Metastasis of malignant myoepithelial carcinoma to the brain.

Myoepithelial tumours are a rare form of salivary gland neoplasm and intracranial metastases have rarely been described. The authors present the case of a 61-year-old patient with a history of primary myoepithelial carcinoma of the right foot. Metastases were found in the third ventricle, left temporal lobe and right frontal lobe. The third ventricular and left temporal lobe tumours were resected in a two-stage operation. Pathology of the third ventricular lesion was most consistent with metastasis. MRI of the spine 2 weeks after his operation revealed an intradural mass with several lesions i

TP53 Gene Status Affects Survival in Advanced Mycosis Fungoides.

TP53 is frequently mutated in different types of neoplasms including leukemia and lymphomas. Mutations of TP53 have also been reported in mycosis fungoides (MF), the most common type of cutaneous lymphoma. However, little is known about the frequency, spectrum of mutations, and their prognostic significance in MF. In this study, we have optimized the protocol for Sanger sequencing of TP53 using DNA extracted from archival paraffin-embedded biopsies. Of 19 samples from patients with stage IIB MF or higher, 31% harbored mutations in TP53. Overall survival of the patients with mutated TP53 was si

Systemic therapy for cutaneous T-cell lymphoma: who, when, what, and why?

CTCL are rare neoplasms. Optimal care requires integrated use of diagnostic and treatment modalities spanning multiple specialties. Current instruments for patient risk stratification and disease measurement across all anatomical compartments are suboptimal. A common treatment dichotomy between early (Dermatology) and advanced stage (Hematology-Oncology) has hindered accrual of long term outcome data. Thus, important facts about natural history, such as frequency and determinants of stage progression, and the impact of specific treatment modalities on these endpoints, are not known. Areas cove

Nasopharyngeal carcinoma: a review.

Nasopharyngeal carcinoma is an umbrella term for a group of malignant epithelial tumors with different etiopathogenesis and a broad range of histopathological appearances. Some types have a dramatically skewed geographical and ethnic distribution and shows virtually 100% association with Epstein Barr virus. The field of other/contributing etiological factors are only partially known and recently a subset of this carcinoma has been linked to transcriptionally active Human Papilloma virus. As with all malignant tumors, the clinical stage of disease is of paramount importance. Despite loco-region

Oligometastatic non-small cell lung cancer (NSCLC): adrenal metastases. Experience in a single institution.

Though the actual incidence of an adrenal oligometastasis is between 1.5 and 3.5 %, secondary adrenal neoplasms occur in less than 10 % patients with non-small cell lung cancer (NSCLC). According to 7° ed. TNM staging system, the presence of an adrenal metastasis (M1b disease) configures stage IV, which is usually associated with poor prognosis. We evaluated if metastasectomy in selected patients with oligometastatic disease improves overall survival. A 15-year retrospective study concerning patients with NSCLC was performed and an oligometastatic disease was found in 1.61 % of the patients. 1

Recurrent invasive thymoma with pleural dissemination : disease management and treatment possibilities.

Thymoma is the most common benign neoplasm of the anterior mediastinum presenting often an agressive behaviour typical for the malignants tumors. The rate of invasive thymoma recurrency is relatively high. We present the case of a 55-year old man with a recurrent invasive thymoma with a pleural dissemination, detected on CT-imaging 2 years following his primary surgery. Since the first pre-operative imaging studies showed no invasion of the adjacent organs and a thymoma was suspected, a surgical resection was decided as a first line treatment. Per-operatively a number of adjacent structures we

Laparoscopic en bloc resection of locally advanced colon cancer with involvement of the liver, small bowel, omentum, and abdominal wall.

BACKGROUND: Locally advanced colon cancer with the involvement of adjacent organs is treated radically with en bloc resection with negative margins. Laparoscopy is seldom recommended for such patients. METHODS: Operative laparoscopic technique is presented for radical en bloc resection of locally advanced right colon cancer. RESULTS: The patient is a 56-year-old morbidly obese (body mass index 47) male, who was scheduled to undergo a routine laparoscopic right hemicolectomy and at exploration was found to have a large mass involving segments V and VI of the liver, small bowel, omentum, and a

Anal canal neoplasms in the modern era: integrating HPV biology, LAST terminology, and evolving molecular classification.

