Apical fibrobullous lung disease in ankylosing spondylitis: case report and literature review.

Ankylosing spondylitis (AS) is associated with several unique pulmonary manifestations such as apical fibrobullous disease (AFBD), which is a rare extra-spinal complication, predominantly occurring in advanced disease. Infectious complications and differential diagnosis of cavitated lung lesions may be challenging, particularly in patients already submitted to immunosuppression. In this report, we present a low body-mass-index 47-year-old male patient, ex-smoker, with AS and severe joint involvement, medicated in the past with anti-TNF-α therapy, who was diagnosed with AFBD and developed pulmo

A Case of Tuberculous Meningitis with Atypical Multiple Lesions.

This case describes a rare case of tuberculous meningitis. A 50-year old female presented with seven days of numbness on the left side, fatigue and a three-day headache. Magnetic resonance imaging (MRI) showed multiple lesions in the dorsal medulla and upper cervical spinal. After admission, she developed a long-term fever, cranial nerves palsy and showed little response to corticosteroid, antibacterial and antiviral therapy. She received a diagnostic anti-tuberculous therapy (ATT); despite that, all examinations for tuberculosis were negative. After ATT lasting 16 days, she recovered and was

Adrenomyeloneuropathy.

A 29-year-old male patient presented with progressive spastic paraparesis of three years duration. He also had gait ataxia which led to recurrent falls. In addition, there was pigmentation of the skin creases, tongue and buccal mucosa. His clinical course was remarkable by recurrent episodes of diarrhea, pulmonary tuberculosis. The investigatory work up showed a normal MRI scan of the brain, spinal cord and normal abdominal structures. The basal serum cortisol levels were low. Adrenomyeloneuropathy was diagnosed and he was started on corticosteroid supplementation. Mineralocorticoid supplement

[Osteoid osteoma of the vertebral column].

The peculiarities of osteoid osteoma of the spine occurring most frequently during adolescence are pointed out and contrasted to the benigne osteoblastoma, based on 5 observations and on studies of the literature. The nidus is usually located excentrically and exceptionally even intrarticular. Two foci may occur side by side or one after another, and the adjacent vertebral end plates may be involved. Spontaneous subsiding of pain is possible. The dorsal portions of the lumbar spine are diseased occasionally under the clinician picture of tight hamstrings, most often, however, with a painful re

Challenges encountered in managing tuberculosis in children: a case report.

BACKGROUND: The management of pediatric tuberculosis involves several challenges, as illustrated in these three cases. Challenges are numerous and each case demonstrates the difficulty and delay in diagnosis, social constraints that delay the diagnosis, and drug dosing schedules that cause serious side effects in children. All three cases received the Bacillus Calmette-Guérin vaccine at birth, and each has documented confirmation of vaccination. CASE PRESENTATION: A 1-year-old Sri Lankan Muslim baby was transferred to a tertiary care center with a week-long fever of unknown origin, accompanie

Single Posterior Surgery Versus Combined Posterior-Anterior Surgery for Lumbar Tuberculosis Patients.

OBJECTIVE: Posterior approach of debridement, interbody graft, and instrumentation, and combined posterior-anterior approach of posterior instrumentation and anterior debridement and interbody graft are two essential surgeries for the surgical treatment of spinal tuberculosis (TB), and, until now, which one should be chosen is still controversial. This study aimed to compare the therapeutic efficacy between the single posterior surgery and combined posterior-anterior surgery for lumbar tuberculosis (LTB) patients to elucidate the role of debridement and the effects that result from posterior s

Posterior Vertebral Column Resection With Intraoperative Manual Retraction for the Treatment of Posttubercular Kyphosis in Upper Thoracic Spine or Cervicothoracic Junction.

STUDY DESIGN: This is a case-series. OBJECTIVE: To evaluate the clinical and radiologic outcomes of posterior vertebral column resection (PVCR) for treatment of posttubercular kyphosis in upper thoracic spine and cervicothoracic junction (CTJ). SUMMARY OF BACKGROUND DATA: Surgical management of severe posttubercular kyphosis in upper thoracic spine or CTJ is challenging. A new technique that combines PVCR and intraoperative manual traction in a single procedure, was developed to maximize the correction rate and minimize the risk of dural buckling and spinal cord injury. However, the safety a

[Tuberculous spondylitis of vertebra with fracture, paraparesis and pleural empyema complications--case report].

INTRODUCTION: Spine tuberculosis is caused by Micobacterium tuberculosis. It is localized in the vertebral body or intervertebral disc. Its diagnosis is often delayed because of nonspecific symptoms and neglected presence of tuberculosis, which leads to serious complications. CASE REPORT: This paper presents a case of tuberculous spondylitis, which was complicated with the fracture of vertebra, paraparesis of lower extremities and pleural empyema. The treatment with antituberculous drugs started after the fracture of 10th and 11th thoracic vertebras. The therapy brought some improvement but p

Late onset Pott's paraplegia in patients with upper thoracic sharp kyphosis.

PURPOSE: The purpose of this study was to determine the clinical results of patients with late onset upper thoracic sharp Pott's kyphosis and to predict the prognosis for Pott's paraplegics. METHOD: The study included five patients who developed late onset upper thoracic (T1-T4) sharp Pott's kyphosis/kyphoscoliosis within a period from 19 to 37 years after the active disease was healed. The kyphosis angle of the patients ranged from 95° to 105°. Among them, three patients suffered onset of paraplegia ranging from 26 to 31 years after spinal tuberculosis was healed. The duration of neurologica

[Surgical treatment for tuberculous spondylitis; a case report].

A 64-year-old woman was admitted with the chief complaints of severe back pain. Chest X-ray film on admission showed an abnormal mass lesion in the right upper mediastinum. Chest tomography and chest CT films revealed destruction of thoracic vertebrae (Th 2 and 3) and paravertebral abscess. Chemotherapy containing RFP was started under diagnosis of active tuberculous spondylitis of thoracic vertebrae. Three months later, curative operation for spondylitis were performed. She underwent the resection of necrotic bone and the anterior spinal fusion using transplanted bone autograft. Her post-oper
