A new model for experimental tendon adhesions in the chicken.

A minimally invasive model using a manual abrader to induce adhesions in the chicken's central digit is described. The flexor synovial sheath and the profundus tendon were abraded with access through small flaps at the level of the proximal and distal phalanges of the avian long toes. The birds were divided into two groups according to the severity of the induced trauma. Group I birds received an abrasion injury and were euthanized to allow biomechanical testing 5 weeks postoperatively. Group II birds had a more severe abrasion and were euthanized similarly and tested 5 weeks after surgery. Re

Repair of the flexor pollicis longus tendon in children.

This is a retrospective study of primary repairs of flexor pollicis longus in 16 children under 15 years of age. Patients with injuries to the median or ulnar nerve at the wrist, crush injuries, skin loss or fracture were excluded. Repairs were carried out within 24 hours using a modified Kessler technique. The mean follow-up was for two years. The final results were evaluated using the criteria of Buck-Gramko and Tubiana. They were good or excellent in all except one patient who had a secondary tendon rupture. When compared with the non-injured thumb, however, there was a significant decrease

Electrophysiological recording of tendon reflexes in cervical myelopathy.

Twenty-six patients with unequivocal, clinically and radiologically confirmed cervical myelopathy were investigated electrophysiogically using needle electromyography (EMG), motor and sensory conduction velocity, F-wave latency and the electromyographic recording of the tendon reflexes (TR) of the biceps brachii, brachioradialis and triceps brachii muscles. Normal latency and amplitude values of the reflex evoked compound muscle potentials (T-wave) were calculated from 74 healthy volunteers. Pathological TR were found in 73.1% of the patients, while EMG--which was the next more effective metho

Bilateral ossification in the Achilles tendon: a case report.

Bilateral symmetrical Achilles tendon ossification is presented, with a fracture on one side. A 57 year old male patient was conservatively treated for bilateral clubfeet in his childhood. Part of his Achilles tendons were replaced by ectopic bony mass and on the left it fractured without any trauma. The fracture was treated surgically by vertical thinning, intraosseal suture "osteosynthesis", and tendon flap plasty. This pathological condition healed in three months. Seven years after the operation the X-ray revealed total bony rebuilding. The ectopic bone mass has the same biological behavio…

[Injuries of the tibialis posterior tendon: diagnosis and therapy].

Complete traumatic rupture of the tibialis posterior tendon is absolutely rare. From the scarce case reports in the medical literature a extreme pronation-abduction or pronation-external rotation mechanism according to the Lauge-Hansen classification can be presumed, leading to a malleolar fracture because of forced pronation, external rotation and dorsiflexion of the foot. With primary suture the prognosis is favorable. Traumatic dislocations, mostly with luxatio pedis sub talo, are treated by atraumatic reduction and refixation of the retinaculum. Again, the prognosis is favorable. Incomplet

Intracapsular origin of the long head of the biceps tendon.

A developmental anomaly of the long head of the biceps tendon was found in a cadaveric shoulder. Findings on arthroscopy, routine MR imaging, and MR arthrography were compared and correlated with results of anatomic dissection. MR arthrography appears to be a very good diagnostic imaging method for depicting this anomaly prior to arthroscopy.

Treatment of flexor tendon injuries: surgeons' perspective.

Medical researchers continue to explore the flexor tendon's response to injury and repair. In recent years, hand surgery and therapy publications have focused on the biomechanics of suture techniques and the benefits of early postoperative motion on surgically repaired flexor tendons. Laboratory and clinical studies have shown that stronger suture techniques can withstand the strain of immediate active motion without a significant risk of tendon rupture or gap formation. Newly proposed therapy techniques and anatomic studies defining the effects of wrist and digital position on tendon excursio…

Limb salvage with Chopart's amputation and tendon balancing.

For several decades, Chopart's amputation has met with some skepticism owing to reports of significant equinus deformity developing soon after the procedure is performed. However, with appropriate tendon balancing, which generally includes anterior tibial tendon transfer and tendo Achillis lengthening, this level of amputation is often more functional than slightly more distal amputations, such as Lisfranc or short transmetatarsal amputations. The authors offer a rationale for this observation, which includes a discussion of the longitudinal and transverse arch concept of the foot. This concep

Pedal giant cell tumor of tendon sheath.

