Frameworks for estimating causal effects in observational settings: comparing confounder adjustment and instrumental variables.

To estimate causal effects, analysts performing observational studies in health settings utilize several strategies to mitigate bias due to confounding by indication. There are two broad classes of approaches for these purposes: use of confounders and instrumental variables (IVs). Because such approaches are largely characterized by untestable assumptions, analysts must operate under an indefinite paradigm that these methods will work imperfectly. In this tutorial, we formalize a set of general principles and heuristics for estimating causal effects in the two approaches when the assumptions a

Effects of dopamine D2/3 and opioid receptor antagonism on the trade-off between model-based and model-free behaviour in healthy volunteers.

Human behaviour requires flexible arbitration between actions we do out of habit and actions that are directed towards a specific goal. Drugs that target opioid and dopamine receptors are notorious for inducing maladaptive habitual drug consumption; yet, how the opioidergic and dopaminergic neurotransmitter systems contribute to the arbitration between habitual and goal-directed behaviour is poorly understood. By combining pharmacological challenges with a well-established decision-making task and a novel computational model, we show that the administration of the dopamine D2/3 receptor antago

Strengthening Mental Abilities with Relational Training (SMART) in multiple sclerosis (MS): study protocol for a feasibility randomised controlled trial.

BACKGROUND: Multiple sclerosis (MS) is a chronic condition of the central nervous system, affecting around 1 in every 600 people in the UK, with 130 new diagnoses every week. Cognitive difficulties are common amongst people with MS, with up to 70% experiencing deficits in higher-level brain functions-such as planning and problem-solving, attention, and memory. Cognitive deficits make it difficult for people with MS to complete everyday tasks and limit their abilities to work, socialise, and live independently. There is a clear need-and recognised research priority-for treatments that can impro

Thalamostriatal circuit as a therapeutic target of alcohol use disorder.

Alcohol Use Disorder (AUD) is a chronic neuropsychiatric disorder characterized by high relapse rates and limited treatment efficacy. In this review, we focus on evidence that chronic alcohol exposure disrupts intralaminar thalamic input to the dorsal striatum, particularly Pf/CM-Pf-related regulation of dorsomedial striatal cholinergic interneurons, a pathway implicated in reversal learning and cognitive flexibility in preclinical models. We further distinguish this circuit from PVT-centered thalamic pathways that influence alcohol-related reward, aversion, and relapse-like behavior through v…

Optimizing Restless Legs Syndrome Care: Integrating Rehabilitation into Multimodal Management.

Restless leg syndrome (RLS) is a common yet frequently under-diagnosed condition. The exact pathophysiology is unknown, however multiple factors including genetic factors, brain iron deficiency and dysfunction of dopaminergic system are associated with RLS. It is a clinical diagnosis confirmed by International Restless Legs Syndrome Study Group (IRLSSG) consensus diagnostic criteria (2012). Several pharmacological and non - pharmacological management options are available for treatment of RLS. Non-pharmacological management options include lifestyle modifications, therapeutic exercises (aerobi

The dangers of failing one or more performance validity tests in individuals claiming mild traumatic brain injury-related postconcussive symptoms.

Evaluating performance validity is important in any neuropsychological assessment, and prior research recommends a threshold for invalid per … e than individuals failing no PVTs on measures of verbal learning and memory, processing speed, and cognitive flexibility. Additionally, individuals failing one versus two PVTs significantly differed only on delayed free recall scores. The current findings suggest that failure of even one PVT should elicit consideration of performance invalidity, particularly in indivi …

Executive functioning in children with Fetal Alcohol Spectrum Disorders: profiles and age-related differences.

The goal of this project was to examine the profile of executive function (EF) deficits and age-related differences among children with FASD. Twenty-nine children with FASD (8 to 16 years of age) completed 8 tests from the Delis-Kaplan Executive Function System (D-KEFS). They had difficulty on many components of EF including cognitive flexibility, inhibition, some measures of verbal fluency, abstract thinking, deductive reasoning, hypothesis testing, problem solving, and concept formation. A distinctive profile emerged with performance being poorest on the card sorting test and relatively high…

Psychedelics can increase long-term cognitive flexibility, leading to normalization of negativity bias and reduction in rumination. We propose a theoretical model that explains how psychedelics can reduce the negativity bias in depressed patients. At the psychobiological level, we hypothesize that the nega …

The role of dopamine D(3) receptors in antipsychotic activity and cognitive functions.

Dopamine D(3) receptors have a pre- and postsynaptic localization in brain stem nuclei, limbic parts of the striatum, and cortex. Their widespread influence on dopamine release, on dopaminergic function, and on several other neurotransmitters makes them attractive targets for therapeutic intervention. The signaling pathways of D(3) receptors are distinct from those of other members of the D(2)-like receptor family. There is increasing evidence that D(3) receptors can form heteromers with dopamine D(1), D(2), and probably other G-protein-coupled receptors. The functional consequences remain to

Multiparty Communication: A New Direction in Characterizing the Impact of Traumatic Brain Injury on Social Communication.

PURPOSE: The purpose of this viewpoint is to advocate for increased study of common ground and audience design processes in multiparty communication in traumatic brain injury (TBI). METHOD: Building on discussions at the 2024 International Cognitive-Communication Disorders Conference, we review common ground and audience design processes in dyadic and multiparty communication. We discuss how the diffuse profiles of neural and cognitive deficits place individuals with TBI at increased risk for keeping track of who knows what in group settings and using that knowledge to flexibly adapt their co

Dynamically shifting from compositional to conjunctive brain representations supports cognitive task learning.

During cognitive task learning, neural representations must be rapidly constructed for novel task performance, then optimized for robust practiced task performance. How the geometry of neural representations changes to enable this transition from novel to practiced performance remains unknown. We hypothesized that practice involves a shift from compositional representations (task-general activity patterns that can be flexibly reused across tasks) to conjunctive representations (task-specific activity patterns specialized for the current task). Functional MRI during learning of multiple complex

Within-individual organization of the human cognitive cerebellum: Evidence for closely juxtaposed, functionally specialized regions.

Specific regions in the cognitive cerebellum are connected to distinct cerebral association networks. Do these cerebellar regions exhibit functional specialization similar to the cerebral cortex? Here, we mapped the cerebellum within intensively studied participants (N = 15) first using connectivity to estimate regions linked to specific networks and then prospectively testing functional response properties in task data within each individual's own idiosyncratic anatomy. A large megacluster extending across Crus I/II was consistently found with subregions linked to five higher-order associatio

Restricted Daily Exposure of Environmental Enrichment: Bridging the Practical Gap from Animal Studies to Human Application.

Daily restricted environmental enrichment (REE) refers to limited, structured periods of enrichment aimed at improving both physical and cognitive well-being in animals and humans. This review explores the significance of REE, focusing on studies that investigate 2 and 3 h daily enrichment protocols. Through an analysis of 21 key studies, this paper highlights how even brief periods of REE can lead to substantial improvements in brain plasticity, cognitive function, and stress resilience. The review tracks the evolution of environmental enrichment from early research on enriched environments i
