Brain oedema is usually associated with intracranial meningiomas in about 50-66%. As underlying causes, different factors like localisation, vascular supply, angiogenic growth factors and histological subtypes are discussed, and its existence is probably multifactorial. We present 11 patients with the rare subtype of secretory meningiomas. Brain oedema was observed in 82%. These tumours are localised mainly at the frontal convexity and at the sphenoid ridge. All 11 patients were female so that hormonal factors also may play a role in the production of peritumoural oedema. The postoperative out…

BACKGROUND: We analyzed the clinical manifestations and therapeutic outcomes of Klebsiella brain abscesses.
PATIENTS AND METHODS: The clinical data of 15 patients with Klebsiella brain abscesses, retrospectively collected over a 14-year period, were studied.
RESULTS: The 15 patients included 13 cases of Klebsiella pneumoniae infection and two cases of Klebsiella oxytoca. All but one case were community-acquired infections. Locations of all of these abscesses were supratentorial. 12 cases involved a single abscess, and three involved multiple abscesses. Gas formation was also found in two cas…

Brain metastases (BrM) occur in 30% to 50% of patients with malignant tumours and represent a relevant cause of morbidity and mortality. As documented by the extensive clinical use of magnetic resonance imaging with contrast enhancement, 25% to 35% of patients with BrM have a single localisation to the brain. Conventional therapeutic options for single BrM consist of either neurosurgery, or whole brain irradiation, or a combination of both. Treatment finality for the majority of the patients is palliation and life expectancy is in the order of a few months. In the last decade, the development …

In the 50 years since Leksell developed the concepts and initial hardware for modern brain radiosurgery, the treatment has progressed to the point where it is used commonly for arteriovenous malformations, benign masses, and metastases. Radiosurgery offers patients an effective treatment of life-threatening lesions with a reasonably low risk for discomfort and injury. In the 1990s, the procedure was used widely as primary and adjuvant treatment. The difficulty of defining the boundaries of primary brain cancers makes determining treatment targets problematic. Better imaging and computing offer…

Neuroimaging studies of neurobehavioral disorders are using new imaging modalities. In dyslexia, anatomic imaging studies demonstrate an abnormal symmetry of the planum temporale. Functional imaging supports the hypothesis that developmental dyslexia is frequently the result of deficits in phonologic processing and that normal reading requires a patent network organization of a number of anterior and posterior brain areas. In autism, anatomic imaging studies are conflicting. Functional imaging demonstrates temporal lobe abnormalities and abnormal interaction between frontal and parietal brain …

HIF-1 is composed of HIF-1alpha and HIF-1beta protein subunits. HIF-1 is induced by hypoxia and binds to promoter/enhancer elements and stimulates the transcription of hypoxia-inducible target genes. Because HIF-1 activation might promote cell survival in hypoxic tissues, we studied the effect of stroke on the expression of HIF-1alpha, HIF-1beta and several HIF-1 target genes in adult rat brain. After focal cerebral ischemia, mRNAs encoding HIF-1alpha, glucose transporter-1 and several glycolytic enzymes including lactate dehydrogenase were up-regulated in the areas around the infarction. HIF …

OBJECTIVE: To review the basic concepts and mechanisms of cerebral edema and to analyze current management of this complex clinical problem.
DEVELOPMENT: Cerebral edema is a stereotyped reaction of the brain to any aggression and is a very common complication in patients with alterations of the blood-brain barrier. The cerebral edema should be recognized and treated early and aggressively to avoid disastrous results such as cerebral herniation. The most important types of edema are the vasogenic and cytotoxic. The first implies primary alterations of the blood-brain barrier, whilst in the sec…

In two patients, men aged 23 and 42 years, a condition that mimicked brain death was observed as a consequence of rapidly progressive complete peripheral paralyses, which included the intrinsic and extrinsic eye muscles. However, the EEG revealed a waking pattern. Maximal supportive therapy was provided, which included haemodialysis for the first patient and artificial ventilation for both patients. A slow recovery was seen after four weeks. The first patient was paralyzed following the ingestion of a large quantity of ethylene glycol and the second by botulism due to the consumption of injudi…

Brain tumors based on their histogenesis, consist of all tumors, derive from the entirely tissue in the intracranial space, both from the neuro-ectodermal/neuro-epithelial tissue and the mesenchymal tissue. By their location they can be divided into infratentorial or supratentorial, and further into deep vs. superficial. The interesting and unique, there are age distribution or location-sex specificity of some brain tumors (BT). WHO Histopathological Typing of Tumors by the CNS, also showing progress on both of their members and new special types of some BT, especially for the meningiomas and …

