BACKGROUND: Therapeutic options for complex anal fistula (CAF) are limited. Video-assisted anal fistula treatment (VAAFT) allows examination of these anatomically complex fistulae from within. The aim of the present study was to evaluate outcomes of VAAFT for a series of CAF.

PURPOSE: There is no systematic description of primary anatomical landmarks that allow a surgeon to reliably and safely navigate the superior and posterior mediastinum's fat tissue spaces near large vessels and nerves during video-assisted endothoracoscopic interventions in the prone position of a patient. Ectopic parathyroids can often present a diagnostic and therapeutic conundrum for clinicians. Mediastinal parathyroid adenomas are usually small and located anteriorly within thymic tissue. To our knowledge, this is the first reported case of a large cystic parathyroid adenoma presenting as a 6-cm posterior mediastinal mass. After a successful thoracoscopic resection, parathyroid hormone levels normalized and the patient was discharged home on postoperative day 2.

A 49-year-old woman presented after a respiratory infection with an abnormal chest roentgenogram demonstrating a cystic calcified mass at the base of the right lung. A chest computed tomographic angiogram demonstrated that the blood supply arose from the abdominal aorta. This extralobar sequestration was surgically resected using video-assisted thoracoscopy without complication. The pathology report showed a cystic hamartoma. This case highlights the importance of preoperative evaluation of the blood supply of suspected sequestrations and the very rare disorder that was found.

AIM: To analyze long-term own experience of NPE treatment in view of evolution of surgical sanitation of pleural cavity.

The results of examination and treatment of 47 patients with pleural effusion of various origins on partialy obliteration pleural cavity was analysed. The efficiency SITS-technology (single-incision thoracoscopic surgery) use in the diagnosis and treatment of patients was proved. When using SITS-technology pleural exudation severity and duration decreased to (27 ± 9)%. Reduce operation trauma and pain intensity and better cosmetic effects with SITS-technology interventions was established.

Although thoracoscopic lobectomy for severe bronchiectasis has been reported in children, this is the first report of double lobectomy of the right middle and lower lobes performed using the video-assisted thoracoscopy in a 9-year-old girl. Giant cell granuloma is an uncommon bony benign lesion that generally involves the mandible and maxilla. It may be locally aggressive and result in extensive tissue destruction in advanced cases. Surgery is the traditional and still the most accepted treatment for giant cell granuloma. The ideal operative technique for thymectomy in myasthenia gravis remains controversial.
