Catheters: design, selection and management.

Urinary catheterization is a common procedure that is performed on 12% of hospital patients (Crow et al, 1996) and 4% of community patients (Getliffe, 1990). When considering catheterization as a method of care, many decisions need to be made in order to select the optimum equipment and provide effective catheter care. This includes selecting the type of catheterization that needs to be performed, i.e. intermittent or indwelling, and choosing the right catheter material, size and balloon infill volume. The type and design features of the urinary drainage and suspensory system have a direct eff

Radiographic appearance of intrathoracic complications of pregnancy.

The authors reviewed all chest radiographs obtained for pregnant women at a university hospital over a 15-year period to determine the intrathoracic complications of pregnancy and diseases occurring during pregnancy. The characteristic physiologic changes seen on chest radiographs during normal pregnancy are reviewed. Examples of intrathoracic diseases that may occur in pregnant patients include pulmonary embolism, amniotic fluid embolism, beriberi, aspiration pneumonia, community-acquired pneumonia, viral pneumonia, asthma, systemic disease, trophoblastic disease and peripartum pulmonary edem

Operative stewardship: reclaiming the role of surgical source control in dental antimicrobial stewardship.

Dentistry accounts for approximately 10% of all antibiotic prescriptions globally, with evidence consistently demonstrating that the majority of these prescriptions are inappropriate. Current dental antimicrobial stewardship programs are defined almost exclusively around prescribing behavior-optimizing antibiotic choice, dose, and duration. This pharmacological framing, while necessary, is structurally incomplete. It overlooks the primary clinical intervention that renders antibiotics unnecessary in the first place: surgical source control through incision and drainage or tooth extraction. In

Reprocessing of loaned surgical instruments/implants in Australia and Brazil: A survey of those at the coalface.

BACKGROUND: Acquisition of surgical instruments (SI) and implants by loaner system is a worldwide practice. Although quality of device reprocessing has been reported to be lower in low and middle-income countries compared with higher income countries, few investigations have been conducted on the management and reprocessing of loaned devices. Thus, in this study we evaluate the practices of management and reprocessing of loaner SI and non-sterile implants (NSI) in a high (Australia) and a middle-income (Brazil) country. METHODS: Cross-sectional surveys (hardcopy or online form) was self-admin

Assessing the Surveillance System for Priority Zoonotic Diseases in the Democratic Republic of the Congo, 2017.

High-functioning communicable disease surveillance systems are critical for public health preparedness. Countries that cannot quickly detect and contain diseases are a risk to the global community. The ability of all countries to comply with the International Health Regulations is paramount for global health security. Zoonotic diseases can be particularly dangerous for humans. We conducted a surveillance system assessment of institutional and individual capacity in Kinshasa and Haut Katanga provinces in the Democratic Republic of the Congo for nationally identified priority zoonotic diseases (

Clarithromycin for early anti-inflammatory responses in community-acquired pneumonia in Greece (ACCESS): a randomised, double-blind, placebo-controlled trial.

BACKGROUND: Addition of macrolide antibiotics to β-lactam antibiotics for the treatment of patients in hospital with community-acquired pneumonia is based on results from observational studies and meta-analyses rather than randomised clinical trials. We investigated if addition of the macrolide clarithromycin to treatment with a β-lactam antibiotic in this population could improve early clinical response-the new regulatory endpoint for community-acquired pneumonia-and explored the possible contribution of modulation of the inflammatory host response to that outcome. METHODS: The ACCESS trial

Iron Deficiency Screening and Treatment in Hospitalized HFrEF Patients: A Quality Improvement Initiative.

BACKGROUND: Iron deficiency affects up to half of patients with heart failure and is associated with worse symptoms, quality of life, and clinical outcomes, even in the absence of anemia. Contemporary guidelines recommend routine screening and intravenous (IV) iron therapy for eligible patients with heart failure with reduced ejection fraction (HFrEF), yet real-world adoption remains limited. PROJECT RATIONALE: Given persistently low screening and treatment rates, we sought to improve inpatient iron deficiency screening and IV iron use among patients hospitalized with acute HFrEF at a communi

Development of a Humanistic Interprofessional Healthcare Model for a Regional Swiss Hospital Using Community-Based Participatory Action Research.

Given the growing pressures on Switzerland's healthcare system, developing innovative approaches that put humanism at the heart of care practices seems essential to respond to patients' complex needs and to support healthcare professionals' well-being. Although several frameworks address humanistic care or interprofessional collaboration, few have been co-developed with healthcare professionals to guide the implementation of both dimensions at the organisational level. This article explores the development of an interprofessional healthcare model aimed at integrating humanistic values into a p

[HOSPITAL ACCREDITATION - TIME FOR RETHINKING?].

