BACKGROUND: Cataract and corneal blindness continue to be leading causes of reversible blindness in India. These can co-exist in a multitude of pathologies such as trauma, healed keratitis (old herpetic scar), chronic degenerative changes such as labrador keratopathy, bullous keratopathy, corneal dystrophies etc. Phacoemulsification in such eyes is rewarding to the patient in terms of minimal intervention, less risk of complications owing to reduced open sky time (as in case of combined keratoplasty), and better predictable visual outcomes. Approach to such eyes with poor visualisation is high…

We describe an approach to phacoemulsification of complicated cataract with preexisting posterior capsule tear caused by an intravitreal injection. Careful preoperative planning and attention to fluidics, low bottle height, appropriate incisions, careful hydrodelineation without hydrodissection, avoidance of nuclear rotation, and use of a dispersive ophthalmic viscosurgical device to tamponade vitreous allows safe phacoemulsification with secure posterior chamber intraocular lens implantation. Biaxial microincision cataract surgery can achieve efficient removal of the lens matter without rotat…

Cataract is the most common cause of reversible blindness worldwide, accounting for approximately 50% of blindness worldwide. Cataract surgery is the most common surgical procedure performed in routine ophthalmic practice. It has undergone tremendous evolution, and the incision size has progressively reduced from 10-12 mm in extracapsular cataract surgery (ECCE) to 6-8 mm for manual small-incision cataract surgery (MSICS) and 2.2-2.8 mm in phacoemulsification. In a developing country like India, with a massive backlog of cataract, everyone cannot afford private surgery like phacoemulsification…

BACKGROUND: White cataracts pose substantial technical challenges during phacoemulsification because preoperative assessment of nuclear hardness is impossible and the risk of intraoperative complications is increased. The Eight-chop technique was developed to mechanically fragment the lens nucleus into eight pieces prior to ultrasound emulsification, potentially reducing intraocular stress. This study aimed to evaluate the safety and efficacy of the Eight-chop technique in eyes with white cataract and to compare surgical outcomes according to intraoperatively assessed nuclear hardness.

We performed light microscopic studies of after-cataract formation after phacoemulsification and out-of-the-bag IOL implantation in 23 rabbit eyes. One day after, epithelial cells at the equatorial area began to move to the pupillary area of the posterior capsule. A week later, we saw adhesion of the anterior capsular edge to the central posterior capsule with newly-formed lens fibers in the capsular space. These findings were similar to after-cataract formation after phacoemulsification only. We found no apparent effect of IOL on after-cataract formation when it was fixed out of the bag.

BACKGROUND: Cataract surgery is one of the most frequently performed eye surgeries worldwide, and among several techniques, phacoemulsification has become the standard of care due to its safety and efficiency. We evaluated the advantages and disadvantages of two phacoemulsification techniques: phaco-chop and divide-and-conquer.
METHODS: PubMed, Cochrane, Embase, and Web of Science databases were queried for randomized controlled trial (RCT), prospective and retrospective studies that compared the phaco-chop technique over the divide-and-conquer technique and reported the outcomes of (1) Endot…