Anal canal neoplasia comprises a heterogeneous and evolving group of precursor lesions and invasive malignancies whose classification increasingly depends on integrating morphology, human papillomavirus association, immunophenotype, and molecular alterations. This review summarizes current concepts in the pathology of anal canal neoplasms, with emphasis on practical application of Lower Anogenital Squamous Terminology, distinction between low-grade and high-grade squamous intraepithelial lesions, and the role of ancillary testing in diagnostically challenging cases. We discuss the expanding sp

INI1-Deficient malignancy involving the pancreas and jejunum: Expanding the clinical spectrum.

SMARCB1/integrase interactor 1(INI1)-deficient malignancies of the gastrointestinal (GI) tract are exceedingly rare, poorly characterized, and notoriously aggressive, posing substantial diagnostic challenges for radiologists due to their nonspecific imaging features and overlap with more common neoplasms. Additionally, fewer than 50 cases of INI1-deficient GI carcinomas have been documented, making their recognition on imaging alone exceptionally challenging. We present a case of a 25-year-old female with a rapidly enlarging INI1-deficient poorly differentiated tumor involving both the pancrea

Mixed pancreatic ductal adenocarcinoma and well-differentiated neuroendocrine tumor: A case report.

BACKGROUND: Pancreatic mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare malignancies affecting the pancreas. The World Health Organization defines MiNENs as neoplasms composed of morphologically recognizable neuroendocrine and non-neuroendocrine components, each constituting 30% or more of the tumor volume. Adenocarcinoma-neuroendocrine carcinoma is the most frequent MiNEN combination. A well-differentiated neuroendocrine tumor (NET) component is rarely reported in MiNENs. CASE SUMMARY: Here we report a rare case with intermingled components of ductal adenocarcinoma and gra

Hypercalcemia Secondary to Elevated PTHrP in an Infant Followed by Progression to Nephrotic Syndrome.

In infants, hypercalcemia from elevated parathyroid hormone-related protein (PTHrP) is rare, often signaling neoplasm or renal or urinary anomalies. We report an infant who presented with failure to thrive and hypercalcemia at 10 months old, with initial evaluation showing elevated PTHrP of unclear etiology with imaging negative for neoplasm and no structural anomalies of the kidneys or ureters on ultrasound. Within 6 months of presentation, the patient developed nephrotic syndrome and by 2 years had progressed to end-stage kidney disease, necessitating kidney transplantation. Genetic testing

Hematologic Malignancies Patients Face High Symptom Burden and Are Lately Referred to Palliative Consultation: Analysis of a Single Center Experience.

Although hematologic neoplasms have been on the vanguard of cancer therapies that led to notable advances in therapeutic efficacy, many patients face significant symptom burden, which make them eligible for early palliative care (PC) integration. However, previous reports demonstrated that hematological malignancies receive more aggressive care at the end-of-life and are less likely to receive care from specialist palliative services compared to solid tumors. Our aim was to characterize symptom burden, performance status and clinical characteristics of a cohort of hematologic malignancies pati

How I Diagnose Primary Myelofibrosis.

OBJECTIVES: Primary myelofibrosis (PMF) is a BCR/ABL1-negative myeloproliferative neoplasm (MPN) with a shorter overall survival and a higher leukemic transformation than other BCR/ABL1-negative MPNs. Diagnosis of PMF can be challenging given its clinical, morphologic, molecular overlap with other myeloid neoplasms also associated with myelofibrosis, and reactive conditions. METHODS: We summarize and discuss the clinical, morphologic, and molecular features useful for diagnosing PMF as well as salient features helpful in distinguishing PMF from myelodysplastic syndrome with associated fibrosi

Serum angiopoietin-2 levels in dogs with splenic haemangiosarcoma, haemangioma, and splenitis.

