Comparative hospitalization risk for SARS-CoV-2 Omicron and Delta variant infections, by variant predominance periods and patient-level sequencing results, New York City, August 2021-January 2022.

BACKGROUND: Comparing disease severity between SARS-CoV-2 variants among populations with varied vaccination and infection histories can help characterize emerging variants and support healthcare system preparedness. METHODS: We compared COVID-19 hospitalization risk among New York City residents with positive laboratory-based SARS-CoV-2 tests when ≥98% of sequencing results were Delta (August-November 2021) or Omicron (BA.1 and sublineages, January 2022). A secondary analysis defined variant exposure using patient-level sequencing results during July 2021-January 2022, comprising 1-18% of we

Kinematic foot types in youth with equinovarus secondary to hemiplegia.

BACKGROUND: Elevated kinematic variability of the foot and ankle segments exists during gait among individuals with equinovarus secondary to hemiplegic cerebral palsy (CP). Clinicians have previously addressed such variability by developing classification schemes to identify subgroups of individuals based on their kinematics. OBJECTIVE: To identify kinematic subgroups among youth with equinovarus secondary to CP using 3-dimensional multi-segment foot and ankle kinematics during locomotion as inputs for principal component analysis (PCA), and K-means cluster analysis. METHODS: In a single ass

Video versus direct laryngoscopy for tracheal intubation in trauma: A secondary analysis of the DEVICE trial.

BACKGROUND: Endotracheal intubation is the most common airway intervention in the deployed combat setting, including far-forward Role 1 battalion aid stations. Intubation in this setting is often performed by novice intubators, adding to the complexity of this time-sensitive intervention. Two types of laryngoscopes are commonly used to perform tracheal intubation: a direct laryngoscope (DL) and a video laryngoscope (VL). Data to inform deployed clinical practice guidelines for the best device are currently lacking. METHODS: We performed a preplanned secondary analysis of the DEVICE trial-a mu

RNA Secondary Structurome Revealed Distinct Thermoregulation in Plasmodium falciparum.

The development of Plasmodium parasites, a causative agent of malaria, requests two hosts and the completion of 11 different parasite stages during development. Therefore, an efficient and fast response of parasites to various complex environmental changes, such as ambient temperature, pH, ions, and nutrients, is essential for parasite development and survival. Among many of these environmental changes, temperature is a decisive factor for parasite development and pathogenesis, including the thermoregulation of rRNA expression, gametogenesis, and parasite sequestration in cerebral malaria. How

Over-the-counter (OTC) medications to reduce cough as an adjunct to antibiotics for acute pneumonia in children and adults.

BACKGROUND: Cough is often distressing for patients with pneumonia. Accordingly they often use over-the-counter (OTC) cough medications (mucolytics or cough suppressants). These might provide relief in reducing cough severity, but suppression of the cough mechanism might impede airway clearance and cause harm. OBJECTIVES: To evaluate the efficacy of OTC cough medications as an adjunct to antibiotics in children and adults with pneumonia. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2011, Issue 3) which contains the Acute Respi

Patient Preference for Instructional Reinforcement Regarding Prevention of Radiation Dermatitis.

BACKGROUND: Although patient preference is a core value within the context of patient-centered models of care, little attention has been paid to determining patient preference for instructional media. Nurses have traditionally used verbal face-to-face instruction as the mainstay of patient education, with written materials being used extensively as teaching guides to supplement verbal instruction or for instructional reinforcement. However, advances in technology have made possible the adding of video instruction to nurses' repertoire of instructional media. OBJECTIVES: The purpose of this st

Peritoneal Metastases in Stage II-III Colon Cancer Represent a Blind Spot for Circulating Tumor DNA Surveillance.

While circulating tumor DNA (ctDNA) may guide adjuvant therapy in stage II-III colon cancer, detection largely hinges on systemic tumor shedding. In a secondary analysis of three prospective ctDNA studies, peritoneal metastases occurred in up to 9.4% of patients, were predominantly isolated, and were enriched among ctDNA-negative recurrences. Molecular and clinical data demonstrate reduced ctDNA levels in peritoneum-only disease, identifying a surveillance blind spot with direct implications for high-risk patients.

Baseline Body Mass Index and Anthropometric Responses to an Immersive Virtual Reality-Based Exercise Intervention: An Exploratory Secondary Analysis of a Randomized Controlled Trial.

Background/Objectives: Whether baseline body mass index (BMI) modifies the anthropometric response to immersive virtual reality (IVR)-based exercise interventions remains unclear. This exploratory secondary analysis of a randomized controlled trial evaluated whether baseline BMI was associated with heterogeneity in the effect of an IVR-based exercise intervention on post-intervention anthropometric outcomes in university students. The treatment effect modification analysis was not prespecified in the parent trial protocol or registry. Methods: Sixty-four participants were randomized to IVR or

Implementation strategy for an antibiotic stewardship bundle to promote optimal treatment choices in neonates with suspected early-onset sepsis (Protect-Neo): a study protocol for a multicentre, prospective interrupted time series and before-after study.

BACKGROUND: Several antibiotic stewardship interventions have been proven effective and safe for reducing the high number of antibiotic prescriptions in late preterm and term neonates at risk of early-onset sepsis (EOS). For successful translation of EOS interventions to clinical practice, implementation strategies should be employed targeting stakeholders. The primary aim of this study is to assess the impact of implementing an antibiotic stewardship bundle, including the EOS calculator, procalcitonin-guided therapy and intravenous-to-oral switch therapy on antibiotic exposure for EOS in Dutc

[Imaging characterization of renal masses. Image atlas].

The diagnostic study, analysis and characterization of renal masses and their behavior is one of the key elements for elaborating a surgical or therapeutic strategy, determining the prognosis, and for the follow-up of treatment efficacy in patients with benign or malignant disease. At present, computed tomography (CT) and magnetic resonance imaging (MRI) are the two most reliable, effective and efficient instruments in this context--offering sensitivity and specificity values in excess of 87%, with a diagnostic relaibility of over 90% in application to renal masses of a cystic or complex natur

Two cases of mucopolysaccharidosis type III (Sanfilippo). A biochemical study.

The mucopolysaccharide and lipid composition of human nervous tissue and viscera from one case of Sanfilippo disease type A and one case of Sanfilippo disease type C, were investigated. In the brain a moderate increase of acid glycosaminoglycans occurred. This phenomenon was much more pronounced in the viscera, especially in the liver. In all tissues this increase was mainly due to an accumulation of heparan sulphate. Changes in lipid composition were noted, but can be regarded as secondary effects. The biochemical results reported also suggest some general conclusions. (a) AGAG and lipid anal

Effect of temperature on binding of warfarin by human serum albumin.

The in vitro binding of warfarin by human serum albumin was studied at various temperatures and at pH 7.4 by a frontal gel filtration technique. The results can be best described in terms of a two class-of-binding site model, in which the numbers of primary and secondary sites are constrained to the average values for all experiments (n1 = 1.38 and n2 = 3.73). Analysis of the temperature dependence of the binding yielded the following thermodynamic parameters: deltaH1 =-2.55 kcal/mole, deltaS1=16.1 eu, and deltaF1=-7.34 kcal/mole for the primary binding and deltaH2=-5.08 kcal/mole, deltaS2=-1.
