Should We Recommend Vitamin K2 Supplement to Prevent Coronary Artery Calcification for Patients Receiving Statins and/or Warfarin?

Several reports suggest that in animal models, as well as in the clinical setting, long-term warfarin use increases coronary artery calcifications. The same has been reported for statins prescribed for patients at risk or with established atherosclerosis. Coronary calcifications are considered a risk marker for further cardiovascular events. However, numerous clinical trials have established that statins reduce the risk for cardiovascular events. Warfarin also has been shown to reduce the risk of cardiovascular events, including re-infarction. It has been suggested that the increase in coronar

Very late presentation of anomalous origin of the left coronary artery from the pulmonary artery: case report.

BACKGROUND: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital coronary anomaly. The enlarged right coronary artery provides retrograde collaterals to supply the left ventricle then preferentially directs into the lower pressure pulmonary artery system causing coronary steal phenomenon. Few patients who survive through adulthood without surgery must have abundant, well-formed functioning collaterals with adequate perfusion of the left ventricle. We present the oldest reported patient with ALCAPA to undergo corrective surgery. CASE PRESENTATION

Unveiling the Microvasculature: The Index of Microcirculatory Resistance and Its Expanding Role in Cardiovascular Care.

Index of microcirculatory resistance (IMR) is a cutting-edge, wire-based tool that advances the capability assessment of coronary microvascular function. By utilizing distal coronary pressure and mean transit time under maximal hyperemia, IMR delivers consistent, reproducible insights into the microvasculature's dynamic health. IMR has been shown to be of particular use as a diagnostic tool in the assessment of coronary microvascular dysfunction (CMD) allowing for diagnostic clarification in conditions like ischemia with non-obstructive coronary arteries (INOCA), acute coronary syndrome (ACS),

Discordant myocardial perfusion defects and coronary artery disease - a multimodality imaging journey in the natural history of cardiac sarcoidosis.

Cardiac sarcoidosis is a rare form of systemic sarcoidosis characterized by formation of non-caseating granulomas in the myocardium, leading to heterogeneous manifestations, including conduction disturbances, arrhythmias, and heart failure. Besides myocardial fibrosis and structural myocardial alterations, cardiac sarcoidosis can impact the epicardial and microvascular circulation, causing ischemia and regional microvascular dysfunction. We present a case of cardiac sarcoidosis with atypical initial presentation but with evidence of disease in myocardial perfusion imaging, which was overlooked

The role of myocardial blood volume in the pathophysiology of angina with non-obstructed coronary arteries: The MICORDIS study.

BACKGROUND: Angina with Non-Obstructed Coronary Arteries (ANOCA) involves abnormal vasomotor responses. While reduced coronary flow is an established contributor to myocardial hypoxia, myocardial blood volume (MBV) independently regulates myocardial oxygen uptake but its role in ANOCA remains unclear. OBJECTIVES: We hypothesized that reduced MBV contributes to ANOCA, and associates with insulin resistance in ANOCA. METHODS: MBV in ANOCA patients was compared to age- and sex-matched healthy controls. ANOCA patients underwent coronary angiography with invasive coronary function testing (CFT) t

Advances in the Computational Assessment of Disturbed Coronary Flow and Wall Shear Stress: A Contemporary Review.

Coronary artery blood flow is influenced by various factors including vessel geometry, hemodynamic conditions, timing in the cardiac cycle, and rheological conditions. Multiple patterns of disturbed coronary flow may occur when blood flow separates from the laminar plane, associated with inefficient blood transit, and pathological processes modulated by the vascular endothelium in response to abnormal wall shear stress. Current simulation techniques, including computational fluid dynamics and fluid-structure interaction, can provide substantial detail on disturbed coronary flow and have advanc

Reproducibility of bolus versus continuous thermodilution for assessment of coronary microvascular function in patients with ANOCA.

BACKGROUND: A bolus thermodilution-derived index of microcirculatory resistance (IMR) has emerged as the standard for assessing coronary microvascular dysfunction (CMD). Continuous thermodilution has recently been introduced as a tool to quantify absolute coronary flow and microvascular resistance directly. Microvascular resistance reserve (MRR) derived from continuous thermodilution has been proposed as a novel metric of microvascular function, which is independent of epicardial stenoses and myocardial mass. AIMS: We aimed to assess the reproducibility of bolus and continuous thermodilution

Coronary microvascular dysfunction is an independent predictor of developing cancer in patients with non-obstructive coronary artery disease.

