Case study of VA Caribbean Healthcare System's community response to Hurricane Maria.

BACKGROUND: Hurricane Maria, which hit Puerto Rico in 2017, catastrophically impacted infrastructure and severely disrupted medical services. The US Department of Veterans Affairs Caribbean Healthcare System (VA CHCS), which serves approximately 67,000 patients and has most of its facilities on the island of Puerto Rico, was able to successfully maintain operations after the hurricane. As a part of the larger VA system, VA CHCS also has a mission to support "national, state, and local emergency management, public health, safety and homeland security efforts." The objective of this study is to

Multicentric and Observational Study of Omalizumab for Chronic Spontaneous Urticaria in Real-Life in Colombia.

BACKGROUND: Although chronic urticaria (CU) is a common, cause of medical consulting both in general practitioners and allergist specialists worldwide, there is little information about its behavior and management in Latin America. Currently, national and international guidelines recommend using Omalizumab for cases refractory to management with antihistamines. Despite advances in the knowledge of Omalizumab for the management of CU, although there are few studies in underdeveloped countries, there are many studies evaluating the impact of Omalizumab treatment. There is not clinical informatio

Facilitators and barriers to point-of-care testing for sexually transmitted infections in low- and middle-income countries: a scoping review.

BACKGROUND: Sexually transmitted infections (STIs) in low- and middle-income countries (LMICs) are predominantly managed by syndromic management. However, most STIs are asymptomatic. These untreated STIs cause individual morbidity, and lead to high STI prevalences. There is increasing interest in the use of point-of-care tests (POCTs) for STIs in LMICs, which could facilitate same day testing and treatment. To best utilise these tests, we must understand the facilitators and barriers to their implementation. The aim of this review is to explore how point-of-care testing for STIs has been imple

Primary sclerosing cholangitis and autoimmune hepatitis overlap syndrome associated with inflammatory bowel disease: A case report and systematic review.

BACKGROUND: A previously healthy 22-year-old woman presented with abdominal pain and jaundice. She had a reagent antinuclear factor (1:640, with a homogeneous nuclear pattern) and hypergammaglobulinemia (2.16 g/dL). Anti-smooth muscle, anti-mitochondrial and anti-liver-kidney microsomal antibody type 1 antibodies were negative. Magnetic resonance cholangiography showed a cirrhotic liver with multiple focal areas of strictures of the intrahepatic bile ducts, with associated dilations. Liver biopsy demonstrated periportal necroinflammatory activity, plasmocyte infiltration and advanced fibrosis.

Quantifying and preventing Plasmodium vivax recurrences in primaquine-untreated pregnant women: An observational and modeling study in Brazil.

Each year, 4.3 million pregnant women are exposed to malaria risk in Latin America and the Caribbean. Plasmodium vivax causes 76% of the regional malaria burden and appears to be less affected than P. falciparum by current elimination efforts. This is in part due to the parasite's ability to stay dormant in the liver and originate relapses within months after a single mosquito inoculation. Primaquine (PQ) is routinely combined with chloroquine (CQ) or other schizontocidal drugs to supress P. vivax relapses and reduce the risk of late blood-stage recrudescences of parasites with low-grade CQ re

Urban segregation and the US heroin market: a quantitative model of anthropological hypotheses from an inner-city drug market.

BACKGROUND: We hypothesize that the location of highly segregated Hispanic and in particular Puerto Rican neighborhoods can explain how Colombian-sourced heroin, which is associated with a large-scale decade long decline in heroin price and increase in purity, was able to enter and proliferate in the US. METHODS: Our multidisciplinary analysis quantitatively operationalizes participant-observation ethnographic hypotheses informed by social science theory addressing complex political economic, historical, cultural and social processes. First, we ethnographically document the intersection of st

Detection and characterization of human T-cell lymphotropic virus type I (HTLV-I) associated T-cell neoplasms in an HTLV-I nonendemic region by polymerase chain reaction.

Human T-cell lymphotropic virus type I (HTLV-I) associated adult T-cell leukemia/lymphoma (ATLL) occurs endemically in southwestern Japan, the Caribbean, and West Africa, but occurs sporadically in most of the rest of the world. However, because ATLL and non-HTLV-I associated T-cell neoplasms share overlapping clinicopathologic features, the prevalence of ATLL in nonendemic regions is unknown. In this study, 75 T-cell neoplasms randomly procured from the metropolitan New York City area were examined by polymerase chain reaction (PCR) for the presence of integrated HTLV-I proviral sequences. Awareness and Use of Saw Palmetto and Other Alternative Medicines Used for Treating Prostate Diseases Among Men Visiting a Urology Clinic in Guyana.

Herbal medicine remains widely used in the Caribbean, including Guyana, where traditional remedies are often used alongside conventional medical care. Saw palmetto (Serenoa repens) is internationally recognized as a common herbal supplement for benign prostatic hyperplasia (BPH) and lower urinary tract symptoms, but local data on its use among Guyanese men with prostate diseases are limited. The objective of this study is to assess awareness of saw palmetto and estimate the prevalence of saw palmetto and other herbal medicine use among men with prostate diseases attending the urology clinic at

HIV-related mortality time trends among children and young adolescents on antiretroviral therapy by age, treatment duration, and region: a systematic review and meta-regression analysis.

