Teeth with double internal inflammatory resorption: report of two cases.

Internal inflammatory resorption is an uncommon condition, which requires the presence of necrotic and infected pulp tissue within the coronal portion of the root canal system as well as inflamed pulp tissue apical to the resorptive defect. The defect usually appears as a bowl-shaped radiolucency within the tooth root. Most cases are asymptomatic and many cases are only seen once the entire pulp has necrosed and all of the root canal system is infected resulting in apical periodontitis. The two cases presented are unusual in that they both had two distinct areas of internal inflammatory resorp

The resolution of a periradicular lesion involving an implant.

Published reports claim that implants can fail from endodontic pathosis involving teeth adjacent to an implant, from preexisting bacteria in an extraction site that had a history of a tooth having a periradicular lesion, and even from an asymptomatic endodontically treated tooth with no clinical or radiographic evidence of pathosis. This report considers the support offered for an endodontic connection to implant failure, and it presents a case that conflicts with the premise that endodontic involvement causes implant failure. The diagnosis and treatment of a radiolucent lesion associated with

Apical actinomycosis as a continuum of intraradicular and extraradicular infection: case report and critical review on its involvement with treatment failure.

This article reports a case of extraradicular actinomycosis that clearly formed a continuum with the intraradicular infection. Histobacteriologic and histopathologic analyses of the root tip and attached apical periodontitis lesion obtained by surgery from a tooth with persistent disease were performed to look for the possible reasons for persistent disease. Although no stainable bacteria were observed in the apparently well-treated main canal, apical ramifications were clogged with dense bacterial biofilms that were contiguous to extraradicular actinomycotic aggregates. A critical review of t

Morphometric analysis of experimentally induced periapical lesions: radiographic vs histopathological findings.

OBJECTIVES: To evaluate the accuracy and reliability of conventional (Kodak Ektaspeed Plus film) and digitized radiographic images to detect the presence as well to estimate the size, as measured by an image analysis programme, of periapical radiolucencies induced in dog teeth in comparison with the histomorphometric data obtained from the same lesions by conventional and fluorescence microscopy. METHOD: After the removal of pulp, the root canals of five premolars from the same animal were left exposed for 7 days after which they were sealed for 60 days. At day 53, three more premolars were o

Root canal treatment on a patient with zinc oxide allergy: a case report.

AIM: To describe successful root canal treatment of a patient with a true zinc oxide allergy and to discuss allergic reactions to dental materials. SUMMARY: Dental materials have been reported as aetiologic agents for both local and systemic allergic reactions. It is essential for the oral healthcare provider to recognize the clinical symptoms associated with allergic reactions and to modify dental treatment, if necessary, to prevent these reactions from occurring. This article describes an unusual case of a patient with an allergy to zinc oxide. To our knowledge, this is the first case of su

Sudden improvement of insulin sensitivity related to an endodontic treatment.

BACKGROUND: Inflammation contributes to the pathogenesis of diabetes. A reciprocal relationship exists between diabetes and chronic periodontitis. This report describes the effects of an acute focal dental inflammation and subsequent endodontic treatment on the required insulin dosage of a 70-year-old man who had moderately controlled diabetes. METHODS: Following an exacerbation of a combined endodontic-periodontic (endo-perio) lesion of tooth #3, the patient noticed a sudden increase in his insulin demand. After 3 weeks, the required dosage was approximately 100% greater. In association with

Incidental findings of pathology and abnormality in pretreatment orthodontic panoramic radiographs.

Panoramic radiographs, in combination with a clinical examination, are routinely used as an aid to orthodontic diagnosis and treatment planning. The aim of this study was to evaluate the prevalence and location of incidental findings of pathology and abnormalities in pretreatment orthodontic panoramic radiographs. A total of 496 patients (232 girls and 264 boys; mean age 11.2 years, SD 2.33) were randomly selected from the Orthodontic Clinic at the Faculty of Odontology, University of Malmö, Sweden. All radiographic examinations were performed between 1999 and 2003 at the Department of Oral Ra

Proliferative periostitis of Garré: Report of a case.

Proliferative periostitis of Garré is described as a productive and proliferative inflammatory response of periosteum to infection or other irritation. This can be odontogenic or non-odontogenic in nature. This is a case report of an odontogenic periostitis resulting from periapical inflammation of endodontic origin. It was successfully treated by nonsurgical endodontics. Antibiotic therapy was not used during the treatment of this patient.

3D-printed model for preclinical training in oral radiology: anatomic and pathological conditions.

