Several cases of Bancroftian filariasis were recently reported on Ouvea Island which is part of the archipelago of New Caledonia located in the south Pacific Ocean. Among a group of children hospitalized for "joint pain", four of the cases of reactive arthritis were due to Yersinia infection. These four cases are described. Some patients had suggestive clinical symptoms but serologic studies were needed to establish the diagnosis. Tissue group studies showed that two children were HLA B27 and one was HLA B7. More than four decades ago, Rickettsia australis was discovered to be the etiologic agent of Queensland tick typhus (QTT), yet many unanswered questions persist about the ecology, epidemiology, and clinical features of this disease. Two cases of diabetic patients with normal TCD4+ cell count with chronic diarrhoea and Cryptosporidium are described herein. In both cases serologic tests for HIV were negative. The fact that these patients developed a pathology usually seen in presence of low TCD4+ cell counts suggests that some immune defect other than cellular might be involved in the pathogenesis of this infection. Authors concluded that intentional search for Cryptosporidium should be considered in the study of the diabetic patient with chronic diarrhoea.

This is a rare case of extensive multifocal type of Kaposi's disease which developed in a young child. The patient's age, the major and progressive weight loss, the appearance of adenopathies and of a suspected tuberculous pleuritis, the evolution of cutaneous lesions extension, followed by death in short time (5 years) led us to suspect an eventual association with HIV infection. Serologic tests performed three times which remained negative and the absence of other important AIDS signs excluded this diagnostic.

Amebic hepatic abscess can mimic other hepatic disorders and is a commonly missed diagnosis. It occurs when Entamoeba histolytica infection of the cecum remains untreated and there is embolic spread to the liver. Symptoms include acute or insidious onset of fever, with or without chills, pain in the right hypochondrium or epigastrium with evidence of hepatomegaly, and intercostal or subcostal tenderness. Diagnosis is established by a combination of radiologic and nuclear scanning procedures, serologic tests and aspiration of the abscess.

Aspergillosis of the cerebral ventricles developed in a 36-year-old heroin abuser who died during the sixth week of illness. The diagnosis was not made during life. A review of our case and of four previous cases from the literature indicates that aspergillosis should be considered in parenteral drug abusers with the syndrome of chronic meningitis or ventriculitis. The diagnosis is difficult to establish, but serologic tests may be helpful. This form of CNS aspergillosis can be cured with antifungal drugs.

A recently recognized sporadic case of Legionnaires' disease occurring in North Carolina is described. Diagnosis was made by special serologic studies performed at the Center for Disease Control, Atlanta. The light-microscopic and electron-microscopic findings are described. Organisms consistent with the etiologic agents of Legionnaires' disease were seen by electron microscopic examination. An eight-month follow-up in this case still showed the presence of some radiologic abnormalities of the pulmonary parynchema, as well as abnormal data on tests of pulmonary function.

Current tests to confirm human T-cell lymphotropic virus (HTLV) infection in individuals at risk of retroviral infection commonly yield indeterminate results. The penile ulcerations of 100 consecutive men were tested for microorganisms. A polymicrobial flora was identified in the ulcers of 97 men. The microorganisms recovered from these ulcers included combinations of anaerobic and aerobic bacteria (including Mycoplasma), herpes simplex virus, yeasts, and filamentous fungi.
