A biomechanical analog of curve progression and orthotic stabilization in idiopathic scoliosis.

A biomechanical analog of curve progression and orthotic stabilization in idiopathic scoliosis has been developed using the classical theory of curved beam-columns. The interaction of the spinal musculature and other supporting structures is incorporated in the model using an equivalent flexural rigidity. The stability of a given scoliotic curve relative to a normal spine is described in terms of the so-called critical load ratio (Pc/Pe). This dimensionless quantity appears in the exact solution of the governing differential equation and boundary conditions. It is defined as the ratio of the l

Scoliotic posture and biological age measured by pubertal growth spurt in adolescent boys and girls.

Puberty is a critical period in the development of children and adolescents. The muscular dystonia observed during this period and the environmental factors present at home and school promote the adoption of abnormal posture and impaired physical performance. Early detection of postural changes provides opportunities for prevention and treatment. The aim of this study was to compare the body posture and the size of the rotational deformation of the trunk in children aged 10-15 years, before and after pubertal growth spurt. The study included 40 subjects, 19 boys and 21 girls from Wroclaw, Pola

The use of patient-reported preoperative activity levels as a stratification tool for short-term and long-term outcomes in patients with adult spinal deformity.

OBJECTIVE Given the recent shift in health care toward quality reporting requirements and a greater emphasis on a cost-quality approach, patient stratification with respect to long-term outcomes and the use of health care resources is of increasing value. Stratification tools may be effective if they are simple and evidence based. The authors hypothesize that preoperative patient-reported activity levels might independently predict postoperative outcomes in patients with adult spinal deformity. METHODS This is a retrospective cohort. A total of 575 patients in a prospective adult spinal deform

Pathogenesis, presentation, and treatment of lumbar spinal stenosis associated with coronal or sagittal spinal deformities.

Sagittal- or coronal-plane deformity considerably complicates the diagnosis and treatment of lumbar spinal stenosis. Although decompressive laminectomy remains the standard operative treatment for uncomplicated lumbar spinal stenosis, the management of stenosis with concurrent deformity may require osteotomy, laminectomy, and spinal fusion with or without instrumentation. Broadly stated, the surgery-related goals in complex stenosis are neural decompression and a well-balanced sagittal and coronal fusion. Deformities that may present with concurrent stenosis are scoliosis, spondylolisthesis, a

From 3 to 8 clusters: a machine learning-based classification of operated adult spinal deformity patients.

BACKGROUND CONTEXT: Adult spinal deformity (ASD) is a heterogeneous condition encompassing diverse etiologies, clinical presentations, and surgical challenges. While previous unsupervised learning models have stratified ASD patients into 3 broad phenotypes, these often lack clinical granularity for surgical decision-making. PURPOSE: To refine the classification of operated ASD patients using unsupervised machine learning and assess whether expanding from 3 to 8 clusters improves clinical discrimination and predictive robustness. Development of a preoperative predictive model for major complications following adult spinal deformity surgery.

OBJECTIVE The operative management of patients with adult spinal deformity (ASD) has a high complication rate and it remains unknown whether baseline patient characteristics and surgical variables can predict early complications (intraoperative and perioperative [within 6 weeks]). The development of an accurate preoperative predictive model can aid in patient counseling, shared decision making, and improved surgical planning. The purpose of this study was to develop a model based on baseline demographic, radiographic, and surgical factors that can predict if patients will sustain an intraopera

Characterization of T9-T10 spinal neurons with duodenal input and modulation by gastric electrical stimulation in rats.

Gastric electrical stimulation (GES) has been suggested as a therapy for patients with gastric motility disorders or morbid obesity. However, it is unclear whether GES also affects intestinal sensory and motor functions. Furthermore, little is known about intraspinal visceroreceptive transmission and processing for duodenal afferent information. The aims of this study were to characterize responses of thoracic spinal neurons to duodenal distension, to determine the afferent pathway and to examine the effects of GES on activity of these neurons. Extracellular potentials of single T9-T10 spinal
