Butyrate promotes epithelial cell healing and improves symptoms when administered rectally in patients with distal ulcerative colitis (UC). It was hypothesized that butyrate may enhance healing in patients with UC by stimulating colonocyte proliferation and/or protein production. Mucosa from the descending colon was obtained from patients with UC (n = 5), Crohn's disease (n = 8), diverticulitis (n = 6), and cancer (normal tissue 10 cm from tumor; n = 10). Epithelial cells were isolated using dispase/collagenase and differential sedimentation and incubated for 4 hr at 37 degrees C with either N…

Diverticular disease of the colon in patients under the age of 40 years is uncommon. Between 1975 and 1985, 58 patients (31 men and 27 women) were admitted for pathologically or radiographically proven acute diverticulitis. Seventeen (29.3 percent) were younger than 40 years. Fifteen of the 17 (88.2 percent) required urgent or emergent surgery for complications of diverticular disease. This represents a significantly (P less than 0.02) larger proportion of that age group than those patients older than the age of 40 (17/41 or 41.5 percent). Twelve of the patients younger than age 40 (70.6 perce…

Colonic lipomas are a rare entity, even lessfrequent colonic lipomatosis (lipohyperplasia) of which we only found very few reports in literature (five cases). We report a case of colonic lipomatosis in a young adult that manifested as acute diverticulitis with perforation and rectalpolyposis requiring surgical resolution.

Adult intussusception caused by an inverted Meckel diverticulum is rare. We report a 39-year-old Turkish man with intussusception due to Meckel diverticulitis. Ileoileal intussusception was suggested by computed tomography. Exploration revealed ileoileal intussusception with Meckel diverticulum. A diverticulectomy with small bowel resection was performed.

Infectious and inflammatory diseases comprise some of the most common gastrointestinal disorders resulting in hospitalization in the United States. Accordingly, they occupy a significant proportion of the workload of the acute care surgeon. This article discusses the diagnosis, management, and treatment of appendicitis, acute cholecystitis/cholangitis, acute pancreatitis, diverticulitis, and Clostridium difficile colitis.

The clinical presentation of primary epiploic appendagitis can mimic diverticulitis or appendicitis. Review of the pathologically confirmed cases in the English literature shows the majority of cases to arise from the sigmoid colon. Pathognomonic findings by computed tomography impact patient management, and it is thus important to recognize these in patients imaged in the emergency setting.

Postmenopausal abnormal uterine bleeding is a common clinical problem addressed in gynaecological practice and should prompt clinical investigation due to the significant prevalence of malignant and premalignant lesions of the endometrium in this age group. Nevertheless, other causes should be considered, since its diagnostic and therapeutic management may differ considerably. Here, we present a case of a colouterine fistula due to chronic diverticulitis presenting with postmenopausal abnormal uterine bleeding. This is an infrequent occurrence and is caused by the rupture of a diverticular abs…

INTRODUCTION: The colovescical fistula is one of the complications of diverticular disease. It can cause significant symptoms like pneumaturia and fecaluria affecting the quality of life and sometimes leading to death, usually secondary to sepsis. We describe two cases of colovescical fistula treated by laparoscopic approach in patients with diagnosis of complicated acute diverticulitis.
CASE REPORT: We studied two patients with clinical, radiological and endoscopic diagnosis of colovescical fistula as a consequence of diverticular disease. We performed a totally laparoscopic treatment with c…

BACKGROUND: Pelvic inflammatory disease is an infection of the upper genital tract, including the uterus, ovaries, uterine tubes, and pelvic peritoneum. Tubo-ovarian abscess and pyosalpinx are common complications associated with pelvic inflammatory disease. They are usually encountered in sexually active women, but rare cases in Virgos have also been described.
CASE PRESENTATION: Here, we report the case of a 50-year-old Virgo woman presenting with pyosalpinx secondary to previous laparotomic sigmoidectomy for acute diverticulitis. Inflammation caused by the woman's diverticulitis and laparo…

Meckel's diverticulum is a real diverticulum located at the antimesenteric portion of intestinal loops and including all layers of the intestinal wall. It is the most common congenital anomaly of the gastrointestinal tract, and its incidence is 1-3%. Many asymptomatic cases are diagnosed when complications occur. A 23-year-old female patient applied to gynaecology emergency clinic with pelvic pain complaint. Laparotomy was performed with the diagnosis of acute abdomen because the physical examination and imaging studies did not exclude tuboovary pathology. Giant Meckel's diverticulitis and isc…

From 1984 to 1993, 11 patients (7 men and 4 women, mean aged 70 years) underwent surgical procedure for sigmoido-vesical fistula due to diverticulitis. Fecaluria and/or pneumaturia was present in 10 patients. The diagnosis of sigmoido-vesical fistula due to diverticulitis was confirmed by urologic and colonic investigations. All patients underwent surgical treatment. One patient underwent creation of a diverting colostomy alone because of a poor general status. Definitive surgical correction of the fistula with resection of the diverticular colon was attempted in the remaining 10 patients, inc…

Although diverticular perforation and diverticulitis is usually a straightforward diagnosis, an occasional case can be difficult. In this report, we describe a patient who initially presented with symptoms suggestive of maxillary sinusitis and hip fracture. Findings of subcutaneous craniofacial emphysema eventually led to the diagnosis of a perforated sigmoid diverticulum with abscess.

Percutaneous drainage of intraabdominal abscesses has become a routine procedure for preoperative confirmation of diagnosis and patient stabilization, and often provides definitive therapy. Diverticulitis is a frequent cause of pericolonic abscess but rarely results in colouterine fistula and pyometrium. We describe the first reported percutaneous drainage of such a case of pyometrium.

This article presents the case of a patient with an acute late infection of the hip prosthesis. At first, complaints in the hip region were in the foreground. Shortly after the revision operation the patient noticed a barking noise during micturition, as sign of a pneumaturia. The following diagnostics showed a perforated sigmoid diverticulitis with a sigmoid-urinary bladder-fistula.

We report a case of sigmoido-vesical fistula due to sigmoid diverticulitis. Magnetic resonance imaging enabled us to visualize the fistula itself in the bladder wall. Magnetic resonance imaging was highly effective in making a precise diagnosis and also provided important additional information for the preoperative work-up of the patient.

Small intestinal diverticulosis is a rare condition, often asymptomatic until complicated with diverticulitis, bleeding, obstruction, or perforation. It predominantly affects elderly men and may present concomitantly with colon diverticulosis. We report the case of a 94-year-old Hispanic woman brought to the emergency department presenting with an acute abdomen. On physical examination, a tense, tender mass was seen in her left inguinal region, initially suspected as an incarcerated inguinal hernia. An abdominal computed tomography scan showed that this mass corresponded to a Nuck cyst. An enc…

An 86-year-old woman presented with a history of endoscopic papillary sphincterotomy for bile duct stones and diverticulitis. The patient was admitted as an emergency case of acute cholangitis due to choledocholithiasis, underwent endoscopic bile duct stenting, and was discharged with a plan for endoscopic lithotripsy. One month later, the patient was readmitted owing to abdominal pain. Abdominal computed tomography at admission showed that the bile duct stent had migrated to the sigmoid colon and the presence of a small amount of extraintestinal gas, suggesting a colonic perforation. Lower ga…