Haemangioma (HA) and haemangiosarcoma (HSA) are among the most common splenic neoplasms in dogs. The survival time in splenic HSA is short, probably due to the lack of proper biological markers allowing early detection. We investigated the serum angiopoietin-2 (Ang-2) concentrations in 9 healthy dogs and 40 dogs with abnormal splenic masses. The Ang-2 concentration differences were further compared in healthy dogs, dogs with splenitis, splenic HA and HSA. The results showed that the Ang-2 level in healthy dogs was significantly lower than in the splenitis and splenic HA cases. Moreover, the An

Dysfunctional labor and hemoperitoneum secondary to an incidentally discovered dysgerminoma: a case report.

BACKGROUND: Ovarian dysgerminoma, a subtype of malignant germ cell tumor (GCT), is a rare ovarian neoplasm that is infrequently found in the gravid patient. When dysgerminomas do occur in pregnancy, the rapidly growing tumors can have a heterogeneous presentation and lead to peripartum complications and morbidity. Due to the rarity of this condition, diagnostic and therapeutic strategies are not well described in the literature. CASE PRESENTATION: A healthy multigravida with an uncomplicated antenatal history presented for elective induction of labor. She had a protracted labor course, persis

Uniportal video-assisted thoracoscopic surgery for lung neoplasms with tracheal bronchus: a case report.

A 63-year-old woman came to our department complaint that an abnormal shadow was identified by chest computed tomography (CT). CT revealed a 21 mm × 18 mm solid nodule in the right upper lobe of lung, which was suspected to be lung cancer. We also found a bronchus directly arising from the trachea and running into the right upper lobe, which appeared to be tracheal bronchus (TB). Then she underwent the right upper lobectomy and mediastinal lymph node dissection using uniportal video-assisted thoracic surgery. During the surgery we found two bronchi which were posterior to the pulmonary hilum r

[Protein-dysregulation in human and murine myeloproliferative neoplasms].

In this work different types of dysregulation of signaling proteins in the context of myeloproliferative neoplasms are examined. In this heterogeneous disease group, uncontrolled cell proliferation plays a crucial role for the initiation of tumorigenesis, which Robert Weinberg described as a "hallmark" for the development of cancer. Protein dysregulation in form of overexpression of GAB2, a protein involved in formation of the CML-pathognomonic BCR/ABL-translocation complex, results in an enhanced disease phenotype in a Bcr/Abl-positive mouse model and disease acceleration is associated with a

Secondary extramedullary plasmacytoma of sigmoid colon in a patient with multiple myeloma: a case report.

BACKGROUND: Extramedullary plasmacytoma is an uncommon tumor that most often involves the nasopharynx or upper respiratory tract. Extramedullary plasmacytoma is a type of plasma cell neoplasm that can present as a primary tumor or secondary to another plasma cell neoplasm, such as multiple myeloma. Secondary extramedullary plasmacytoma is usually noted in the advanced stages of the disease. Involvement of the gastrointestinal tract occurs in approximately 10% of cases. GATA1 insufficiencies in primary myelofibrosis and other hematopoietic disorders: consequences for therapy.

GATA1, the founding member of a family of transcription factors, plays important roles in the development of hematopoietic cells of several lineages. Although loss of GATA1 has been known to impair hematopoiesis in animal models for nearly 25 years, the link between GATA1 defects and human blood diseases has only recently been realized. Areas covered: Here the current understanding of the functions of GATA1 in normal hematopoiesis and how it is altered in disease is reviewed. GATA1 is indispensable mainly for erythroid and megakaryocyte differentiation. In erythroid cells, GATA1 regulates earl

[Intraoperative frozen sections in diseases of the female genital tract].

Intraoperative frozen sections are particularly important for ovarian tumors because definitive preoperative histology is not possible. The diagnostic accuracy of frozen sections is highest for primary invasive ovarian carcinomas and benign ovarian lesions, followed by borderline tumors and poorest for ovarian metastases and rare neoplasms, such as germ cell tumors. Endometrial carcinoma should be diagnosed preoperatively by curettage or biopsy. For endometrioid endometrial carcinomas the indications for lymphadenectomy are often based on intraoperative assessment of the uterus. The differenti
