CT-scan imaging of iron marked chorda tympani nerve: anatomical study and educational perspectives.

The chorda tympani nerve (CTN) is the last collateral branch of the facial nerve in its third intraosseous portion just over the stylomastoid foramen. After a curved course against the medial aspect of the tympanum where it is likely to be injured in middle ear surgery, CTN reaches the lingual nerve in the infratemporal fossa. Knowledge of CTN topographic anatomy is not easily achieved by the students because of the deep location of this thin structure. The aim of this study was to assess the spatial relationships of the CTN in the infratemporal fossa. Therefore, ten nerves were dissected in f

Individual, High-Precision 3D Mandibular Model for Finite Element Analysis of Three-Unit Bridges: A Biomechanical Pilot Study.

Tooth-supported fixed partial dentures (FPDs) exhibit complex biomechanical behaviour because occlusal loads are transferred through the periodontal ligament (PDL) and heterogeneous mandibular bone. This pilot study aimed to develop a patient-specific NURBS-based finite element analysis (FEA) workflow for anatomically realistic mandibular reconstruction and to evaluate the biomechanical effect of geometric simplification in tooth-supported FPD simulations. Cone beam computed tomography data from a single subject were segmented and reconstructed into a layered three-dimensional model of the man…

Anaesthetic Challenges in a Case of Oral Carcinoma With Anticipated Difficult Airway Posted for Tumour Excision and Reconstruction Surgery.

Mandibular surgery, edentulous jaw, denture wear, and ageing are all risk factors for persistent mandibular ridge resorption and weakening. The tongue occludes the upper airway due to the mandible's edentulous condition. All of these factors contribute to the difficulties in regulating the airway. An adequate preoperative review assisted in classifying this index patient as having a high risk of difficult airway management, and appropriate actions were made to facilitate effective airway care. A 60-year-old male presented to casualty with a complaint of squamous cell carcinoma of the right buc

Management of Temporomandibular Joint Ankylosis With Dentofacial Deformities in Children.

This retrospective study described the authors' experience in the treatment of temporomandibular joint (TMJ) ankylosis with dentofacial deformities in 18 pediatric patients during a 4-year period. These patients underwent different types of arthroplasty with condylar reconstruction, simultaneously with treatment of dentofacial deformities. Re-ankylosis was confirmed if maximal incisal opening (MIO) was <20 mm. Clinical outcomes were evaluated in terms of oral function, radiography, and medical photography. Patients were followed up for a mean time of 24.8 months. No infections, re-ankylosis, o

Robot-Assisted Osteotomy and Reconstruction with AR Guidance in Maxillofacial Reconstructive Surgery.

The treatment of maxillofacial tumors requires coordinated resection and reconstruction. Conventional template-guided open osteotomy techniques may increase the risk of tissue injury. Additionally, the accuracy of titanium plate-based reconstruction is limited by the lack of standardized positional criteria, thereby potentially affecting alignment consistency. To address these challenges, we propose a maxillofacial tumor treatment system termed RAMRS, comprising 2 modules: robot-assisted osteotomy and augmented reality-guided reconstruction (ARR). Within therobot-assisted osteotomy module, a h

Measures of health-related quality of life and socio-cultural aspects in young patients who after mandible primary reconstruction with free fibula flap.

BACKGROUND: The reconstruction of mandibular defects after trauma or tumor resection is one of the most challenging problems facing reconstructive surgeons. Although the primary intended outcome of surgery to treat head-and-neck malignancies is still the disease-free survival of the patient, health-related quality of life (HRQOL) is now seen as an essential secondary outcome. This study aims to evaluate HRQOL outcomes in young patients undergoing primary mandible reconstruction with free fibula flap and to collect information about their socio-cultural situation. METHODS: The HRQOL outcomes o

Complex maxillary fractures: role of buttress reconstruction and immediate bone grafts.

Between 1978 and 1984, 558 patients with complex facial fractures have been treated. One hundred and seventy-one of these patients have had complex Le Fort fractures of the maxilla. In this group of patients, the importance of direct anatomic reconstruction of the anterior maxillary buttresses has been assessed. Complete exposure of the injured buttresses will facilitate assessment of the exact fracture pattern. Direct fixation of the medial and lateral maxillary buttresses on each side, in combination with immediate bone-graft reinforcement or replacement of comminuted or missing buttresses,

A hydrogel-based triboelectric noncontact-ionic pressure dual-function sensing system for measuring mandibular movement trajectory and bite force.

This study proposes a flexible sensing system for simultaneously measuring mandibular movement trajectories and bite force during mastication. The system employs a dual-mode sensing mechanism: the triboelectric module realizes non-contact detection of movement trajectories, while the ionic supercapacitor module achieves contact-based quantitative monitoring of bite force. The triboelectric unit delivers a peak voltage of 0.2 V, with signal attenuation below 5% after 10,000 s of continuous cycling, enabling stable capture of dynamic occlusion trajectories. The ionic unit exhibits a high sensiti

Neuropathic Pain Due to Iatrogenic Lingual Nerve Lesion: Nerve Grafting to Reduce Otherwise Untreatable Pain.

Iatrogenic lingual nerve lesion is a well-known and unfortunate complication after mandibular third molar removal. Occasionally, the nerve injury can cause severe neuropathic pain. Here, the authors present the history of 2 patients with lingual nerve injury due to mandibular third molar removal, and with severe neuropathic pain in the craniomandibular region. Pharmacotherapy and physiotherapy did not reduce the pain, and ultimately, the lingual nerve was surgically explored. Scar tissue and a lingual nerve neuroma were observed and resected in both patients. In the first patient, the gap betwee

Useful Method for Intraoperative Monitoring of Facial Nerve in a Scarred Bed.

Facial nerve is the main cranial nerve for the innervation of facial expression muscles. Main trunk of facial nerve passes approximately 1 to 2 cm deep to tragal pointer. In some patients, where a patient has multiple operations, fibrosis due to previous operations may change the natural anatomy and direction of the branches of facial nerve. A 22-year-old male patient had 2 operations for mandibular reconstruction after gunshot wound. During the second operation, there was a possible injury to the marginal mandibular nerve and a nerve stimulator was used intraoperatively to monitor the nerve a…

CT imaging-led diagnosis of Parry-Romberg syndrome: A case report and literature review highlighting craniofacial and neurologic involvement.

Parry-Romberg syndrome (PRS) is a rare neurocutaneous disorder characterized by progressive unilateral facial atrophy involving soft tissues, fat, muscle, and sometimes underlying bone and brain tissue. The exact cause remains unclear, although autoimmune, neurogenic, vascular, traumatic, and inflammatory mechanisms have been proposed. We report a 43-year-old man in whom progressive unilateral facial atrophy was incidentally identified during evaluation after facial trauma. Computed tomography and three-dimensional craniofacial reconstruction demonstrated characteristic unilateral soft-tissue
