variability (HRV) refers to the beat-to-beat fluctuations in the cardiac rhythm occurring due to modulation of the pacemaker (sinoatrial node) activity of the heart by the sympathetic and parasympathetic branches of the autonomic nervous system. Nowadays, cardiovascular diseases and their risk factors are increasingly occurring at a younger age (children and adolescents) and recording of HRV in them will help us to identify cardiovascular autonomic derangement earlier. However, to be used clinically, normative data has to be established in this age group considering othe…

Impact of type of intervention for aortic valve replacement on heart rate variability.

BACKGROUND: It is known that coronary heart surgery leads to varying degrees of cardiac autonomic derangement, clinically detectable as depression of heart rate variability (HRV) parameters. Few studies report that also surgical replacement of the aortic valve (SAVR) may lead to HRV abnormalities, while very little is known about the autonomic effects obtained after less invasive aortic valve replacement techniques. The study aimed to evaluate HRV after SAVR and to compare it with two less invasive techniques, transapical (TaAVI) and tranfemoral (TfAVI) aortic valve implant. METHODS: Time-dom…

Autonomic modulations of heart rate variability and performances in short-distance elite swimmers.

PURPOSE: Endurance exercise is associated with high cardiac vagal tone, but how the cardiac autonomic control correlates with shorter anaerobic performances is unknown. Therefore, the aim of this study was to evaluate how autonomic modulations of heart rate (HR) variability (V) correlate with performances of short- (<1 min) and very short (<30 s) duration in elite athletes. METHOD: Thirteen male swimmers, national-level crawl specialists in short (100-m) and very short (50-m) distances, were enrolled. HR was recorded during 15-min supine rest: (1) in the morning after wake up, (2) in the afte…

Heart rate variability in unmedicated patients with bipolar disorder in the manic phase.

AIMS: Decreased heart rate variability (HRV) has been proposed in bipolar disorder. To date, there has been no adequate study that has investigated resting HRV in unmedicated patients with bipolar disorder in the manic state. METHODS: To examine whether bipolar mania is associated with decreased HRV, 61 unmedicated patients with bipolar mania and 183 healthy volunteers aged 20-65 years were recruited for this case-control analysis. The Young Mania Rating Scale (YMRS), Clinical Global Impression-Severity, Hamilton Depression Rating Scale (HAM-D), and Hamilton Anxiety Rating Scale (HAM-A) were …

Heart rate significantly influences the relationship between atrial fibrillation and ankle-brachial index.

OBJECTIVES: Both atrial fibrillation (AF) and vascular disease share several risk factors and the two diseases often coexist. The patients with AF were reported to have a decreased ankle-brachial index (ABI). However, ABI was also reported to have an inverse relationship with heart rate (HR). Because AF patients often have a transiently or persistently rapid HR, this study aimed to assess whether AF was significantly associated with decreased ABI and whether HR could significantly influence the relationship between AF and ABI. METHODS: We included 166 AF and 1336 non-AF patients from subjects…

Effect of icodextrin on heart rate variability in diabetic patients on peritoneal dialysis.

INTRODUCTION: Spectral analysis of heart rate variability is a noninvasive method for evaluating autonomic cardiovascular dysfunction under various clinical conditions, such as in dialysis patients, in whom an imbalance between the sympathetic and parasympathetic nervous system appears to be an important risk factor for sudden cardiovascular death and arrhythmia. ♢ OBJECTIVE: We compared the effect of icodextrin-based dialysis solution, an option that allows for better metabolic and fluid overload control, with that of glucose-based dialysis fluid on sympathetic and parasympathetic activity i…

Plasma FGF23 levels and heart rate variability in patients with stage 5 CKD.

UNLABELLED: Fibroblast growth factor 23(FGF23) is a bone-derived hormone which regulates mineral homeostasis but may also have a role in cardiovascular disease. Here, we found that higher plasma FGF23 was independently associated with decreased heart rate variability in stage 5 CKD patients and parathyroidectomy may reverse these abnormal indicators. INTRODUCTION: Lower heart rate variability (HRV) in patients with chronic kidney disease (CKD) compared with healthy controls is associated with increased risk of cardiovascular disease (CVD). Higher levels of plasma FGF23 also predict higher ris…

Attenuated heart rate response is associated with hypocretin deficiency in patients with narcolepsy.

STUDY OBJECTIVE: Several studies have suggested that hypocretin-1 may influence the cerebral control of the cardiovascular system. We analyzed whether hypocretin-1 deficiency in narcolepsy patients may result in a reduced heart rate response. DESIGN: We analyzed the heart rate response during various sleep stages from a 1-night polysomnography in patients with narcolepsy and healthy controls. The narcolepsy group was subdivided by the presence of +/- cataplexy and +/- hypocretin-1 deficiency. SETTING: Sleep laboratory studies conducted from 2001-2011. PARTICIPANTS: In total 67 narcolepsy pa…

Ablation of swallowing-induced atrial tachycardia affects heart rate variability: a case report.

A 47-year-old man underwent slow pathway ablation for slow-fast atrioventricular nodal reentrant tachycardia. Following the procedure, he felt palpitations while swallowing, and swallowing-induced atrial tachycardia was diagnosed. Swallowing-induced atrial tachycardia arose from the right atrium-superior vena cava junction and was cured by catheter ablation. After the procedure, the patient's heart rate variability changed significantly, indicating suppression of parasympathetic nerve activity. In this case, swallowing-induced atrial tachycardia was related to the vagal nerve reflex. Analysis …

Smartphone applications (apps) for heart rate measurement in children: comparison with electrocardiography monitor.

Heart rate (HR) measurement is essential for children with abnormal heart beats. The purpose of this study was to determine whether HR measurement by smartphone applications (apps) could be a feasible alternative to an electrocardiography (ECG) monitor. A total of 40 children, median age of 4.3 years, were studied. Using four free smartphone apps, pulse rates were measured at the finger (or toe) and earlobe, and compared with baseline HRs measured by ECG monitors. Significant correlations between measured pulse rates and baseline HRs were found. Both correlation and accuracy rate were higher i…