Accreditation provides an incentive to promote processes improving quality and enhancing patient safety, both in hospital and community settings. For the past three decades, many western, as well as developing countries, have adopted this methodology. Israel too has taken this pathway to adopt accreditation in general, geriatric and psychiatric hospitals as part of a national mission. Beyond the benefits to the patient, accreditation has numerous advantages: raising the professional accountability of healthcare providers, defining targets for improvement based on the strategies of the Ministry

Neonatal sepsis.

Neonatal sepsis is the cause of substantial morbidity and mortality. Precise estimates of neonatal sepsis burden vary by setting. Differing estimates of disease burden have been reported from high-income countries compared with reports from low-income and middle-income countries. The clinical manifestations range from subclinical infection to severe manifestations of focal or systemic disease. The source of the pathogen might be attributed to an in-utero infection, acquisition from maternal flora, or postnatal acquisition from the hospital or community. The timing of exposure, inoculum size, i

Quality of life in acne vulgaris: Relationship to clinical severity and demographic data.

BACKGROUND: Acne vulgaris is known to impair many aspects of quality of life. However, the correlation of this impairment with clinical severity remains equivocal despite various school, community and hospital-based studies. AIM: A hospital-based study was undertaken to measure the impairment of quality of life of patients of acne vulgaris and correlate it with the severity of lesions. METHODS: This was a cross-sectional, questionnaire-based study in a cohort of 100 patients of acne vulgaris attending the outpatient department of our referral hospital. A physician measured the severity of le

Implementation Outcomes of 36 CDC-funded Statewide Perinatal Quality Collaboratives, 2023-2024.

CONTEXT: Maternal and infant morbidity and mortality remain high in the United States, and improving quality of clinical care is one strategy to improve outcomes. Perinatal Quality Collaboratives (PQCs) are statewide networks of teams working to improve maternal and infant health by supporting quality improvement (QI) initiatives. PQCs execute deliberate activities to support implementation of QI; implementation outcomes can be used to measure the execution of these activities. OBJECTIVE: To examine PQC implementation outcomes to better understand implementation achievements. DESIGN: We adap

Teledialysis in Community Settings: Evaluation of Feasibility, Safety and Effectiveness.

The number of patients with end-stage kidney disease requiring haemodialysis (HD) is increasing worldwide, placing substantial pressure on healthcare systems. In Central Queensland, Australia, many patients, including Indigenous Australians, live far from major centres providing HD services. Teledialysis (Tele-D), involving virtual nephrologist-led ward rounds, can improve access to specialist care while supporting community-based dialysis. This study evaluated the feasibility, safety and effectiveness of Tele-D for satellite HD units in Central Queensland. This retrospective cohort study comp

A Survey Study on the Status of Somatic Symptoms in Young and Middle-Aged Patients with Mental Illness during Long-Term Hospitalization.

Psychiatric disorders include severe psychiatric disorders and those in general with some psychiatric disorders having a clear etiology or in which a significant psychiatric predisposing factor is present. Whereas, psychiatric disorders precisely refer to those characterized by mild depression and mild anxiety and appear to affect a large number of people in any community. It has been reported that the disease is highly prevalent and has a huge impact on the individual, family, and community levels, resulting in a heavy burden on the healthcare system of a country. To explore the status of som

Antibiotic policies in acute English NHS trusts: implementation of 'Start Smart-Then Focus' and relationship with Clostridium difficile infection rates.

OBJECTIVES: The objective of this study was to establish how antibiotic prescribing policies at National Health Service (NHS) hospitals match the England Department of Health 'Start Smart-Then Focus' recommendations and relate to Clostridium difficile infection (CDI) rates. METHODS: Antibiotic pharmacists were surveyed regarding recommendations for empirical treatment of common syndromes ('Start Smart') and antimicrobial prescription reviews ('Focus') at their hospital trusts. If no response was provided, policy data were sought from trust websites and the MicroGuide app (Horizon Strategic Pa

Bridging the Gap Between General and Transplant Infectious Disease Practice.

The population of iatrogenic immunocompromised individuals is steadily rising as we continue to improve in our ability to manage diseases, including cancer, autoimmune disease, and end-organ dysfunction, with increasing access to novel agents and transplantation. Enhanced survivorship allows these patients to return to their home communities and lives, leading to an increased number of immunocompromised patients across all healthcare systems. However, expertise in immunocompromised host infectious diseases or transplant ID (TID) is concentrated in urban centers and often in academic settings.
