Ocular-induced abnormal head postures: A systematic review and analysis.

Abnormal head postures (AHPs) are frequently adopted as compensatory mechanisms by individuals affected by various ocular diseases to optimize the utilization of their visual field or alleviate symptoms such as diplopia. We review the causal relationship between ocular diseases and the resulting AHPs. An extensive literature search was conducted to identify and summarize relevant medical studies providing evidence of AHPs in patients diagnosed with conditions like Duane retraction syndrome, Brown syndrome, superior oblique palsy, and other ocular disorders. The analysis explores the proportion…

Digital twin framework for postural tachycardia syndrome and autonomic disorders.

Autonomic disorders, especially those characterized by orthostatic intolerance such as Postural Tachycardia Syndrome (POTS), remain diagnostically and therapeutically challenging due to their complex pathophysiology and limited access to specialized care. This paper proposes a conceptual framework for applying digital twin technology to POTS and other autonomic disorders. A digital autonomic twin-a dynamic, virtual replica of a patient's autonomic system-offers a transformative approach to understanding, predicting, and managing these conditions. A dynamic digital twin framework integrates mec

Autonomic Dysfunction and Postural Orthostatic Tachycardia Syndrome Secondary to Abuse.

Postural orthostatic tachycardia syndrome (POTS) and other forms of autonomic dysfunction are increasingly recognized in patients with histories of physical or psychological trauma. Emerging evidence suggests that trauma-related dysregulation of the autonomic nervous system may manifest as POTS, with hypervigilance and hyperadrenergic states proposed as potential mechanisms. However, the relationship between sexual abuse and subsequent autonomic dysfunction remains unclear. We present a series of patients referred to cardiology for suspected POTS, highlighting clinical features, potential mech

Developmental Trends in Postural Adjustments During Reaching in Early Childhood.

Reaching is a fundamental motor skill essential for daily living, with over 50% of daily activities involving reaching movements. Understanding the development of postural adjustments made during reaching in early childhood is crucial for motor development. This study investigated the developmental characteristics of postural adjustments made by children aged 3-6 years during reaching tasks. A total of 135 typically developing children from Chinese kindergartens participated in this study. Kinematic and electromyographic (EMG) data were collected using an infrared motion capture system and sur

Biological sex-dependent differences in postural orthostatic tachycardia syndrome.

AIMS: This prospective, cross-sectional study aimed to identify sex-based differences in diagnostic and symptom experiences in postural orthostatic tachycardia syndrome (POTS). METHODS AND RESULTS: Data from participants ≥ 16 years with physician-confirmed POTS enrolled in the Australian POTS registry between 1 May 2021 and 30 April 2024 were analysed. Health-related quality of life was assessed using the EuroQol 5 Dimension tool. Composite autonomic symptom score (COMPASS-31) assessed autonomic symptom burden. Self-reported sociodemographic and diagnostic journey data informed diagnostic exp

The influence of postural threat-induced anxiety on locomotor learning and updating.

The ability to adapt our locomotion in a feedforward (i.e., "predictive") manner is crucial for safe and efficient walking behavior. Equally important is the ability to quickly deadapt and update behavior that is no longer appropriate for the given context. It has been suggested that anxiety induced via postural threat may play a fundamental role in disrupting such deadaptation. We tested this hypothesis, using the "broken escalator" phenomenon: Fifty-six healthy young adults walked onto a stationary walkway ("BEFORE" condition, 5 trials), then onto a moving walkway akin to an airport travelat

Temporal adaptation of the postural control following a prolonged fin swimming.

PURPOSE: Postural control deteriorates following a transition between two environments, highlighting a sensory conflict when returning to natural conditions. Aquatic immersion offers new perspectives for studying postural control adaptation in transitional situations. Our aim is to study immediate and post-task static postural control adaptation on land after a prolonged fin swimming exercise in total immersion. METHODS: Standing static postural control was assessed in 14 professional or recreational SCUBA divers (11 men, 3 women; 33.21 ± 10.70 years), with eyes open and closed, before, immed

Benefits of postural sway to succeed in goal-directed visual tasks.

When individuals stand, they sway and so have to maintain their balance. It is generally expected that task performance is worse when standing and swaying than when sitting and therefore not swaying. In contrast, we hypothesized that greater sway is associated with better task performance in the absence of external perturbations of posture. Twenty-four healthy, young adults performed two goal-directed, modified Stroop tasks (incongruent and reversed incongruent) in four body position conditions (standing against a vertical surface, and standing freely with a wide, standard or narrow stance). C…

Hyperadrenergic Postural Tachycardia Syndrome: Clinical Biomarkers and Response to Guanfacine.

