Cost and quality of life of overlooked eye care needs of children.

BACKGROUND: The objective of this research was to conduct a systematic review and cost analysis to summarize, from the Ministry of Health perspective, the costs families might incur because of their child's prescription for refractive errors and amblyopia correction. METHODS: Databases including MEDLINE, Embase, BIOSIS, CINAHL, HEED, ISI Web of Science, and the Cochrane Library as well as the gray literature were searched. Systematic review was conducted using EPPI-Reviewer 4. Percentage difference in cost of glasses and patches per patient per various diagnoses were computed. The cost of gla…

Reappraisal of astigmatism induced by periocular capillary hemangioma and treatment with intralesional corticosteroid injection.

OBJECTIVE: To document refractive status and visual acuity before and after intralesional corticosteroid injection in children with astigmat … igmatism and spherical error, the amount of anisometropia (spherical equivalent) remained constant. Amblyopia was not observed before treatment and was observed in only 2 of 13 children after treatment. Complications were limited to adrenal suppression with transient reductions of linear growth and localized eyelid necrosis. CONCLUSIONS: Intralesional cort …

Critical Periods in Vision Revisited.

For four decades, investigations of the biological basis of critical periods in the developing mammalian visual cortex were dominated by study of the consequences of altered early visual experience in cats and nonhuman primates. The neural deficits thus revealed also provided insight into the origin and neural basis of human amblyopia that in turn motivated additional studies of humans with abnormal early visual input. Recent human studies point to deficits arising from alterations in all visual cortical areas and even in nonvisual cortical regions. As the new human data accumulated in paralle…

Comparison of stereoacuity in patients of anisometropia, isometropia and emmetropia.

PURPOSE: To compare the stereoacuity in patients with anisometropia, isometropia, and emmetropia. METHODS: A cross-sectional study was conducted on 1403 subjects (range: 5-45 years) divided into anisometropes (n = 403), isometropes (n = 500), and emmetropes (n = 500). There were 258 amblyopic eyes among anisometropes and 156 amblyopic eyes among isometropes. Stereoacuity was measured using the Titmus stereo test consisting of a combination of contour targets. RESULTS: There were 675 males and 728 females. A significant (P < 0.001) reduction in stereoacuity was found in anisometropes as compa

Lights and darks of a picture. The life of Giovanni Francesco Barbieri, "il Guercino" - the squinter.

PURPOSE: To be cross-eyed is a wide-spread dysfunction of the eye and squinting is currently regarded as unattractive. However, in ancient times, societies saw squinting as a sign of piety or even beauty. Even Venus was depicted heterophoric quite often. The most famous artist who was cross-eyed was Giovanni Francesco Barbieri (*08.02.1591; †22.12.1666, both Italy). His nickname was "Il Guercino" which means "the squinter." This article reflects on vision and art from the perspective of a cross-eyed person. METHODS: Life and work of Giovanni Francesco Barbieri was analyzed and interpreted bas

[Discrimination difficulties--expression of disordered relative localization?].

Spatial uncertainty was examined according to a procedure suggested by Bedell and Flom (1981) ("triangle procedure") and additionally with a line-division test designed by Kundt. A horizontal line of a length of about 20 degrees and a shorter one of 4.3 degrees was divided into two equal parts. Strabismic amblyopes mark the middle of such a line with less precision and greater uncertainty than visually normal subjects do. Visual acuity was measured by single optotypes (Landolt rings), as well as by line optotypes with spaces of 2.6 min of arc between each other (C-test). Surprisingly, there wa

Coats' disease in a patient with Cornelia de Lange syndrome.

A 16-month-old boy had the mental and physical retardation, low-pitched cry, phocomelia with syndactyly, hirsutism, low-set ears, bushy eyebrows, elongated eyelashes, blepharoptosis, and strabismus characteristic of Cornelia de Lange syndrome along with ophthalmoscopic findings characteristic of Coats' disease. Cryotherapy made the abnormal telangiectatic vessels less prominent, but macular scarring produced poor visual acuity. Occlusion therapy was unsuccessful.

