Cerebral phaeohyphomycosis.

A 14-year-old girl with a rare form of fungal brain abscess involving the dura and brain parenchyma is reported. No portal of entry of the infection was found. Histological findings and fungal culture both indicated that the causative agent was Xylohypha bantiana, an uncommon dematiaceous fungus. A review of the literature suggests that this infection, which affects primarily young male patients, exhibits distinct neurotropism. Despite therapy, the prognosis is generally poor.

Mortality in epilepsy following brain infections in adults: A register-based population-wide study.

OBJECTIVE: Acquired epilepsy is associated with increased risk of death, but whether this extends to epilepsy after brain infections is unknown. Our objective was to estimate mortality and causes of death in patients with epilepsy after four types of brain infections. METHODS: A population-wide Swedish cohort was created from the National Patient Registry, including all patients over 18 years of age who received inpatient care in 2000-2010 for herpes encephalitis, brain abscesses, other encephalitis, or bacterial meningitis. Cox proportional hazards modelling was used to compare the hazard ra…

Streptococcus intermedius Acute Meningitis in an Immunocompetent Child.

Streptococcus intermedius is a commensal bacterium increasingly associated with brain abscesses. It can infect the brain through a direct extension of an inflammatory process or a hematogenous route. Meningitis due to this pathogen is sporadic and has not been described in children before. We report a case of an immunocompetent 6-year-old boy who developed bacterial meningitis due to S. intermedius .

[A Case of Cerebellum Abscess due to Fusobacterium nucleatum that Developed Because of Poor Dental Hygiene].

We report a case of cerebellum abscess due to Fusobacterium nucleatum(F. nucleatum) in a 60-year-old man. He was admitted to our hospital complaining of headache and dizziness. On admission, he was lucid with the following vital data:blood pressure, 136/89 mmHg;heart rate, 65 beats/min;body temperature, 37.0℃;and oxygen saturation, 100%. He had a moderate headache and could not walk straight. In general, there were no abnormal findings except for his poor dental hygiene. Laboratory findings revealed elevated white blood cell counts(10,900/mm3)and brain MRI revealed a mass shadow that was suspe

Reversible alterations in brain metabolites during therapy for disseminated nocardiosis using proton magnetic resonance spectroscopy.

We report reversible abnormalities in magnetic resonance spectra acquired from a patient with AIDS undergoing antibiotic and corticosteroid therapy for disseminated nocardiosis, a rare opportunistic infection of immunosuppressed patients which can cause cerebral abscess formation. There was no clinical, CT or MRI evidence of HIV-1 encephalitis. MR spectra were acquired before and after treatment using a two-dimensional chemical shift imaging technique (TR 1500ms, TE 130ms). Prior to treatment, a rise in the choline to creatine ratio and a reduction in the N-acetylaspartate to creatine ratio we

Diagnostic difficulties with central nervous system actinomycosis.

When faced with expanding brain lesions of unknown origin showing a ring-shaped enhancement on post-contrast imaging, we use definite criteria to direct further investigation and distinguish among a number of possible diagnostic hypotheses. However, a correct diagnosis may be difficult in some cases, especially when dealing with less frequent conditions. This is the case of actinomycosis, a highly treatable but insidious infection for which nowadays there may be a low level of attention. Brain localization is associated with a significant morbidity and may represent a true diagnostic pitfall.

[Orbital and intracranial complications of rhinosinusitis].

Rhinogenic orbital and intracranial complications occur in 0.8 and 0.01% of patients with rhinosinusitis, respectively. The author recognizes 9 and 8 forms of rhinogenic orbital and intracranial complications, respectively. Conservative treatment is preferable in non-purulent orbital and intracranial complications resultant from acute inflammation of the paranasal sinuses. In the above complications taking place in chronic paranasal inflammation it is valid to conduct a sanitizing operation on the paranasal sinuses. Purulent intracranial and orbital complications dictate the necessity of major

Cerebral abscesses imaging: A practical approach.

