The Kudo total elbow arthroplasty in patients with rheumatoid arthritis.

A Kudo total elbow arthroplasty (TEA) was performed in 36 elbows in 35 patients with rheumatoid arthritis. Of those 35, 4 died, 6 prostheses were revised, and 2 were lost to follow-up. Twenty-four elbows with a mean follow-up of 58 months were radiologically and clinically reviewed. Sixteen were scored as excellent by use of the Mayo score and Hospital for Special Surgery 2 score. The mean increase in active motion was 25 degrees. Two humeral and four ulnar radiologic loosenings were noted. Two early dislocations were successfully treated with closed reduction and cast immobilization, two pati

Treatment of subcapital fractures of the fifth metacarpal bone: a prospective randomised comparison between functional treatment and reposition and splinting.

We did a prospective study to compare the results of treatment of subcapital fractures of the fifth metacarpal bone by closed reduction and splinting or by functional treatment. Twenty-nine consecutive patients were randomly divided into the two treatment groups (functional n = 14, and reposition and splinting n = 15). The results of treatment were satisfactory in both groups. Functionally treated patients recovered their grip force and range of movement of the affected hand a little sooner. All fractures in both groups had united within three months. There were no complications. We conclude t

Intra-articular distal radius fractures: arthroscopic assessment of radiographically assisted reduction.

The optimal means of assessing articular displacement during closed reduction of distal radius fractures is unknown. The purpose of this study was to evaluate the in vivo accuracy of fluoroscopy (C-arm) and plain radiographs (XR) in measuring articular step-off and gap and to determine if postreduction arthroscopy can identify malreduced intra-articular fractures that would benefit from reduction. Fifteen intra-articular distal radius fractures underwent closed manipulation and percutaneous pinning. Reduction was assessed sequentially by C-arm, XR, and wrist arthroscopy. The width of gapping b

Transitional ankle fracture management using a new joystick technique.

PURPOSE: To investigate the feasibility and short-term clinical efficacy of the arthrography-assisted joystick technique for the treatment of adolescent transitional ankle fracture. METHODS: A retrospective analysis was performed in 10 adolescent transitional ankle fracture patients treated with the arthrography-assisted joystick technology. There were 7 male patients and 3 female patients with a mean age of 12.80 ± 1.81 years (range: 10-16 years). All cases were closed fractures, and the operation was performed after the failure of manual reduction [X-ray or computed tomography (CT) showed t

Irreducible Ankle Fracture from an Interposed Chaput Fragment: A Case Report.

CASE: A 23-year-old active-duty military service member sustained an unusual irreducible fracture-dislocation of the ankle joint. Preoperative computed tomography showed an anterolateral tibial (Chaput) fragment entrapped within the distal syndesmosis, preventing adequate reduction. An open reduction of the fragment was performed in the operating room, which allowed near-anatomic reduction and internal fixation of the tibiotalar joint. At the 21-month follow-up visit, the patient remained on active duty in the Navy without limitations. CONCLUSION: To our knowledge, this rare injury pattern, a

Treatment of subtrochanteric femoral fracture with long proximal femoral nail antirotation.

OBJECTIVE: Subtrochanteric femoral fractures are severe injuries. Although many treatment methods have been developed, controversy exists regarding the optimal management of these fractures. This study evaluated the clinical outcome of subtrochanteric femoral fractures fixed with long proximal femoral nail antirotation (PFNA-long). METHODS: Between October 2006 and February 2008, 25 patients with traumatic subtrochanteric fractures of the femur were treated with PFNA-long. Closed reduction and fixation were performed in 20 cases. In the remaining 5 cases, closed reduction was difficult, so li

Screw Fixation of Pediatric Tibial Tubercle Fractures.

BACKGROUND: Tibial tubercle fractures are rare injuries that account for <1% of physeal fractures. These fractures are thought to be increasing in frequency, particularly in young, adolescent males who participate in basketball and other sports with repeated running and jumping. The tibial apophysis becomes mechanically vulnerable as the proximal tibial physis closes from posteromedial to anterolateral, enabling the quadriceps to overpower the chondroepiphysis and avulse the proximal tibial epiphysis from the tibial metaphysis. DESCRIPTION: Position the patient supine with the leg on a bump o

Treatment of displaced flexion-type pediatric supracondylar humeral fractures in the prone position.

PURPOSE: This study is to report a new method to reduce and fix the displaced flexion-type pediatric supracondylar fracture in the prone position. METHODS: Ten children with displaced flexion-type supracondylar humeral fractures treated between 2007 and 2013 were reviewed. There were three girls and seven boys, with a mean age of 9.5 years. The fracture was reduced by gentle traction of the forearm and gradual extension of the elbow in the prone position. Two or three crossed Kirschner wires (K-wires) were inserted percutaneously to secure the fracture reduction. Radiographic evaluation inclu

Intraoperative imaging with a 3D C-arm system after zygomatico-orbital complex fracture reduction.

PURPOSE: During the repair of zygomatico-orbital complex (ZMC) fractures, the lateral orbital wall and/or the orbital floor is often reduced by merely reducing the zygoma. Intraoperative 3D imaging can help surgeons decide whether the orbit must be reconstructed as well. The purpose of this study was therefore to assess the usefulness of intraoperative 3D C-arm imaging in evaluating the adequacy of fracture reduction. METHODS: A total of 21 patients with unilateral ZMC fractures were enrolled in this retrospective study. Four fractures were treated with a closed reduction technique. Seventeen

The use of Ender nails in tibial shaft fractures.