The authors present a brief review of giant cell tumor of tendon sheath and three case reports. A discussion emphasizing the histologic characteristics of this lesion demonstrates the benign-to-malignant variability of these neoplastic growths. Special attention is directed to a case with aggressive histologic characteristics. Reexcision after surgery should be considered in cases where microscopic examination reveals a lesion with characteristics suggestive of potentially aggressive behavior.

Nonoperative treatment of posterior tibial tendon dysfunction.

One of the most common causes of acquired flatfoot deformity in adults is dysfunction of the posterior tibial tendon. The main function of the posterior tibial tendon is to invert the midfoot and lock the transverse tarsal joints (talonavicular and calcaneocuboid joints). When the tendon fails to function properly, a progressive flatfoot deformity develops. Because the disease process is a continuum, a staging system has been devised to offer guidelines for nonoperative and operative treatment of this problem. The rationale for nonoperative treatment of this disorder is to support the longitud

The outcome of tendon transfers for C6-spared quadriplegics.

The purpose of this study was to review retrospectively and evaluate a uniform group of C6-spared quadriplegics who had similar surgical procedures. Eight patients undergoing 12 procedures were reviewed at an average of 3.8 years follow-up. There were three bilateral procedures. All patients had extensor carpi radialis longus to flexor digitorum profundus and brachioradialis to flexor pollicis longus transfers to improve grip strength and key pinch. All patients reported subjective improvements in quality of life, activities of daily living and patient-centred goals. There were six excellent a

Biomechanical study of tibialis anterior tendon transfer.

The tibialis anterior tendon transfer has been used in the treatment of recurrent congenital clubfoot and paralytic equinovarus foot deformities in cerebral palsy. This study attempts to determine the optimal site of tibialis anterior tendon insertion for ankle and foot motion and to compare the split and whole tendon transfer. Ten fresh normal anatomic leg specimens were used. The lateral half of the tibialis anterior tendon was detached from its insertion, passed beneath the extensor retinaculum, and anchored to the appropriate tarsal bone by a barbed staple. Tension was applied, and ankle a

Closed tendon injuries of the hand and wrist in athletes.

Closed-tendon injuries of the hand and wrist are very common in the athletic population. Most of these injuries, if seen acutely, can be treated successfully with nonoperative management, although some do require operative treatment. It is those injuries, however, that go undiagnosed until the end of the season, which may result in permanent disability.

Recurrent dislocation of the extensor carpi ulnaris tendon.

Five patients with recurrent dislocation of the extensor carpi ulnaris (ECU) tendon resulting from an athletic injury were treated by reconstruction of the ECU tendon sheath, and each had a satisfactory result. Two types of disruption of the fibro-osseous sheath were found. In two cases in which the fibro-osseous sheath ruptured radially, the torn sheath lay on its ulnar groove beneath the ECU tendon. These patients were treated by direct suture of the sheath over the ECU tendon. In three cases in which the fibro-osseous sheath ruptured ulnarly, the torn sheath lay superficial to the ECU tendo

Arthroscopic absence of the long head of the biceps tendon.

Absence of the long head of the biceps tendon was detected during arthroscopic evaluation of shoulder pain in a weight-lifter. Magnetic resonance imaging and ultrasound showed the presence of a hypoplastic tendon with altered depth and width of the bicipital groove. The tendon-glenoid insertion shown by magnetic resonance imaging was apparently normal.

Dislocation of the tibialis posterior tendon: diagnosis and treatment.

Traumatic dislocation of the tibialis tendon occurred from minor ankle sprains in a 37-year-old male and a 53-year-old female. Both complained of local pain at the medial malleolus, and both walked with a limp. The diagnosis was suspected by clinical examination, in one case with 2 months' delay, and verified by ultrasound, computed tomography, and magnetic resonance imaging. The male patient was initially treated for an "uncomplicated ankle sprain." For various reasons surgery was delayed 4 months. During this interval the male patient complained of pain and severe dysfunction, requiring anal

Extensor tendon rupture due to Kienböck's disease.

Extensor tendon rupture as a complication of Kienbock's disease is rare. We report a case of attritional rupture of the extensor tendon to the index finger by a comminuted dorsal fragment of the lunate after Kienböck's disease in an elderly woman. Excision of the fragment and extensor reconstruction with tendon graft led to a favourable result.
