The impact of oedema on anthropometric measurements in nutritional surveys: a case study from Zaire.

Six surveys for protein-energy malnutrition were carried out in sequence in Bas-Zaire beginning at the end of 1978 at the estimated height of the famine and continuing throughout the recovery period. Utilizing a stratified multi-stage sampling technique, over 1000 children aged 6 months to 6 years were measured anthropometrically and examined for the presence or absence of bilateral pedal oedema in each survey. The proportions of children who were less than 80% of the reference median weight-for-height and who had oedema decreased, respectively, from 12.8% and 14.4% initially to 2.1% and 1.8%

Nutritional aspects of magnesium metabolism.

The absolute necessity for magnesium in plant and animal nutrition is easily appreciated when one realizes that magnesium is found in high concentration in cells. It is necessary for photosynthesis in plants and for all reactions involving adenosine triphosphate in plant and animal cells. Although it is abundant in nature in general, deficiencies occur in both plants and animals. Human beings need about 5 mg of magnesium per kg of body weight per day. Infants and young children need twice as much. Children and women during pregnancy or lactation require significantly greater amounts than norma

Trace minerals in paediatric gastroenterology: zinc and copper.

A number of aspects of zinc and copper metabolism are relevant in the clinical practice of pediatric gastroenterology. Although the diagnosis of zinc and copper deficiency is problematic, deficiencies and altered metabolism of these two nutrients are seen in a host of hepatic and intestinal diseases of childhood. Several medical and surgical interventions can adversely affect zinc and copper nutriture as well. Continuing scientific research into the biology of zinc and copper will provide new insights into the pathophysiology of pediatric gastrointestinal diseases.

Vitamin C Deficiency in an Adolescent With Severe Anorexia Nervosa.

This article reports on the case of a 14-year-old non-binary patient with anorexia nervosa - restrictive subtype (AN-R), presenting with severe malnutrition and the cutaneous manifestations of scurvy. Scurvy is an uncommon nutritional deficiency caused by a lack of dietary vitamin C. In this case, we highlight the clinical features of vitamin C deficiency, a rare but treatable nutritional deficiency that should not be overlooked in patients presenting with malnutrition, including those with eating disorders. This case reviews the pathophysiology, common clinical manifestations, and prevention

Exercise-induced amenorrhea and bone health in the adolescent athlete.

Female participation in high school athletics has increased 800% in the last 30 years. The problem of exercise-induced amenorrhea was initially thought to be analogous to hypoestrogenism, but recent studies suggest that nutritional issues underlie most of the pathophysiology and that the mechanism is different from that seen in the primary hypogonadal state. Exercise-induced amenorrhea can be an indicator of an energy drain, and the presence of the other components of the female athlete triad-bone density loss and eating disorders-must be determined as well. Addressing skeletal problems relate

Nutrition, anxiety and hormones. Why sex differences matter in the link between obesity and behavior.

Obesity and mood disorders are two of the most serious health issues of modern times. These health conditions are often linked, with obesity acting both as a cause and consequence of anxiety and depression. Although sex differences in the relationship between obesity and mood disorders are observed in clinical populations, the relative influence of biology versus societal conditioning is unclear. In part, this is because sex effects are rarely examined in the animal models used to derive our understanding of basic biological mechanisms. Due to the perceived confounding nature of hormonal fluct

A Knowledge-based Decision Support System for recommending safe recipes to individuals with dysphagia.

BACKGROUND: Dysphagia is a disorder that can be associated to several pathological conditions, including neuromuscular diseases, with significant impact on quality of life. Dysphagia often leads to malnutrition, as a consequence of the dietary changes made by patients or their caregivers, who may deliberately decide to reduce or avoid specific food consistencies (because they are not perceived as safe), and the lack of knowledge in how to process foods are critics. Such dietary changes often result in unbalanced nutrients intake, which can have significant consequences for frail patients. This

Changing the culture around hospital-based nutrition.

Following a serious incident and inquest after the death of a patient due to choking at Sheffield Teaching Hospitals NHS Foundation Trust, the Trust put in place an action plan and implemented strategies to reduce the risk of recurrence. Four key actions were identified as essential to try to reduce the risk of a similar event: introduction of a standard operating procedure for mealtimes that included a pre-meal safety 'pause'; use of an electronic communication icon to indicate modified diet/fluid requirements, from emergency department and onward as a patient is transferred; job-specific mea

Metabolic and Nutritional Issues after Lower Digestive Tract Surgery: The Important Role of the Dietitian in a Multidisciplinary Setting.

Many patients undergo small bowel and colon surgery for reasons related to malignancy, inflammatory bowel disease (IBD), mesenteric ischemia, and other benign conditions, including post-operative adhesions, hernias, trauma, volvulus, or diverticula. Some patients arrive in the operating theatre severely malnourished due to an underlying disease, while others develop complications (e.g., anastomotic leaks, abscesses, or strictures) that induce a systemic inflammatory response that can increase their energy and protein requirements. Finally, anatomical and functional changes resulting from surge

Complications Associated With Nasogastric Tube Placement in the Acute Phase of Stroke: A Systematic Review.

This systematic review explores the frequency of complications associated with nasogastric tube (NGT) placement in patients with acute stroke. The unique condition of the dysphasic, immobile, confused immunosuppressed patient who has had an acute stroke hampers any consistent inference from other neurological or nonneurological condition in which NGT placement is used. Twelve studies including 921 patients fed by NGT in the acute phase (first 4 weeks) were included in the analysis. The overall quality of the included studies was good. The main limitation was the heterogeneity and small size of

Hair loss in systemic disease.

A careful history and examination of shed hairs will reveal the etiology of most alopecias due to systemic processes. Telogen effluvium is preceded by a severe systemic stress occurring at least two months prior to the loss of normal club hairs. Most other causes of hair loss involve damage to the hair follicle, which leads to the shedding of dystrophic, brittle anagen hairs. A history of drug ingestion or nutritional compromise or concurrent symptoms suggestive of a genetic, endocrinologic, collagen vascular, or infectious etiology will lead to an accurate diagnosis.