The results of 31 fractures of the tibial shaft treated with closed Ender nailing and early weight-bearing are reported. Twenty-five of these fractures were closed and six open. All fractures united between 6 and 16 weeks after operation. No case of deep wound infection was encountered. Comminuted fractures of the proximal and distal third were prone to angular deformities and needed cast protection post-operatively. Nail reinsertion became necessary in two patients. Delay of operation renders closed reduction very difficult. The advantages of this method are: 1) normal knee and ankle movement

Bilateral Costoclavicular Brachial Plexus Block for Closed Reduction of Bilateral Elbow Dislocation: A Case Report and Review of the Literature.

Bilateral brachial plexus block is approached cautiously because of concerns regarding local anesthetic systemic toxicity, respiratory compromise, and bilateral upper-limb motor block. We describe a 60-year-old man with bilateral posterolateral elbow dislocation after a road traffic accident who underwent closed reduction and cast application under bilateral ultrasound-guided costoclavicular brachial plexus block using 10 mL of local anesthetic on each side. The procedure was completed without general anesthesia or deep sedation. This case highlights that low-volume bilateral costoclavicular b

Management of prehospital shoulder dislocation: feasibility and need of reduction.

PURPOSE: Dislocation of the shoulder is rare in the prehospital setting. The medical specialities of the emergency physicians are heterogeneous, and the level of experience is different. Aim of this study was to evaluate the feasibility, sufficiency, and need of prehospital reduction. METHODS: Over 12 months, 16 rescue stations in Germany and Austria documented cases. Points of examination were: incidence of reduction, influence of pathological findings, therapy and effectiveness of reduction. RESULTS: We included 70 patients. A reduction was undertaken in n = 47 (66.6 %). In n = 70 (100 %)

Anterior Hip Dislocation with Cranial Femoral Head Osteochondral Lesion and Isolated Quadrilateral Plate Fracture: A Case Report.

CASE: A 28-year-old man involved in a traffic accident presented with anterior hip dislocation associated with an acetabular fracture. Initially, closed reduction of the coxofemoral dislocation was performed under general anesthesia. Subsequently, reduction and internal fixation of the quadrilateral plate were achieved using the Stoppa intrapelvic approach with the use of lateral window of the ilioinguinal approach. CONCLUSION: Only a few cases of obturator-type hip dislocations, femoral head osteochondral lesion, and associated isolated fractures of the quadrilateral plate have been reported

Operative treatment of isolated epiphyseal fracture of the distal fibula: 1 case report and literature review.

Pediatric ankle injuries are common; ankle epiphyseal fractures are also common in children. But isolated distal epiphyseal fibular fractures of the distal fibula are clinically rare. We describe one unusual case of an adolescent with a completely displaced Salter-Harris type II distal fibular epiphyseal fracture. The attempt of closed reduction failed, and the patient required open reduction and internal fixation. The localized periosteum and the superior peroneal retinaculum were avulsed from the distal fibular metaphysis, with the peroneal tendons underneath exposed but no obvious subluxati

Modified Extension Block Technique for Bony Mallet Injury of the Hallux.

Bony mallet injury of the hallux is uncommon. In the few reports of this injury, authors have described surgical treatments such as closed reduction with percutaneous pinning and open surgical fixation with Kirschner wires or a suture anchor. However, the appropriate surgical management for this injury remains controversial. In this article, we describe a case of bony mallet injury of the hallux repaired with the modified extension block techniqueusing 3 Kirschner wires. This method is an effective and simple treatment to allow anatomic reduction of the displaced articular fracture fragment wi

[Percutaneous stabilization of traumatic spondylolisthesis in a child].

A 12-year-old boy suffered a rare occurrence of a traumatic spondylolisthesis (L5/S1) without neurological alterations after being partially buried underneath a collapsing brick wall. Additionally he sustained a third degree open fracture of the left distal fibula and epiphysiolysis of the left distal tibia. A closed reduction and percutanous dorsal instrumentation L5/S1 as well as an open reduction and osteosynthesis of the tibia and fibula were performed. After 6 months the instrumentation was completely removed and an unrestrained range of motion of the lumbar spine and the upper ankle join

[Total carpometacarpal dislocations. Two case reports].

We report two cases of carpometacarpal dislocation of the four long fingers. They were diagnosed in emergency and treated by open reduction and stabilization by intramedullary pinning. They were followed for an average of 30 months. The results were rated good, and the two patients had an excellent grip strength, no deformity, no instability, no disabling or limited prehension. Dislocation or fracture-dislocation of the carpometacarpal joints are uncommon injuries. The diagnosis can be easily missed. The authors recommend closed or open reduction and fixation by pins and immobilisation in a pl

Fracture-dislocation of the lumbosacral spine. Report of a case and review of the literature.

A rare lumbosacral fracture-dislocation occurred in a 22-year-old woman. In a review of the literature describing 17 additional cases, the injury was classified as rare and usually caused by severe trauma. The mechanism of injury is hyperflexion and rotation. In general, attention is drawn to the association of fractured transverse processes as an important clue to the diagnosis. Neural injury is frequent, as are fractures of the lumbosacral facet joints and sacral promontory. Attempts at closed reduction are futile, and the treatment of choice is open reduction and internal fixation with lumb

Multiple carpometacarpal dislocations. A review of four cases.

The cases of four patients with carpometacarpal fracture-dislocations of the ulnar four joints are reported, in all of whom there was extensive additional trauma. Open reduction was necessary in three patients, and only one was treated successfully by a closed reduction. He then had persistent but painless subluxation of the metacarpal bases. On end-result evaluation, two of the other three patients had occasional discomfort and diminished grip strength. One patient had a diminished range of motion of the wrist joint at one-year follow-up, and one patient had narrowing and sclerosis of the car
