Immunotherapy with a depigmented, polymerized vaccine of Olea europaea pollen allergens. Significantly reduces specific bronchial and skin test reactivity in sensitized patients after one year of treatment.

Clinical manifestations after the inhalation of Olea europaea pollen are very frequent in Spain. Forty-five patients with a clinical history of asthma and sensitivity to O. europaea pollen were included in a randomized trial to evaluate the safety and the clinical efficacy of immunotherapy with a new chemically modified extract (depigmented and glutaraldehyde-polymerized) of Olea europaea. The study was conducted following good clinical practices and appropriate consent forms were signed. Patients were divided into three groups of 15 individuals: Group A received a maximum concentration of 44 …

Medico-legal aspects of histopathology practice.

Medico-legal problems experienced by histopathologists differ from those of other clinicians as they are rarely in direct contact with patie … s provided the patient's identity is kept confidential. Nevertheless, it would be prudent to revise consent forms for surgery and autopsies to include the possibility that tissues removed can be stored or used for research and education. Good medical practice in pathology encourages a willingness to consult colleagues when in doubt, but advises that the treatin …

Simulated-patient evaluations during formative interviews predict exam performance better than self-reported empathy and the ability to recognize others' emotions.

BACKGROUND: Communication skills are central to clinical practice and are typically assessed through Objective Structured Clinical Examinations (OSCEs). Prior research indicates that self-reported questionnaires, simulated-patient evaluations, and expert assessments each offer valuable yet non-interchangeable perspectives on medical students' communication skills. However, it remains unclear whether these assessment modalities, when collected during the academic year, can predict subsequent OSCE performances and thereby help identify students struggling to meet communication competency standar

Level and correlates of disrespect and abuse among newborns in selected public hospitals of Addis Ababa, Ethiopia.

BACKGROUND: The provision of respectful and dignified maternal and newborn care is an important component of the quality of childbirth care. Although a growing body of evidence was generated on disrespect and abuse (D&A) of women during childbirth in the past decade there is limited evidence on D&A experienced by newborns. Our study aimed to determine the level of and factors associated with D&A among newborns. METHODS: We conducted the study in three public hospitals in Addis Ababa. We directly observed childbirth care starting from the first stage of labor through two hours after the birth

Human-Centered Design of a Multistakeholder Reporting Dashboard for Disseminating HIV Implementation Research in the Ending the HIV Epidemic Initiative: Development and Formative Evaluation Study.

BACKGROUND: Interactive dashboards have expanded public access to HIV surveillance and programmatic data, but existing tools do not synthesize implementation research evidence into formats accessible to the diverse stakeholders who could use it. Designing such tools requires deliberate alignment with users' needs, language, and decision-making contexts. OBJECTIVE: We describe the human-centered design (HCD) process used to develop and conduct a formative evaluation of the Ending the HIV Epidemic (EHE) Implementation Science (IS) Reporting Dashboard, a public-facing tool intended to synthesize

Effectiveness of a home-based physical exercise intervention in patients with fragility fractures on functional independence and hospital readmissions: a protocol for a randomised controlled trial.

INTRODUCTION: Patients with fragility fractures are two times as likely to suffer future fractures as their peers who have not suffered a fracture. In addition, 40% of those who suffer fragility fractures do not recover their level of functioning in terms of activities of daily living after 1 year. The present study aims to verify the hypothesis that a semipersonalised home-based exercise intervention may improve patients' independence and reduce the number of hospital admissions compared with usual care for a population that suffers fragility fractures. METHODS AND ANALYSIS: This parallel-ar

The "Chopsticks" Method: A Technique for Capsular Retraction During Hip Arthroscopy Utilizing Switching Sticks.

BACKGROUND: Hip arthroscopy has become an effective method for treating several symptomatic disorders of the hip, which includes femoroacetabular impingement (FAI). Capsulotomy is often necessary for visualization of hip pathology; however, this presents multiple challenges in both capsular management intraoperatively and final capsule closure. Current techniques include use of capsular suspension sutures to provide adequate views of the central and peripheral compartments. We present an atraumatic technique that may be used in conjunction with suspension sutures to decrease the need for exces

Harmful alcohol use in rural Uganda: a pilot study from the Kigezi sub-region.

INTRODUCTION: Currently, Uganda has the highest per capita alcohol consumption in Africa, and the negative effects of harmful use of alcohol are quite prevalent. Some rural areas face a complex set of underlying factors that may be responsible for this trend, including unemployment and easy access to cheap alcohol. Kigezi sub-region is one of the areas most affected by the harmful use of alcohol. This paper characterizes harmful use of alcohol in form of patterns of use, timing for drinking, types of drinks; and identifies factors associated with it in the sub-region’s tourist area around Lake

Type 4 Acromioclavicular Joint Separation Reconstruction With Suspensory Fixation, Allograft, and Suture Augmentation.

BACKGROUND: This is a case of surgical treatment of a type 4 acromioclavicular (AC) joint separation via an arthroscopically assisted approach and reconstruction with combined suspensory fixation, allograft ligament reconstruction, and suture augmentation. AC joint injuries are relatively common, representing approximately 9% of shoulder injuries and approximately 50% of sport-related shoulder injuries. Most AC joint injuries do well when treated conservatively. However, the literature supports surgical treatment for high-grade injuries, failed conservative treatment, and open injuries or thos

Paratenon-Sparing Quad Tendon Graft Harvest.

BACKGROUND: In recent years, the quadriceps tendon has gained popularity for primary anterior cruciate ligament (ACL) reconstruction surgery due to its robustness, easy access, and ability to be tailor-made to each patient. However, concerns remain regarding potential complications associated with quadriceps tendon harvest, including impaired healing and damage to the extensor mechanism. INDICATIONS: Preserving the quadriceps paratenon, which is a source of blood supply, nutrients, and therapeutic cells during ACL reconstruction surgery, may improve postoperative outcomes by mitigating potent

Revision Anterior Glenoid Reconstruction With Distal Tibia Allograft Combined With Open Capsular Shift.

BACKGROUND: Managing patients with recurrent anterior shoulder instability and glenoid bony deficiency remains a challenge. Multiple graft options, including iliac crest, distal clavicle, coracoid, and distal tibia allograft have been used. There is a growing body of evidence that supports distal tibia allograft for glenoid restoration in patients with shoulder instability due to its ability to restore the articular surface as well as the glenoid depth and curvature. INDICATIONS: Surgical indications for anterior glenoid reconstruction with distal tibia allograft combined with open capsular s

Blood Flow Restriction Training for Meniscus Repair Surgery.

BACKGROUND: Blood flow restriction (BFR) is a training tool that involves wearing a tourniquet or occlusive device during exercise. Data support that low-load training with BFR may produce muscle hypertrophy similar to standard high-load training. Because of the weight-bearing and range of motion (ROM) restrictions after meniscal repair, patients encounter substantial atrophy of lower extremity musculature. We perform BFR for these patients to limit atrophy postoperatively with the goal of facilitating their return to prior function and sports. INDICATIONS: We incorporate BFR in the postopera
