Slowed nerve conduction with wrist flexion in carpal tunnel syndrome.

Phalen's wrist flexion test may reproduce the sensory symptoms of carpal tunnel syndrome (CTS). We have used this test in conjunction with motor and sensory latency studies in 20 patients with symptomatic CTS in one or both hands. The test caused an increase in the motor or sensory latency in half the patients. In 2 patients the latencies were abnormal only after wrist flexion. The test may thus be useful in diagnosis of borderline cases.

Moebius syndrome, peripheral neuropathy and hypogonadotrophic hypogonadism.

The association of congenital ophthalmoplegia and facial paresis (Moebius syndrome) with a variety of other developmental somatic defects has been widely recognised. Its co-existence with hypogonadism of hypothalamic/pituitary origin and subclinical peripheral neuropathy has been reported and in this paper we describe the second case of the Moebius syndrome in association with hypogonadotrophic hypogonadism and a progressive peripheral neuropathy of mixed axonal and demyelinating type.

Selective cell loss in the lateral geniculate nucleus of adult cats following binocular lid suture.

Two weeks of binocular lid suture in adult cats caused selective cell loss in lamina A1 of the lateral geniculate nucleus. The loss was manifested as a shift in the latency distribution of LGN cell responses to optic chiasm shock. Proportionately fewer LGN cells with slowly conducting retinal afferents (X-cell range) were in evidence following binocular suture. This change was observed only in lamina A1 while laminae A and C retained normal latency distributions. These observations confirm and extend recent reports of the susceptibility of adult cat visual system to environmental modification.

Thoracic outlet syndrome in whiplash injury.

Thirty-five cases of thoracic outlet syndrome complicating whiplash or cervical strain injury were studied. Thirty cases had confirmation by the demonstration of slowed ulnar nerve conduction velocity (UNCV) through the thoracic outlet. Two distinct groups of patients were found. An acute group, seen an average of 3 1/2 months post injury, had severe neck pain with often mild or incidental thoracic outlet syndrome. A chronic group, with symptoms persisting more than 2 years after cervical injury, often had thoracic outlet symptoms as the predominant complaint. This study suggests that the arm

Utility of neuromuscular ultrasound and comparison of NMUS with electrodiagnostic tests in dominant spinocerebellar ataxia.

OBJECTIVE: To estimate and compare the prevalence of sensory neuronopathy and neuropathy in autosomal dominant spinocerebellar ataxia (SCA) using neuromuscular ultrasound (NMUS) and traditional electrodiagnostic tests (EDX). METHODS: We compared NMUS [median and ulnar nerve cross-sectional areas (CSAs)] with EDX [sensory (sural, radial, median, ulnar); motor (median, ulnar, tibial)] findings from previously published and newly recruited patients with SCA (44 in total; SCA1 = 8, SCA2 = 27, SCA3 = 2; SCA6 = 7). Sensory neuronopathy was diagnosed by reduced nerve CSA on NMUS and non length-depen

Nerve ultrasound in asymptomatic hereditary transthyretin amyloidosis carriers.

OBJECTIVE: The aim of our study was to perform high-resolution nerve ultrasound in asymptomatic carriers of transthyretin (TTR) p.Glu89Gln (p.Glu109Gln) mutation and to evaluate its role as a complementary non-invasive diagnostic tool in early detection of amyloid deposits in peripheral nerves. METHODS: The study included 21 asymptomatic carriers of TTR p.Glu89Gln (p.Glu109Gln). History, neurological examination, nerve conduction studies, sympathetic skin response, SudoScan and high-resolution nerve ultrasound were performed. RESULTS: Ultrasound pattern sum score was abnormal in 70% of the c

Molecular Characterization of Oxaliplatin-Induced Peripheral Neurotoxicity: The Complex Spectrum of Painful Manifestations.

BACKGROUND AND AIMS: Oxaliplatin (OHP) induced peripheral neurotoxicity (OIPN) is a complex spectrum comprising an acute and a chronic form. Acute OIPN leads to unpleasant transient sensations in the 24-72 h after chemotherapy, due to a temporary dysfunction in ion channels (i.e., axonal hyperexcitability in the absence of anatomical damage). Whereas chronic OIPN is characterized by painful manifestations. Literature data showed that a more pronounced acute OIPN could be an early predictor of chronic OIPN; thus, acute OIPN is becoming a possible target to prevent chronic OIPN. We went back to

Effect of Forearm Supination and Pronation on Median & Ulnar Nerve Conduction Velocity Among Throwers, Archers, and Non-Athletes.

The distinct and specialized movements performed in different sports disciplines may significantly influence nerve performance, potentially affecting nerve responses and the overall function within the respective athletic activities. The purpose of this study is to find the effect of forearm supination and pronation across the elbow joint on ulnar and median nerve conduction velocity (NCV) in throwers, archers, and non-athletes. A total of 34 participants both male and females were recruited with a body mass index (BMI) between 18.5 and 24.9 kg/m2. Nerve conduction study (NeuroStim NS2 EMG/NCV

Convolutional neural network for brachial plexus segmentation at the interscalene level.

BACKGROUND: Regional anesthesia with ultrasound-guided brachial plexus block is widely used for patients undergoing shoulder and upper limb surgery, but needle misplacement can result in complications. The purpose of this study was to develop and validate a convolutional neural network (CNN) model for segmentation of the brachial plexus at the interscalene level. METHODS: This prospective study included patients who underwent ultrasound-guided brachial plexus block in the Anesthesiology Department of Beijing Jishuitan Hospital between October 2019 and June 2022. A Unet semantic segmentation m

Clinical and Electrophysiological Characteristics of Very Early Guillain-Barré Syndrome.

PURPOSE: This study compared the clinical and electrodiagnostic (EDX) features and long-term outcomes of patients with very early Guillain-Barré syndrome (VEGBS, duration of illness ≤4 days) and those with early/late (>4 days)-presenting GBS. METHODS: One hundred patients with GBS were clinically evaluated and categorized into VEGBS and early/late GBS groups. Electrodiagnostic studies were performed on the bilateral median, ulnar, and fibular motor nerves and the bilateral median, ulnar, and sural sensory nerves. Admission and peak disability were assessed using the 0 to 6 Guillain-Barré Synd

Provocative changes in nerve conductions: Fact or fiction?

At times electrodiagnostic medical consultants (EMCs) are asked to perform studies in both a neutral position, and then again after the patient is in a provocative position that exacerbates symptoms, to assess for measurable electrophysiologic changes. While this approach might seem initially appealing, particularly when standard studies are not effective at diagnosis, empiric studies in several conditions have been unimpressive. Studies in median neuropathy at the wrist, thoracic outlet syndrome, piriformis syndrome, and radial tunnel syndrome have failed to demonstrate reproducible changes i