OBJECTIVE: The aim of this study was to evaluate the impact of a 3D-printed model with simulated oral changes on the teaching of radiographic evaluation. METHOD: A model of an adult patient with several simulated alterations was designed, including impacted teeth, dentigerous cyst, mesiodens, coronal fractures, periodontal resorptions, periapical lesions, and exostoses. The radiographic images obtained were evaluated by postgraduate students using a questionnaire. The data obtained were analysed with descriptive and inferential statistics. RESULTS: The 3D model produced satisfactory images f

Microscopic Endodontics in Infected Root Canal with Calcified Structure: A Case Report.

Calcium deposited within a root canal due to exogenous stimuli may hamper root canal treatment. In endodontic treatment, an operating microscope allows the conditions within the root canal to be directly viewed and evaluated. This report describes a case in which an operating microscope was used to facilitate the excision of a calcified structure from within a root canal at an early stage in the treatment of an infection. An 18-year-old man was referred to our clinic due to suspected chronic suppurative apical periodontitis of the right maxillary central incisor. Periapical radiography confirm

Nonsyndromic bilateral multiple impacted supernumerary mandibular third molars: a rare and unusual case report.

A supernumerary tooth is that which is present additionally to the normal series and can be found in any region of the dental arch. An impacted tooth is defined as the one which is embedded in the alveolus, so that its eruption is prevented, or the tooth is locked in position by bone or the adjacent teeth. The occurrence of multiple supernumerary teeth in only one patient in the absence of an associated systemic condition or syndrome is considered as a rare phenomenon. The occurrence of supernumerary teeth in the lower molar region is rare. A prevalence of less than 2% of cases occurring in th

Treponema denticola in disseminating endodontic infections.

Treponema denticola is a consensus periodontal pathogen that has recently been associated with endodontic pathology. In this study, the effect of mono-infection of the dental pulp with T. denticola and with polymicrobial "red-complex" organisms (RC) (Porphyromonas gingivalis, Tannerella forsythia, and T. denticola) in inducing disseminating infections in wild-type (WT) and severe-combined-immunodeficiency (SCID) mice was analyzed. After 21 days, a high incidence (5/10) of orofacial abscesses was observed in SCID mice mono-infected with T. denticola, whereas abscesses were rare in SCID mice inf

The dental pathology of northern elephant seals (Mirounga angustirostris).

Skulls (n=104) of northern elephant seals from California were examined macroscopically. The animals varied in age but the numbers of each sex were roughly equal. The majority (86%) of teeth were available for examination. The mandibular first premolars were the most common teeth to be congenitally absent, with 2.3% missing. Supernumerary teeth (usually a supplemental mandibular molar) were associated with only 1.4% of normal teeth (or empty alveolar sockets). At least one persistent deciduous tooth was present in 38% of skulls, 70% of which were juvenile skulls. The majority (95.8%) of premol

Radiographic determination of the position of the maxillary lateral incisor in the cleft alveolus and parameters for assessing its habilitation prospects.

OBJECTIVE: Develop patient selection criteria and guidelines for the diagnostic work-up for early secondary alveolar bone graft designed to accommodate a lateral incisor in the cleft. DESIGN: A literature review with clinical examples. INTERVENTIONS: For the majority of cleft lip and palate (CLP) patients, the timing of the secondary alveolar bone graft procedure is devised to accommodate the eruption of the maxillary canine. However, when the lateral incisor is present in the cleft area, a decision may be made to perform early grafting in order to salvage an additional tooth. Appropriate ca

[Therapy refractory idiopathic thrombocytopenia. Contraindication for dental surgery interventions].

Hemorrhagic diathesis represents a high risk for oral surgery. In clinical practice various kinds of diathetic diseases are seen that are of plasmatic, vascular, thrombocytic and hypertensive origin. Usually these patients will be primarily referred to the hospital as high-risk cases. Therefore, a broad spectrum of diagnostic and therapeutic interdisciplinary methods are available, so that as a rule it is possible to obtain a clear picture of the severity of the bleeding tendency. We represent a case of a 58-year-old woman who had decayed teeth with chronic periodontal and periapical lesions,

Radiographic evaluation.

The bulk of the tooth, that is, the root and most of the periodontium, can be visualized only by means of radiographs. Consequently, a lot of pathology remains undiscovered if clinical examination does not involve radiography. Although lesions such as caries can be recognized without radiography, the full extent of the lesions, and whether pulpal and periapical involvement is present as a consequence, cannot be assessed. In other words, a clinical examination is not complete without radiography. Periodontal disease, endodontic disease, caries, resorptive lesions, fractures, bone pathology, and