AIMS: Cardiovascular disease and cancer share common pathogenesis and risk factors. Coronary microvascular dysfunction (CMD), reflecting impaired coronary microvascular dilation in response to stress, is related to a higher risk of major cardiovascular events; however, its association with cancer has not been explored. METHODS AND RESULTS: A retrospective study on 1042 patients with non-obstructive coronary artery diseases (NOCADs) was performed. Data regarding demographic, clinical history, diagnostic coronary reactivity test, and cancer occurrence were collected. Coronary microvascular dysf

Coronary Flow Velocity Reserve Reduction Is Associated with Cardiovascular, Cancer, and Noncancer, Noncardiovascular Mortality.

BACKGROUND: Coronary flow velocity reserve (CFVR) measured in the left anterior descending artery during high-dose vasodilator stress echocardiography interrogates both epicardial and microcirculatory coronary function and has been inversely associated with chronic inflammation and microvascular dysfunction, as well as with the presence of obstructive coronary artery disease. Microvascular dysfunction and chronic inflammation are common mechanisms of disease in cardiovascular (CV) and non-CV conditions. We aimed to assess whether CFVR is associated with all-cause death, but more specifically w

Myocardial CT perfusion compared with transthoracic Doppler echocardiography in evaluation of the coronary microvascular function: An iPOWER substudy.

BACKGROUND: A significant number of women with angina and no obstructive coronary artery disease (CAD; <50% stenosis) have coronary microvascular dysfunction (CMD) which carries an adverse cardiovascular prognosis. Coronary microvascular function can be evaluated by transthoracic Doppler echocardiography (TTDE) as a coronary flow velocity reserve (CFVR) and by static CT myocardial perfusion (CTP) as a myocardial perfusion reserve (MPR). Whether these methods are correlated is not known. We assessed the correlation between CFVR and MPR and investigated whether women with angina, CMD and no obst

Assessment of Myocardial Perfusion by Angiographic Methods in Tortuous Coronary Arteries.

The relationship between coronary tortuosity (CorT) and tissue-level myocardial perfusion is not clear. We investigated tissue perfusion in myocardial territories supplied by tortuous coronary arteries. Among patients who had undergone coronary angiography, patients with reported CorT, those with ≥1 coronary artery were included in the study group (100 patients). The control group included patients with normal coronary arteries (100 patients). Thrombolysis In Myocardial Infarction frame count (TFC) and myocardial blush grade (MBG) were calculated for each coronary artery. Mean TFC was signific

[Vascular functional alterations in hypertrophic cardiomyopathy].

Hypertrophic cardiomyopathy is a hereditary, not uncommon cardiac disease, which is associated with asymmetric thickening and hypertrophy of the interventricular septum unrelated to hemodynamic reasons. Despite hypertrophic cardiomyopathy is considered to be a disorder of the heart muscle, several associated vascular alterations have been described. The aim of the present review is to summarize vascular functional alterations in hypertrophic cardiomyopathy.

Effects of intra-aortic balloon pumping on graft flow in coronary surgery: an intraoperative transit-time flowmetric study.

BACKGROUND: We investigated hemodynamic effects of intra-aortic balloon pumping (IABP) in in-situ and aorta-coronary (A-C) grafts during coronary artery bypass grafting (CABG). METHODS: One hundred seventy-two grafts, including 84 in-situ left internal thoracic arteries (LITAs), were examined intraoperatively with a transit-time flowmeter in 84 patients who had prophylactic IABP. The following measurements were obtained for each graft during off-IABP and on-IABP: mean flow, maximal flow, pulsatility index, and diastolic filling index. Coronary angiograms were performed 14 +/- 5 days after cor

Unidirectional communication between the circumflex and right coronary arteries: a very rare coronary anomaly and cause of ischemia.

Intercoronary communication is an exceptionally rare congenital malformation. A 43-year-old male was admitted to our cardiology department with symptomatic stable angina. ECG and echocardiography was normal. Exercise electrocardiography showed ST depression in inferior leads. No atherosclerotic plaque in the coronary arteries was detected on coronary angiography. However, a unidirectional intercoronary communication between the circumflex and right coronary arteries, which was leading a coronary steal from right to left, was observed. Although intercoronary communication is generally not relat

Coronary angiographic characteristics of patients with permanent artificial pacemakers.

BACKGROUND: The cause of severe cardiac conduction disturbances is often uncertain. The aim of this study was to examine a group of patients with permanent pacemakers who underwent coronary arteriography to determine the extent of coronary atherosclerotic disease that might be responsible for the conduction disturbances. METHODS AND RESULTS: Forty-three consecutive patients with a permanent pacemaker and 36 matched control patients were investigated. The coronary angiographic study included measurement of diameter and stenosis severity, qualitative assessment of flow, and classification of pa
