Long-standing cystoid macular edema was evaluated in three groups of aphakic patients: Group A following vitrectomy, Group B without vitrectomy, and group C without vitrectomy but with associated background diabetic retinopathy. After a two-year follow-up period, no significant differences in resolution of cystoid macular edema and visual acuity were found between the three groups.

Aphakic and pseudophakic corneal edema have become the primary indication for corneal transplantation. At the time of this surgery, implantation of an intraocular lens (IOL) may be considered. We describe our technique for inserting a flexible anterior chamber IOL during keratoplasty to avoid any damage to intraocular structures.

A retrospective review of 51 secondary intraocular lens (IOL) implantations was made to determine the relative accuracy of the SRK regression formula and the refraction method described by Holladay in preoperative prediction of emmetropic IOL power. The regression formula accurately predicted emmetropia within 1 diopter (D) in 63% of patients; the refraction method accurately predicted within 1 D in only 26% of cases. The authors evaluated the intraocular penetration of gentamicin (50 mg/ml) into aphakic rabbit eyes following anodal iontophoresis (0.75 mA for 10 min). Gentamicin levels were determined at 0.5, 4, 8, 16, and 24 hrs after iontophoresis (n = 6 eyes for each time) using an agar diffusion bioassay. Peak levels of 72.04 +/- 6.1 (means +/- SE) micrograms/ml for the corneas and 77.8 +/- 3.0 micrograms/ml for the aqueous humor were obtained at 30 min after iontophoresis. A prospective series of 25 eyes received an intraocular lens (IOL) of power calculated for planned ametropia, by means of the formulae of R. D. Binkhorst, from data of axial length, corneal curvature, and postoperative anterior chamber depth. A simple and comfortable procedure is used to measure the decay of oxygen content of a soft contact lens on a human cornea when the anterior surface of the lens is blocked from further supply of oxygen. Calculations from this measurement give the steady-state oxygen tension under the soft contact lens and the oxygen flux into the cornea when the eye is open but wearing a contact lens. When the procedure is applied to phakic and aphakic human eyes, the results indicate that for both states, the oxygen uptake by the cornea is about the same when the uptake levels are above 1 microliter/cm2 . hr