BACKGROUND: A subset of patients with postural tachycardia syndrome (POTS) are thought to have a primary hyperadrenergic cause. We assessed clinical biomarkers to identify those that would benefit from sympatholytic therapy. METHODS: We measured sympathetic function (supine muscle sympathetic nerve activity, upright plasma norepinephrine, and blood pressure responses to the Valsalva maneuver) in 28 patients with POTS (phenotyping cohort) to identify clinical biomarkers that are associated with responsiveness to the central sympatholytic guanfacine in a separate uncontrolled treatment cohort o

Novel pharmacotherapeutic options for the treatment of postural orthostatic tachycardia syndrome.

INTRODUCTION: Postural tachycardia syndrome (POTS) is a disorder characterized by a constellation of symptoms including lightheadedness, fatigue, and palpitations when upright, associated with an increase in the heart rate (HR) of > 30 beats per minute when changing from a lying down to standing position or head-up tilt position and not associated with orthostatic hypotension. The causes as well as the management of POTS are not quite fully understood. AREAS COVERED: We performed a literature review on the diagnosis and management of POTS, and this article includes an overview of novel pharma

Sex-dependent evolution of whole-body postural alignment with age.

PURPOSE: The goal of this study was to explore sex-related variations of global alignment parameters and their distinct evolution patterns across age groups. METHODS: This multicentric retrospective study included healthy volunteers with full-body biplanar radiographs in free-standing position. All radiographic data were collected from 3D reconstructions: global and lower limb parameters, pelvic incidence (PI) and sacral slope (SS). Lumbar lordosis (LL), thoracic kyphosis (TK) and cervical lordosis (CL) were also assessed as well as the lumbar and thoracic apex, and thoracolumbar inflexion po

Postural and spinal stability analysis for different floor sitting styles.

In contrast to Western countries, traditional floor-seating cultures are prevalent in Korea, Japan, the Middle East, and Africa, where sitting on the floor in static positions such as squatting, kneeling, or sitting cross-legged is common. Most studies on sitting posture have predominantly focused on chair sitting in Western cultures, resulting in a cultural bias. This study aimed to investigate the effects of different cushion types (floor and traditional cushions of 3-cm, 5-cm, and 8-cm thickness) and seating postures (cross-legged, mother's leg, and kneeling) on measures of postural stabili…

Effects of galvanic vestibular stimulation on postural righting reactions in hemiplegia.

Patients with post-stroke hemiplegia often exhibit reduced ability to maintain sitting balance, a crucial factor for predicting prognosis. Galvanic vestibular stimulation (GVS) influences postural control by stimulating vestibular organ. Although several studies have focused on GVS in static postures, no studies have demonstrated the influence of GVS on righting reactions. Therefore, we aimed to investigate the effects of GVS on postural righting reactions in seated patients with stroke-induced hemiplegia. Using a vertical board (VB), righting reactions were induced by tilting the VB at 10° af…

Postural Tremor Caused by Hirayama Disease Mimicking Essential Tremor.

BACKGROUND: Postural tremor is a common clinical situation. Timely and accurate diagnosis is essential for effective treatment. However, clinicians often encounter difficulties distinguishing between essential tremor and other etiologies due to overlapping symptoms and atypical features. CASE DESCRIPTION: A twenty-year-old man presented with a five-year history of progressive hand tremors. Neurological examinations were notable for asymmetric postural tremors in both hands, with mild distal finger muscle wasting and subtle kinetic tremors. NCS/EMG revealed neurogenic changes in the C7-C8 myot

Persistent postural-perceptual dizziness: A functional neuro-otologic disorder.

Persistent postural-perceptual dizziness (PPPD) is a functional neuro-otologic disorder that is the most frequent cause of chronic vestibular syndrome. The core vestibular symptoms include dizziness, unsteadiness, and non-spinning vertigo, which are exacerbated by an upright posture or walking, active or passive motion, and exposure to moving or complex visual stimuli. PPPD is mostly precipitated by acute or episodic vestibular diseases; however, its symptoms cannot be accounted for by its precipitants. PPPD is not a diagnosis of exclusion, but may coexist with other structural diseases. Thus,…

Dystonic posturing with athetoid movements in stroke without thalamic lesion.

Complex hyperkinetic movement disorders are a rare complication of stroke, frequently involving posterolateral contralateral thalamic lesions. One of the proposed mechanisms for these presentations is proprioceptive impairment, hence not involving deregulation of the basal ganglia-thalamocortical circuits. We report a patient who presented with dystonic posturing and athetoid movements with onset 2 years after right frontoparietotemporal stroke. Brain MRI showed no thalamic lesion. Based on the phenomenology, a diagnosis of pseudochoreoathetosis was proposed. To our knowledge, this is the firs
