The terrible triad of the shoulder.

This case shows that the terrible triad of the shoulder-anterior dislocation, brachial plexus injury, and rotator cuff tear-can occur in younger populations than previously described. A high index of suspicion for this triad of injury should be maintained, particularly in those young individuals with grossly unstable shoulders. Once axillary nerve function has begun to return, simultaneous repair of all soft tissue injuries can lead to a stable, functional joint.

Causes of severe injuries in wheelchair user motor vehicle passengers: pilot study for future safety restraint system.

BACKGROUND: As population aging progresses in Japan, an increasing number of older wheelchair users regularly travel in the rear space of motor vehicles. Poor seatbelt fit is the main cause of severe or fatal injuries in collisions. The current study sought to examine the potential risks regarding seatbelts for wheelchair users in Japan using national statistics, and to elucidate the mechanisms of injuries among wheelchair users involved in frontal collisions using sled tests. We propose a restraint system for wheelchair users traveling in vehicles. METHODS: To identify cases of collisions in

The Relationship Between Maximum Shoulder Horizontal Abduction and Adduction on Peak Shoulder Kinetics in Professional Pitchers.

BACKGROUND: Repetitive horizontal shoulder abduction during pitching can cause increased contact between the posterosuperior aspect of the glenoid and the greater tuberosity of the humeral head, theoretically putting baseball pitchers at increased risk of shoulder internal impingement and other shoulder pathologies. HYPOTHESIS: Increased shoulder horizontal abduction is associated with increased shoulder anterior force, while increased horizontal adduction is associated with increased shoulder distraction force. STUDY DESIGN: Descriptive laboratory study. LEVEL OF EVIDENCE: Level 4. METHOD

A Systematic Review and Meta-analysis of Injury in Crossfit.

The present study aims to investigate the incidence and pattern of injury in Crossfit. This study used a database search of Crossfit AND Injury for systematic review and metaanalysis. Crossfit seems to be an effective exercise method to decrease weight and BMI while improving function. High intensity interval training improves blood pressure, body fat percentage, lung capacity and pancreas function. Articles meeting inclusion showed that shoulder, back and then knee injuries were the most commonly selfreported in surveys. Incidence of injury is similar to that of other common recreational spor

Management of recent first-time anterior shoulder dislocations.

The management of a first episode of anterior shoulder dislocation starts with an analysis of the causative mechanism and a physical examination to establish the diagnosis. Based on the findings, the case can be classified as simple or accompanied with complications, most notably vascular or nerve injuries. Two radiographs perpendicular to each other should be obtained to confirm the diagnosis then repeated after the reduction manoeuvres. Additional imaging studies may be needed to assess concomitant bony lesions (impaction lesions or fractures). External reduction should always be attempted a

Clinical update: MR imaging of the shoulder.

Magnetic resonance imaging has become an important diagnostic adjunct in the evaluation of shoulder conditions, and the technology continues to evolve. Direct magnetic resonance arthrography can improve detection of labral and rotator cuff pathology, especially partial thickness tears of the rotator cuff. Special positioning, such as abducted-externally rotated views, improves visualization of the rotator cuff and posterior superior labrum in throwing athletes. Diagnosis-specific sequencing such as fat suppression, spin-echo and proton-density techniques, and higher power magnets (3.0 T) allow

An orthopaedic surgeon's guide to ultrasound imaging of the healthy, pathological and postoperative shoulder.

Ultrasound (US) imaging is an efficient, easy to use and inexpensive tool allowing for facilitated diagnosis and management of the painful shoulder. It remains primarily used by radiologists and rheumatologists, despite having shown excellent diagnostic accuracy when used by different medical specialities in their office-based consultation. It also has advantages over other imaging modalities in the evaluation of the postoperative shoulder for rotator cuff integrity and correct anchor and suture placement, as well as rotator cuff analysis following arthroplasty. Integration of US imaging into

Osseous Defects Seen in Patients with Anterior Shoulder Instability.

Shoulder surgeons need to be aware of the critical size of the glenoid or humeral osseous defects seen in patients with anterior shoulder instability, since the considerable size of osseous defect is reported to cause postoperative instability. Biomechanical studies have identified the size of the osseous defect which affects stability. Since engagement always occurs between a Hill-Sachs lesion and the glenoid rim, when considering the critical size of the Hill-Sachs lesion, we have to simultaneously consider the size of the glenoid osseous defect. With the newly developed concept of the gleno

MRI of Little Leaguer's shoulder.

The MRI appearance of 'Little Leaguer's shoulder' has not been previously reported in the radiology literature. Purported etiologies include proximal humeral epiphyseolysis, osteochondrosis of the proximal humeral epiphysis, stress fracture of the proximal humeral epiphyseal plate, and rotational stress fracture of the proximal humeral epiphyseal plate. We describe magnetic resonance imaging findings in four patients and review the literature.

[Arthroscopic treatment of traumatic anterior glenohumeral instability].

The main pathology in traumatic anterior instability of the shoulder is a Bankart lesion and capsuloligamentous laxity. Success in the treatment of glenohumeral instability mainly relies on an anatomical attachment of the fibrocartilaginous labrum and restoration of tension of the capsuloligamentous structures. Suture anchors are preferred for the fixation of soft tissues. Excessive capsular laxity can be treated by capsular plication or thermal capsulorrhaphy. Arthroscopy enables examination and repair of the glenohumeral structures, with several advantages over open techniques, including les

Predictors of Clinically Significant Radiographic Shoulder Pathology in the Emergency Department.

BACKGROUND: Although there are no clinical decision rules for radiograph use among persons with shoulder pain, they are ordered for most patients. Previously published reviews have demonstrated that radiography is overutilized in evaluating emergency department (ED) patients with shoulder pain, and clinical factors might define patients in whom plain film radiography need not be performed. OBJECTIVES: The objectives of this study were to identify predictors of clinically significant shoulder pain and develop a clinical decision radiograph-ordering rule for adult ED patients with shoulder pain

Prevalence of the anterosuperior capsulolabral anatomical variations and their association with pathologies of the glenoid labrum: a systematic review and meta-analysis.

PURPOSE: Differentiating the anatomical variations of the anterosuperior portion of the glenoid labrum from pathologies is important to avoid unnecessary iatrogenic complications resulting from inaccurate diagnosis. Additionally, the presence of several variations was reported to be conductive to lesions involving the glenoid labrum. Thus, the aim of this study was to state the prevalence rates of the sublabral recess, sublabral foramen, and the Buford complex, and to verify their association with labral lesions. METHODS: Systematic search of electronic databases was conducted to gain potenti

Superior labral anterior and posterior (SLAP) lesions of the long bicep insertion on the glenoid: management in athletes.

INTRODUCTION: SLAP injuries are common in athletes but there is no consensus on different aspects such as pathophysiology and treatment options. Currently, the main controversy in the treatment of SLAP II injuries is deciding whether to make a repair or tenodesis. Clinical outcomes have varied according to the patients' age, sports, or work activity. This review aims to present the evidence at points of contention regarding pathophysiology, treatment options, outcomes, return to activities, and complications of type II SLAP. MATERIAL AND METHODS: The relevant literature on SLAP injuries and t