The management of strabismus.

The original causes of strabismus and the complications that develop from efforts by a child to adjust to the condition may present a complex diagnostic and therapeutic problem. The complications of suppression, amblyopia, contractures and anomalous visual habits may become more difficult to correct than the cause of the strabismus. The longer the duration of strabismus, the more complex the problem, the more difficult the treatment, and the less the probability of functional cure. Treatment should be started early. Correction may be brought about by the use of glasses, eye patches and orthopt…

Pediatric open-globe injury: Current perspectives, surgical principles, and literature review.

Open-globe injury (OGI), referring to the full-thickness violation of the eye wall, is a severe consequence of ocular trauma which can lead to permanent vision loss. Here, we review the current literature on pediatric OGI including a focused review of studies published in the past 10 years. Scoring systems for final visual prognosis, important aspects of clinical examination, and imaging at the point of care are discussed. We furthermore provide an overview of primary and secondary surgical management of the initial OGI and sequelae. Amblyopia management differentiates pediatric from adult vis…

Atypical presentation of Best Disease.

CLINICAL CASE: A 43-year-old man was treated for reduced visual acuity, initially attributed to strabismic amblyopia. On fundus examination, bilateral neurosensory detachments (NSD) were observed in posterior pole, surrounded by deposits of lipofuscin. His 3-year-old son was also examined and circumscribed NSD was observed with the presence of pseudohypopyon in OD and a fibrosis scar in OS. The Arden ratio were decreased in electrooculography (EOG) in both patients, and genetic studies revealed a single mutation of the BEST1 gene. DISCUSSION: The existence of extensive bilateral NSD may be an…

Approach to diagnosis and management of optic neuropathy.

Visual loss consequent to anterior visual pathway involvement can occur in a variety of clinical settings. In a tropical country like India, apart from the usual suspects, nutritional, infective, and toxic amblyopia have to be considered in the differential diagnosis. The mode of onset (acute/chronic), unilateral versus bilateral involvement, accompanying occular pain or the lack of it, and pattern of visual loss are some of the pointers which help to differentiate optic neuropathy clinically. The presence of concurrent neurological deficits, evidence of other systemic illnesses, and the resul…

Congenital ptosis.

Congenital blepharoptosis presents within the first year of life either in isolation or as a part of many different ocular or systemic disorders. Surgical repair is challenging, and recurrence necessitating more than one operation is not uncommon. Not all patients with congenital ptosis require surgery, but children with amblyopia due to astigmatic anisometropia or deprivation may benefit from early surgical correction. A variety of surgical procedures to correct congenital ptosis have been described. The choice of procedure depends on a number of patient-specific factors, such as degree of pt…

Intralesional bleomycin for the treatment of periocular capillary hemangiomas.

Periocular infantile capillary hemangiomas do not always respond well to conventional treatment modalities such as systemic or intralesional corticosteroids, radiotherapy or debulking surgery. The authors describe the use of intralesional bleomycin injections (IBIs) to treat potentially amblyogenic lesions in two cases where other modalities have failed. In both cases monthly IBIs successfully cleared the visual axis of the affected eye before the age of 1 year thus preventing permanent sensory deprivation amblyopia. A total of five and nine injections, respectively, were used and no significa…

[Cataract in children--not only an ophthalmological problem].

Cataract is an opacity of the lens that leads to loss of vision and even blindness. It is responsible for 10.7-14% of the children who are blind. The etiology of cataract is unknown. Cataract in children can be congenital or acquired, unilateral or bilateral. Unilateral congenital cataract is an important cause of amblyopia and strabismus. Cataract can occur as an isolated disorder or as coexisting problem with other diseases. Differential diagnostics requires a number of tests regarding the patient and his relatives. The management should be complex including prophylaxis, diagnosis and treatm…
