Implementing computer‑aided detection for TB active case finding: A qualitative study of feasibility and acceptability in Tondo, Manila.

Tuberculosis (TB) remains the leading cause of death from a single infectious agent, with over 1.23 million deaths globally in 2024. Despite available treatment and prevention strategies, timely and accurate detection remains challenging, particularly in high-burden settings such as the Philippines, which accounted for 6.8% of the global burden of TB in 2024. Computer-aided detection (CAD) software integrated with digital chest X-ray offers enhanced screening capacity, but its success depends on feasibility and acceptability within local health systems. This qualitative study explored percepti

International migration of Africa-born health workers: a scoping review of drivers and policies, 2010-2024.

BACKGROUND: Africa faces a critical shortage of human resources for health (HRH), crucial for improving population health. We described drivers of and policies to manage the migration of Africa-born HRH from source (African) countries in Africa to destination (non-African) countries. METHODS: We conducted a scoping review, using the Arksey and O'Malley framework, using data from peer-reviewed articles (2010-2024) and gray literature from institutions like Africa Centers for Disease Control (CDC) and World Health Organization (WHO). We used push-pull theory to identify economic, professional,

Disease diagnosis and arthroplasty type are strongly associated with short-term postoperative patient-reported outcomes in patients undergoing primary total shoulder arthroplasty.

BACKGROUND: Prognostic factors for total shoulder arthroplasty (TSA) clinical outcomes are incompletely understood. This study investigates the associations of preoperative patient, disease-specific, and surgical factors with 1-year postoperative PENN Shoulder Score (PSS) in patients undergoing primary TSA. METHODS: Cleveland Clinic patients undergoing primary anatomic TSA (aTSA) or reverse TSA (rTSA) for glenohumeral osteoarthritis (GHOA) or rotator cuff tear arthropathy (CTA) between February 2015 and August 2019, and having complete preoperative and 1-year postoperative patient-reported ou

Are emojis ready to promote the WHO 5 moments for hand hygiene in healthcare?

BACKGROUND: Hand hygiene is universally recognized as a cornerstone measure for the prevention of healthcare-associated infections. Although the WHO "My five Moments for hand hygiene" poster has been used for more than a decade to delineate hand hygiene indications and promote action, adherence levels among healthcare workers are still notoriously low and disquieting. To compensate for the lack of effective hand hygiene communication, we aimed to evaluate emojis as possible surrogates for the non-verbal aspects of hand hygiene behaviour. METHODS: Following a thorough review of the Unicode ver

The Effect of Intra-articular Cocktail Versus Femoral Nerve Block for Patients Undergoing Hip Arthroscopy.

PURPOSE: To compare clinical efficacy and complication rate as measured by postoperative falls and development of peripheral neuritis between intra-articular blockade and femoral nerve block in patients undergoing arthroscopic hip surgery. METHODS: An institutional review board approved retrospective review was conducted on a consecutive series of patients who underwent elective arthroscopic hip surgery by a single surgeon, between November 2013 and April 2015. Subjects were stratified into 2 groups: patients who received a preoperative femoral nerve block for perioperative pain control, and

The dangers of accepting a single diagnosis: case report of concurrent Plasmodium knowlesi malaria and dengue infection.

BACKGROUND: Dengue and malaria are two common, mosquito-borne infections, which may lead to mortality if not managed properly. Concurrent infections of dengue and malaria are rare due to the different habitats of its vectors and activities of different carrier mosquitoes. The first case reported was in 2005. Since then, several concurrent infections have been reported between the dengue virus (DENV) and the malaria protozoans, Plasmodium falciparum and Plasmodium vivax. Symptoms of each infection may be masked by a simultaneous second infection, resulting in late treatment and severe complicat

Systematic review of cost and cost-effectiveness of different TB-screening strategies.

BACKGROUND: Interferon-γ release assays (IGRAs) for TB have the potential to replace the tuberculin skin test (TST) in screening for latent tuberculosis infection (LTBI). The higher per-test cost of IGRAs may be compensated for by lower post-screening costs (medical attention, chest x-rays and chemoprevention), given the higher specificity of the new tests as compared to that of the conventional TST. We conducted a systematic review of all publications that have addressed the cost or cost-effectiveness of IGRAs. The objective of this report was to undertake a structured review and critical app

Gout: a practical approach to diagnosis and management in low- and middle-income countries and resource-limited settings.

The prevalence of gout is increasing worldwide, with rapid rises reported in low- and middle-income countries (LMICs), particularly in parts of Asia, Latin America, and Africa. Several international organizations have published guidelines for optimal gout management, most of which are from developed countries with high-income settings. Therefore, implementing these recommendations in LMICs and other resource-limited settings (RLS) is often difficult due to several barriers. These include a shortage of rheumatologists and other relevant specialists, workforce constraints, limited access to adva

Provider costs of Antiretroviral therapy (ART) in Zimbabwe: The value of using time-driven activity based costing methods in a low resource setting.

Although ART has transformed HIV into a manageable chronic condition, significant cost and logistical challenges persist, threatening progress toward the UNAIDS 95-95-95 targets. Budget allocation to the health sector declined by over 30% in the last decade in Zimbabwe, attributed to donor fatigue and emergence of pandemics. The time-driven activity-based costing (TDABC) method was used to estimate the provider costs of ART and inform resource allocation for sustained ART programming. A descriptive cross-sectional study in 11 facilities across Zimbabwe's four levels of care collected data usin

A cross-sectional survey of community health workers and their roles in Indiana.

