Detained and Dying: Ethical Issues Surrounding End-of-Life Care in Prison.

Prisons are increasingly being called upon to provide end-of-life (EOL) care within the restrictive correctional environment. Several relatively recent phenomena have brought medical ethics to the forefront of prison EOL care-including aging behind bars, a paradigm shift in prison culture, the increasing rate of in-prison deaths, and the corresponding prison hospice movement. This article examines prominent ethical issues that emerge for prison personnel who are tasked with providing care to terminally ill offenders by presenting three offender composite characters that exemplify dying offende…

Contextualising the social capital of Australian Aboriginal and non-Aboriginal men in prison.

Social capital is a valuable resource that has received little attention in the prison context. Differences in the construct and accessibility of bonding, bridging, and linking social capital exist for Aboriginal Australians in mainstream society, but were previously unexplored in prison. This study seeks to understand contextual differences of social capital for Australian Aboriginal and non-Aboriginal men in prison. Thirty male inmates participated in qualitative interviews across three New South Wales (NSW) correctional centres. Interviews were completed between November 2014 and March 2015

Abstinence Social Support Among Ex-Prisoners With Substance Use Disorders.

The relationships between abstinence social support, substance use, and abstinence self-efficacy were examined among a sample of ex-prisoners exiting inpatient treatment for substance use disorders. Ordinary least squares regressions and bootstrapping procedure were used to test whether the relationship between abstinence social support and substance use would be mediated by abstinence self-efficacy. Significant relationships between variables were observed, and abstinence self-efficacy mediated the relationship between abstinence social support and substance use. Findings suggest that abstine

Impact of mass-screening on tuberculosis incidence in a prospective cohort of Brazilian prisoners.

BACKGROUND: Globally, prison inmates are a high-risk population for tuberculosis (TB), but the specific drivers of disease and impact of mass screening interventions are poorly understood. METHODS: We performed a prospective cohort study to characterize the incidence and risk factors for tuberculosis infection and disease in 12 Brazilian prisons, and to investigate the effect of mass screening on subsequent disease risk. After recruiting a stratified random sample of inmates, we administered a questionnaire to ascertain symptoms and potential risk factors for tuberculosis; performed tuberculi…

Offenders with Cognitive Deficits in a Canadian Prison Population: Prevalence, Profile, and Outcomes.

Impaired cognitive function has been associated with criminal behavior. In Canada it is unknown the extent to which this disorder affects federal inmates or its impact on key correctional outcomes. In this study, 488 incoming male offenders were assessed on the Cognistat, a neuropsychological screening tool. Twenty-five percent of offenders were found to have some level of cognitive deficit. Lower levels of educational achievement, unstable employment history, learning disability, serious alcohol problems, and symptoms of Attention Deficit Hyperactivity Disorder (ADHD) were significantly assoc

A robust estimate of the number and characteristics of persons released from prison in Australia.

OBJECTIVE: To estimate the number and characteristics of adults released from prison in Australia. METHOD: We calculated ratios, stratified by age, sex and Indigenous status, by comparing the number of persons released from prison in New South Wales (NSW), with the number in NSW prisons on 30 June of the corresponding year. These stratified ratios were applied to Australia-wide prison data to estimate the number and characteristics of persons released annually. RESULTS: We estimated that in 2013, 38,576 persons were released from prison in Australia - 25.3% more than the daily prison populat…

Buprenorphine dose induction in non-opioid-tolerant pre-release prisoners.

BACKGROUND: In a previously reported randomized controlled trial, formerly opioid-dependent prisoners were more likely to enter community drug abuse treatment when they were inducted in prison onto buprenorphine/naloxone (hereafter called buprenorphine) than when they received counseling without buprenorphine in prison (47.5% vs. 33.7%, p=0.012) (Gordon et al., 2014). In this communication we report on the results of the induction schedule and the adverse event profile seen in pre-release prisoners inducted onto buprenorphine. METHOD: This paper examines the dose induction procedure, a compar

The paradox of control: An ethnographic analysis of opiate maintenance treatment in a Norwegian prison.

BACKGROUND: Opiate maintenance treatment (OMT) is increasingly being offered in prisons throughout Europe. The benefits of OMT in prison have been found to be similar to those produced by OMT in community settings. However, prison-based OMT has been a controversial issue because of fear of the diversion of OMT medications and the development of black markets for prescription drugs such as buprenorphine and methadone. Prison-based OMT thus involves a delicate balance between the considerations of control and treatment. METHODS: This article reports on an ethnographic study of a prison-based OM…

A Domino Effect? The Spread of Implantation of Penile Foreign Bodies in the Prison System.

Subcutaneous penile insertion of foreign bodies is a practice performed globally but has mostly been reported outside of the United States. An incarcerated 29-year-old Caucasian male in a midwestern prison whittled a domino into a dog bone shape and placed it into his ventral penile subcutaneous tissue. He presented to our facility with erosion of the corners of the foreign body through his skin without evidence of infection. Self-insertion of foreign bodies into penile subcutaneous tissue by incarcerated American men for sexual enhancement is more widespread than previously reported. Erosion
