BACKGROUND: The goal of this retrospective study was quantitative calculation of area and volume of isolated orbital floor fractures from computed tomography (CT) and correlation of these data with post-traumatic ophthalmologic findings.
PATIENTS AND METHODS: A total of 76 patients with isolated orbital floor fractures were evaluated radiologically and clinically. CT scanning was performed in coronal sections (1.5-mm to 3.0-mm slice thickness) with contiguous table feed. Orbital floor and fracture area as well as volume of displaced tissue were measured and calculated from the CT dataset. The…

A 71-year-old female patient fell in her garden, inducing a skin wound on the temporal left eyebrow. Skin disinfection and wound closure were performed elsewhere, an X-ray image did not reveal a foreign body. She was referred to our service three days later with a progressive left periorbital swelling. Clinical inspection demonstrated a painfully, fluctuant swelling around the wound with an inflammatory pseudoptosis of the left eye. Vision was reduced on the left eye; anterior and posterior segments of both eyes were unharmed. After opening the wound sutures a purulent liquid was drained and a…

We present an overview of the treatment and clinical outcome of 15 orbital lymphomas, carried out in our center from 1993 to 2002. The surgical approach was determined by the location, and type of the lesion. Lateral orbitotomy was performed in five laterally located lesions, a transconjunctival approach in five medial, basal, extra-, intraconal lesions. Pterional intradural and extradural and intradural approaches (three) were used in case of intracranial involvement or location in the apex and optic canal. Two lesions of the lid or extraconal space were operated via eyebrow incision. Ten pat…

CASE REPORT: Patient who consults for a recidivant orbital mass. Preliminary clinical and image studies suggested an initial differential diagnosis between a lachrymal gland tumor or an orbital located pseudotumor. However, the clinical evolution of the case required a re-evaluation of the preliminary diagnosis. The clinical, histologic and serologic findings were key in the final diagnosis.
DISCUSSION: Wegener's granulomatosis is a necrotizing vasculitis. Classically it affects the upper airways, lungs and kidneys. Between 30% and 70% of patients present ophthalmic symptoms during the course…

PURPOSE OF REVIEW: Orbital fractures are usually associated with ocular and intracranial injuries. The recent literature on orbital fractures is analyzed with emphasis on epidemiology, assessment, and surgical management.
RECENT FINDINGS: In most countries, traffic accidents are the leading cause of orbital fractures. Orbitozygomatic fractures are extremely frequent but rarely addressed in the ophthalmic literature. CT scanning remains the gold standard for assessing orbital fractures, especially with the new CT technology (multislice CT), which has improved the acquisition of coronal images …

Fulminant processes of the orbit can cause considerable diagnostic difficulties with regards to the clinical, radiological and microbiological appearance. Rhino-orbito-cerebral mucormycosis is a rare but, when untreated, fatal orbital disease capable of destructive infiltration of soft tissue and bone. It occurs preferably in the ill adjusted diabetic or in the course of an immunodeficiency. The following case report describes a leukaemia patient developing a severe rhino-orbito-cerebral mucormycosis which was initially not recognised. Since untreated mucormycosis is fatal, an aggressive surgi…

PURPOSE: To evaluate the MR imaging findings of orbital hemangiomas in pediatric and adult groups.
MATERIALS AND METHODS: MR imaging findings of seven orbital hemangiomas were reviewed. All patients had axial and coronal T1 and T2 weighted images. Axial and coronal fat suppressed (STIR) images and postcontrast T1 weighted images in three planes were obtained in order to evaluate the relation with surrounding structures. Localization, size, margins and signal characteristics of the lesions were analyzed.
RESULTS: Four of the cavernous hemangiomas were intraconal, one was extraconal. The locat…

This article describes a rare cause of orbital apex syndrome in a child--juvenile nasopharyngeal angiofibroma. A high level of suspicion while treating an adolescent male with chronic nasal obstruction, epistaxis, and a nasal mass will promote early diagnosis. Multidisciplinary care is advisable, especially for advanced disease.