BACKGROUND: In the US, there is a lack of information about the community health worker (CHW) workforce, which is critical to inform workforce policy, development, and training initiatives. In this manuscript, we report the results of a cross-sectional workforce survey of CHWs in Indiana. METHODS: We conducted a statewide electronic survey of CHWs in Indiana. Eligible participants were self-reported CHWs and over the age of 18. The survey was adapted from an existing tool and included questions about CHWs' demographics, employer, compensation, and roles and responsibilities. The survey was pi

Rejected sickness cash benefit claims after 180 days of sick leave in the Swedish rehabilitation chain: A nationwide register-based study.

AIM: Since a lack of financial security among vulnerable groups could further hamper health and well-being, this study scrutinises factors predicting rejected prolonged sickness cash benefit claims among people on compensated sick leave of more than 180 days with a rejection between days 181 and 365. METHODS: All 246,872 claims for employed people on sick leave recorded in the Swedish official statistics register between January 2018 and June 2021 were analysed. Claim outcome was evaluated using logistic regression with odds ratios recalculated to relative risks (RR) with 95% confidence inter

Predictors of Meniscal Allograft Transplantation Outcome: A Systematic Review.

This systematic review investigates factors associated with outcomes after meniscal allograft transplantation (MAT). The PubMed, Scopus, and Cochrane Central Register databases were used to search relevant articles in April 2018. Studies that evaluated at least one association between a factor and outcomes were extracted. Of 3,381 titles, 52 studies were finally analyzed. Data about predictors, patient-reported outcome scores (PROMs), and failure rates were extracted for quantitative and qualitative analysis. A total of 3,382 patients and 3,460 transplants were identified. Thirty different pre

Epidural lysis of adhesions for failed back surgery and spinal stenosis: factors associated with treatment outcome.

BACKGROUND: Failed back surgery syndrome (FBSS) is a challenging problem. One treatment advocated to treat FBSS is epidural lysis of adhesions (LOA). The results of studies examining LOA for FBSS have been mixed, but are limited because no study has ever sought to identify factors associated with outcomes. METHODS: We performed this multicenter, retrospective study in 115 patients who underwent LOA for FBSS (n = 104) or spinal stenosis (n = 11) between 2004 and 2007. Twenty-seven demographic, clinical, and procedural variables were extracted from medical records and correlated with the outcom

Validation of an adaptation of the stress process model for predicting low back pain related long-term disability outcomes: a cohort study.

STUDY DESIGN: Twelve-month cohort study. OBJECTIVE: The aim of the study was to examine the ability of an adaptation of the stress process model to predict different outcomes among low back pain (LBP) sufferers. SUMMARY OF BACKGROUND DATA: Recently, the stress process model was adapted and was shown to be useful to partially explain long-term disability related to low back pain, an important occupational health problem. METHODS: French-speaking compensated workers on sick leave because of subacute common LBP (N=439) completed a questionnaire including the adapted stress process model's fact

This symposium was organized to provide recent informations concerning pneumoconiosis and mycobacterial diseases in Japan. Pneumoconiotic workers have been decreasing in number and in severity because of change in industrial structure and improvement of occupational health measures. But radiological figures of dust exposed worker are going to be complex and be difficult for diagnosis due to aging, smoking and of complicated respiratory and non-respiratory diseases. Major complications such as pulmonary tuberculosis and tuberculous pleurisy are decreasing but non-tuberculous mycobacterial infec

Do Patient Demographic and Socioeconomic Factors Influence Surgical Treatment Rates After ACL Injury?

INTRODUCTION: Anterior cruciate ligament (ACL) injuries may be managed nonoperatively in certain patients and injury patterns; however, complete ACL ruptures are commonly reconstructed to restore anterior and lateral rotatory stability of the knee. While ACL reconstruction is well-studied, the literature is sparse with regard to which socioeconomic patient factors are associated with patients undergoing ACL reconstruction rather than nonoperative management after diagnosis of an ACL injury. The current study seeks to evaluate this relationship between patient demographics as well as socioecono

Nonmedical Factors Influencing Early Deaths in Burns: A Study of the National Burn Repository.

It is well-established that survival in burn injury is primarily dependent on three factors: age, percent total-body surface area burned (%TBSA), and inhalation injury. However, it is clear that in other (nonburn) conditions, nonmedical factors may influence mortality. Even in severe burns, patients undergoing resuscitation may survive for a period of time before succumbing to infection or other complications. In some cases, though, families in conjunction with caregivers may choose to withdraw care and not resuscitate patients with large burns. We wanted to investigate whether any nonmedical

Multicomponent intervention versus usual care for management of hypertension in rural Bangladesh, Pakistan and Sri Lanka: study protocol for a cluster randomized controlled trial.

BACKGROUND: High blood pressure (BP) is the leading attributable risk for cardiovascular disease (CVD). In rural South Asia, hypertension continues to be a significant public health issue with sub-optimal BP control rates. The goal of the trial is to compare a multicomponent intervention (MCI) to usual care to evaluate the effectiveness and cost-effectiveness of the MCI for lowering BP among adults with hypertension in rural communities in Bangladesh, Pakistan and Sri Lanka. METHODS/DESIGN: This study is a stratified, cluster randomized controlled trial with a qualitative component for evalua

A systematic review of preoperative predictors for postoperative clinical outcomes following lumbar discectomy.

BACKGROUND CONTEXT: Sciatica is often caused by a herniated lumbar intervertebral disc. When conservative treatment fails, a lumbar discectomy can be performed. Surgical treatment via lumbar discectomy is not always successful and may depend on a variety of preoperative factors. It remains unclear which, if any, preoperative factors can predict postsurgical clinical outcomes. PURPOSE: This review aimed to determine preoperative predictors that are associated with postsurgical clinical outcomes in patients undergoing lumbar discectomy. STUDY DESIGN: This is a systematic review. METHODS: This