Brain abscesses (BAs) are focal infections of the central nervous system (CNS) that start as a localised area of weakening of the brain parenchyma (cerebritis) and develops into a collection of pus surrounded by a capsule. Pyogenic (bacterial) BAs represent the majority of all BAs; in some cases, the diagnostic and therapeutic management can be challenging. Imaging has a primary role in differentiating BAs from other lesions. Conventional magnetic resonance imaging (cMRI) is essential for the identification of the lesion, its localisation and its morphological features. However, cMRI does not …

Unroofed Coronary Sinus Presenting as Cerebral Abscess: A Case Report.

A sixty eight year-old woman with a long-standing history of hypertension, dizziness and a history of congenital heart disease presented with speech difficulties and disorientation. She was diagnosed with a brain abscess, confirmed by a stereotactic biopsy. Transthoracic echocardiographic evaluation revealed a persistent left superior vena cava (PLSVC) with an unroofed coronary sinus (URCS) along with a small secundum atrial septal defect. Her heart catheterization showed a partially unroofed coronary sinus along with a bidirectional shunt. She was referred for surgical closure of her unroofed…

Nocardiosis in adult-onset Still's disease and vasculitis syndrome.

Four cases of nocardiosis in patients with adult-onset Still disease and vasculitis syndrome are presented. Three patients developed lung abscesses and 1 case developed a brain abscess. All were treated with high-dose corticosteroids, and 3 were given cyclosporine when they developed nocardiosis. All patients were successfully treated with antibiotics; cyclosporine was discontinued in 2 cases. These cases indicate that systemic nocardiosis can develop in patients with various rheumatologic diseases who are treated with corticosteroid and immunosuppressive drugs such as cyclosporine.

Presacral neuroenteric fistula presenting with meningitis and vaginal fistula: a case report.

Neuroenteric fistulas are rare congenital malformations arising from a persistent connection between the embryonic neural and intestinal tissues; they are extremely rare in the lumbosacral area. Neuroenteric fistulas should be suspected whenever children present with anaerobic meningitis in the absence of brain abscesses or respiratory infections. This patient is the first patient who presented with a presacral cyst that separately connected to the nervous system and the vagina. It is thought that the rape of this 12-year-old patient resulted in soilage of the spinal fluid through dual fistula…

Craniopharyngioma and abscess: When tumor and infection co-exist.

Pituitary abscesses are rare entities that may occur in a previously healthy gland or in the setting of a pituitary tumor. Only eleven cases of abscesses associated with craniopharyngioma have been reported in the literature. The etiology is unknown and the diagnosis is difficult because there are no specific clinical or radiologic features that allow us to suspect the synchrony of both entities before surgery. Intraoperative findings and culture are the only definitive signs to confirm this association. Here we present the first surgical video showing this unique association and highlighting

Intracranial actinomycosis in juvenile patients. Case report and review of the literature.

A case of actinomycotic brain infection in a juvenile patient is described. Cases of actinomycosis affecting the head and neck are rare, particularly in juvenile patients. In this case complete resolution of the infection was achieved by means of surgical treatment and prolonged antibiotic therapy. The authors emphasize the importance of a combined approach for treatment of this unusual brain infection and stress the difficulties involved in the diagnosis of this pathology.

Massive posterior fossa tuberculous abscess developing in a young child treated for miliary tuberculosis. Possible role of very rapid acetylation of isoniazid.

A 21-month-old infant presented with acute obstructive hydrocephalus due to a large tuberculous abscess in the posterior fossa 3 months after starting treatment for miliary tuberculosis. Insertion of a ventriculo-peritoneal shunt resulted in some clinical improvement but subsequent neurological deterioration occurred due to massive enlargement of the tuberculous abscess despite apparently adequate antituberculosis therapy. Repeated drainage procedures of the abscess eventually resulted in resolution and clinical improvement. As part of the workup for poor weight gain and the unusual clinical c