BACKGROUND: Past cohort studies have examined mortality among children and young adolescents (aged 0-14 years) who have received antiretroviral therapy (ART), but no systematic reviews have been undertaken to synthesise these findings. Our study aims to provide the most comprehensive global estimates of paediatric mortality among children and adolescents receiving ART. METHODS: For this systematic review and meta-regression analysis, we searched PubMed and Embase from Jan 1, 1990 to July 31, 2024 for studies reporting mortality among children and young adolescents living with HIV who were on

Alcohol use and associated risk factors among female sex workers in low- and middle-income countries: A systematic review and meta-analysis.

Due to its widespread use in the sex work industry, female sex workers (FSWs) in low- and middle-income countries (LMICs) are at high risk of harmful alcohol use and associated adverse health outcomes. Factors associated with harmful alcohol use include violence, mental health problems, drug use, sexual risk behaviors and HIV/STIs. To our knowledge, there has been no quantitative synthesis of FSW alcohol use data to date. This systematic review and meta-analysis aims to provide an estimate of the prevalence of harmful alcohol use among FSWs in LMICs and to examine associations with common heal

Disparities in diagnosis and outcomes in American patients with transthyretin cardiac amyloidosis.

Although Afro-Caribbean (AC) race has been associated with worse outcomes in many cardiovascular diseases, its potential association with transthyretin cardiac amyloidosis (ATTR-CA) is less understood. We aimed to assess the relationship between race and serum biomarkers, adverse cardiac remodeling, and outcomes in AC vs white ATTR-CA patients. 114 AC and 117 white patients confirmed ATTR-CA who underwent cardiac magnetic resonance (CMR) exam were identified. The relationship between race and the primary endpoint-defined by all-cause mortality or heart failure hospitalization-was assessed usin

Contraceptive use in Latin America and the Caribbean with a focus on long-acting reversible contraceptives: prevalence and inequalities in 23 countries.

BACKGROUND: The rise in contraceptive use has largely been driven by short-acting methods of contraception, despite the high effectiveness of long-acting reversible contraceptives. Several countries in Latin America and the Caribbean have made important progress increasing the use of modern contraceptives, but important inequalities remain. We assessed the prevalence and demand for modern contraceptive use in Latin America and the Caribbean with data from national health surveys. METHODS: Our data sources included demographic and health surveys, multiple indicator cluster surveys, and reprodu

Global situation of dengue and dengue haemorrhagic fever, and its emergence in the Americas.

About two-thirds of the world's population live in areas infested with dengue vectors, mainly Aedes aegypti. All four dengue viruses are circulating, sometimes simultaneously, in most of these areas. It is estimated that up to 80 million persons become infected annually although marked underreporting results in the notification of much smaller figures. Currently dengue is endemic in all continents except Europe and epidemic dengue haemorrhagic fever (DHF) occurs in Asia, the Americas and some Pacific islands. The incidence of DHF is much greater in the Asian countries than in other regions. In

Effects of climate change on physical inactivity: a panel data study across 156 countries from 2000 to 2022.

BACKGROUND: Climate change is amplifying heat exposure worldwide; however, its consequences for global physical inactivity, and the resulting effects on mortality and economic burden, remain unquantified. METHODS: We analysed a longitudinal dataset spanning 156 countries from 2000 to 2022 using a binned fixed-effects panel regression model. The model examined the relationship between the primary outcome-the age-standardised prevalence of physical inactivity in adults (aged ≥18 years)-and annual exposure to different temperature ranges. Estimated exposure coefficients and climate projections u

Multifaceted assessment of the content of potentially toxic elements and microplastics in the sargassum habitats of Caribbean beaches, Mexico.

The present study aimed to evaluate potentially toxic elements (PTEs) (Al, As, B, Ba, Ca, Cd, Co, Cr, Cu, Fe, Hg, Li, Mg, Mn, Ni, Pb, S, Se, Sr, Tl, Zn) in sargassum habitats at two distinct sites: Puerto Morelos (PM) and Sian Ka'an Biosphere Reserve (SK) along the Mexican Caribbean coast. Median dissolved concentrations of S (1937.8 mg/L), Mg (1172.5 mg/L), and Ca (408.7 mg/L) were relatively higher than those of other elements studied. Similarly, elemental concentrations in sediment followed an order of Ca > Mg > Sr > S > Fe > Al > B > Ba > Mn > Cr > Se > Cu > As > Zn > Pb > Tl > Li > Ni > C

Sickle Cell Lung Disease in a 9-year-Old Presenting with Wheezing: Investigating Causal Relationships, Asthma or Acute Chest Syndrome.

BACKGROUND: Sickle cell disease (SCD) affects about 7.7 million people worldwide, and this has been increasing annually due to an increasing population in sub-Saharan Africa and the Caribbean. Pulmonary complications contribute significantly to mortality due to sickle cell disease. We have frequently heard wheezing in children with sickle cell disease that has responded to beta agonists such as salbutamol leading us to wonder whether this was asthma or a presentation of acute chest syndrome. We present this case to highlight the co-occurrence of sickle cell disease and asthma, and to re-ignite