We investigated the prognosis of 42 children with traumatic brain injuries. The main etiology was a traffic accident in 46 cases, especially during walking and bicycling, and child abuse in 7 cases. Eighteen cases of acute subdural hematoma 18 cases distributed at all ages, 9 cases of diffuse axonal injury mainly during school age, 4 cases of chronic subdural hematoma under 2 years. These were all caused by child abuse. Fifteen cases showed a good prognosis with independent activities of daily living (ADL). The main type of injury was diffuse axonal injury in this group. Twelve cases showed a …

Tubercular meningitis and intracranial tuberculomas are the two frequent manifestations of neurotuberculosis with their variable incidence in different countries. Brain stem tuberculomas are even more unusual, accounting for 2.5-8% of all intracranial tuberculomas. We present here six paediatric cases of brain stem tuberculosis, where well-formed tuberculomas were demonstrated in five symptomatic cases; however, only a hypodense ill-defined lesion was found in the brain stem of one child who presented with features of tubercular meningitis. Three of these children had other associated tubercul…

The brain distribution of 109CdCl2 following administration into either the tail vein, the lateral ventricle or the olfactory bulb was studied to clarify permeability of the brain barrier system to cadmium (Cd) and Cd movement in the cerebrospinal fluid (CSF) and the brain extracellular fluid. One hour after intravenous (i.v.) injection, 109Cd was largely concentrated in the choroid plexus, and 109Cd concentration in the major part of the brain parenchyma, except for the circumventricular organs such as the pineal gland and the regions around them, was low. Six days after i.v. injection, 109Cd…

TM staging proposed by UICC was classified mainly by histology and tumor size (extension). Staging trial was done by using the data of astrocytic tumor cases of the supratentorial region collected both during the periods between 1978 to 1987 by the Brain Tumor Registry of Japan (Permission for publication: Sep. 1994). Survival curves classified by T classification in each histology, such as astrocytoma, malignant astrocytoma, glioblastoma, showed a great differences from each other, even at the same T4 group of each histology. In this sense, the cases in stage IV should not be discussed as a w…

OBJECTIVE: To present two cases of post-infectious encephalitis of the brain stem (ETC) in infancy, which is very infrequent at this age.
CLINICAL CASES: Two patients aged 4 months and 9 months respectively had a previous history of a catarrhal illness a few days before the onset of encephalitis. The clinical condition was of subacute onset and torpid course, characterized by ataxia, reduced level of consciousness, involvement of the pyramidal tracts and paralysis of the cranial nerves. No significant information for the diagnosis of either case was obtained from CT. MR showed lesions at the …

BACKGROUND: An extensive literature describes structural lesions in apraxia, but few studies have used functional neuroimaging. We used positron emission tomography (PET) to characterize relative cerebral glucose metabolism in a 65-year-old, right-handed woman with progressive decline in ability to manipulate objects, write, and articulate speech.
OBJECTIVE: To characterize functional brain organization in apraxia.
DESIGN AND METHODS: The patient underwent a neurological examination, neuropsychological testing, magnetic resonance imaging, and fludeoxyglucose F 18 PET. The patient's magnetic …

The case of a pregnant patient who had a massive intracraneal haemorrhage at 18 weeks of gestation is presented. Patient's neurological damage evolved to brain death, but the fetus continued in good condition. The decision of withdrawing life support or to continue supporting the mother's life to allow fetal development aroused difficult ethical questions, both to relatives and professionals. This is an exceptional situation of a heart beating cadaver and a non viable fetus whose life depends on the continuation of treatments that are considered as experimental. A good decision should be based…

In a retrospective study, we report the clinical characteristics of chronic brain oedema (CBO) in 34 patients with neurocysticercosis (NCC) who presented diffuse brain oedema upon computed tomography (CT) as a common point. All patients received dextrochlorpheniramine, and, 94.1% of them also albendazole. A predominance of females (73.5%) aged 11-40 years (92.3%) was seen. Headache occurred in 94.1% of patients, nausea/vomiting in 47.1%, epileptic manifestations in 41.1% and psychiatric disorders in 38.2%. Hyperreflexia was obtained in 82.3% of patients, papilloedema in 58.8% and normal neurol…

Pediatric central nervous system neoplasms include a spectrum of both glial and nonglial tumors that differ significantly in location and biological behavior from those of adults. Brain tumors in infants and children most often arise from central neuroepithelial tissue, whereas a significant number of adult tumors arise from central nervous system coverings (e.g., meningioma), adjacent tissue (e.g., pituitary adenoma), or metastases. Most adult brain tumors are supratentorial malignant gliomas, whereas the most common malignant pediatric brain tumor is the cerebellar primitive neuroectodermal …