BACKGROUND: Infections of the orbit and of the periorbital region are not uncommon. Even today they constitute a serious problem, in spite of modern antibiotic treatment, with a potential risk of lethal complications. Orbital infections are most prevalent in children and adolescents. The acute orbit has many causes, but the most frequent is an occurrence secondary to acute rhinosinusitis.
PATIENTS AND METHODS: Based on clinical cases, different causes and the present state of diagnosis are presented. The discussion deals with therapeutic strategies depending on the stage according to the curr…

Orbital cysticercosis is a benign infection with a variety of clinical presentations. Awareness of all of the possible clinical manifestations could aid early diagnosis and prevention of late sequelae. We report a case of bilateral, multifocal, intraconal orbital cysticercosis, which is an extremely rare presentation.

A 50-year-old male presented with progressive proptosis in the right eye for 10 years. Decreased visual acuity to hand-motion level was also noted. Orbital computed tomography revealed a large homogenously enhanced mass lesion in the right retrobulbar region with optic nerve compression. The remainder of the ocular examination was normal, except for right mature cataract. The tumor was completely removed through lateral orbitotomy. Histologic findings showed a picture of hemangiopericytoma. One month after cataract extraction and intraocular lens implantation in the right eye, the patient had …

PURPOSE: To determine the incidence and risk factors of complications associated with Supramid orbital implants.
METHODS: A retrospective chart review was performed to document complications in a series of 41 patients receiving Supramid implants during orbital reconstruction or fracture repair. Implant size, duration of time between trauma and implant insertion, and history of prior orbital surgery were examined as possible risk factors for the development of complications.
RESULTS: Four patients had hemorrhage within the implant capsule. An orbital abscess developed in a single patient. All…

We present an overview of the treatment and clinical outcome of five orbital peripheral nerve tumours, carried out in our centre from 1999 to 2003. The surgical approach was determined by the location and extension of the lesion. Supraorbital orbitotomy was performed in two superiorly located lesions, a transconjunctival approach in one medial, basal, extraconal lesion. A pterional extradural approach was used in two cases with involvement of the apex, superior orbital fissure and cavernous sinus. Three patients were diagnosed as having schwannoma, one as neurofibroma, and one as cystic mixed …

Pyoderma gangrenosum belongs to the chronic, neutrophilic and necrotic dermatoses. These very painful, inflammable and purulent skin-ulcers often arise after injury or surgical intervention. About half of the patients suffer from systemic disease, for example hematological, gastrointestinal or rheumatic diseases. Our report includes a 77-year-old female with a multilocular emergence of pyoderma gangrenosum following surgery for an orbital pseudotumor. An additional lesion was detected on her anterior neck. Because the possibility of wound-infection was suspected, the lesion was treated locally…

INTRODUCTION: Nontraumatic orbital diseases only exceptionally require emergency treatment. In this paper an emergency is considered when the diagnosis and/or treatment must be done within a maximal period of 1 week.
MATERIALS AND METHODS: All patients with nontraumatic orbital diseases from the Department of Ophthalmology of the University of Lausanne and from the office of one of the authors (N.D.) over the last 35 years were studied, in view of their degree of emergency. Data from 77 patients (28 females and 49 males), aged from birth to 85 years, were collected.
RESULTS: Idiopathic infla…

To determine the appropriate early management of orbital cellulitis and the current bacterial etiology and to evaluate the clinical usefulness of orbital computed tomographic imaging for this infection, a prospective study of orbital cellulitis was conducted during a 2-year period, 1999 to 2000, after the introduction of a conservative medical management plan designed by our pediatric infectious diseases, ophthalmology and otolaryngology services. Basically patients did not have surgical intervention unless progressive involvement of the optic nerve occurred after 24 to 36 h of intravenous ant…
