Current and new pharmacotherapy options for non-alcoholic steatohepatitis.

INTRODUCTION: There is an unmet medical need for an effective anti-fibrotic treatment for NASH with advanced fibrosis. AREAS COVERED: The a … onists, as well as acetyl-CoA carboxylase inhibitors, are also expected. Among antidiabetic agents, semaglutide, a novel GLP-1 RA, is ongoing for NASH stage 1-3 fibrosis in a phase 2 trial. Furthermore, the combination of GLP-RA/glucagon receptor agonist and GLP-RA/gastrointestinal peptide agonist are promising future options.

GLP-1 Receptor Agonists in Dentistry: Promise, Pitfalls, and Evidence Gaps.

GLP-1RAs (glucagon-like peptide-1 receptor agonists) have become one of the fastest-growing drug classes worldwide, with prescriptions for o … based on pharmacovigilance analyses, including xerostomia, dysgeusia, and anosmia, particularly for semaglutide. Gastrointestinal side effects, particularly nausea (15% to 50%), vomiting (5% to 24%), and gastroesophageal reflux, pose a plausible but unquantified risk for tooth erosion and secondary caries. A bidirectional link between GLP-1 signaling and the …

Effect of GLP-1 receptor agonist on nutrient intake: A narrative review.

Humans have a primitive defense mechanism called metabolic adaptation that slows voluntary weight loss. It works by downregulating hormones that reduce food intake and by lowering the metabolic rate. Second-generation GLP-1 receptor agonists, such as semaglutide or tirzepatide, promote weight loss by targeting this hormonal system to control appetite. Appetite can be divided into hunger, satiety, satiation, and reward eating. This review will examine how GLP-1 receptor agonists and newer satiety hormone agonists influence appetite and its components. Research indicates that these new satiety h…

Reprogramming NAD+ homeostasis and FOXO3a-Nrf2 signaling mitigates bleomycin-driven pulmonary fibrosis.

Pulmonary fibrosis arises from intertwined oxidative, inflammatory, and profibrotic processes, whereas current therapies target only parts of this network. Here, we evaluated an adjunctive strategy in which nicotinic acid (NA), a NAD+-supporting supplement/adjuvant, was added to semaglutide (SEMA), a GLP-1 receptor agonist. The prespecified objective was to determine whether adding NA to SEMA provides greater protection than SEMA alone in bleomycin (BLM)-induced pulmonary fibrosis. Rats were challenged with BLM and treated with SEMA, NA, or SEMA+NA for 21 days. Biochemical, molecular, histolog…

Glucagon-like peptide-1 receptor analogues and beyond: emerging obesity pharmacotherapies.

Obesity is a chronic disease associated with multiple health risks. Multimodal treatments including lifestyle interventions, pharmacotherapies and bariatric surgery should be the standard of care for obesity management. Bariatric surgery remains the most effective treatment yielding to sustainable weight loss (WL) of about 20-30%. Having understood better the role of the gut-brain axis on appetite, the field of obesity pharmacotherapy has been advancing rapidly. The recent approvals for glucagon-like peptide-1 (GLP-1) receptor agonist (RA) semaglutide 2.4 mg and the dual GLP-1 and glucose-depe…

Metformin-Enhanced Digital Therapeutics for the Affordable Primary Prevention of Diabetes and Cardiovascular Diseases: Advancing Low-Cost Solutions for Lifestyle-Related Chronic Disorders.

Each year, over 1 million people in the United States die from diabetes and cardiovascular diseases (CVDs). These largely preventable chronic conditions also create a financial burden on patients, payers, and healthcare systems. The popularity of GLP-1-based management of cardiometabolic conditions can escalate healthcare spending, while incentivizing digitization of semaglutide (Ozempic, Wegovy), tirzepatide (Mounjaro), and others using the "prescription drug use-related software" (PDURS) framework. In this article, we highlight opportunities to advance digital-first interventions and metform…

Beyond Glycemic Control: Concurrent GLP-1 Receptor Agonist Use Is Associated with Reduced Urinary Adverse Events Following OnabotulinumtoxinA Treatment in Non-Diabetic Adults with Overactive Bladder.

Semaglutide, a GLP-1 (glucagon-like peptide-1) receptor agonist, is widely prescribed for weight loss in non-diabetic populations. Given the link between obesity and overactive bladder (OAB), we explored whether GLP-1 use would improve adverse urinary events beyond its weight loss benefit for non-diabetic adults undergoing onabotulinumtoxin A (BTX-A) treatment for OAB. Using the TriNetX database, we conducted a retrospective cohort study of non-diabetic OAB patients treated with BTX-A alone or with concurrent GLP-1 therapy. Propensity score matching (1:1) was adjusted for age, race, ethnicity,…

(Un)intended consequences of mass-prescribing GLP-1s.

It is difficult to overstate how much glucagon-like peptide-1 (GLP-1) pharmacotherapies have inundated the public discourse on obesity. An avalanche of scientific papers, news articles, and social media posts debate the efficacy, affordability, and appropriateness of GLP-1 receptor agonists not just for obesity but also skin disease, erectile dysfunction, pregnancy, and addiction. Attempting to cut through the noise, the World Health Organization (WHO) recently published the first-ever global guidelines recommending GLP-1s for the treatment of obesity, while committing to develop a global fram
