Reconstructive Middle Ear Surgery: Techniques to improve hearing.

Conductive hearing loss is a common cause of deafness and disability, particularly in children and young adults. This article presents a brief overview of the various methods currently available for reconstructing the tympanic membrane and middle ear ossicular chain, including some comments as to their indications and limitations. Schematic diagrams showing these techniques illustrate the various types of repair described.

Characterization of cytokines present in middle ear effusions.

Retention of inflammatory mediators and cells in the middle ear cleft during chronic otitis media with effusion (COME), results in ongoing inflammation with the potential for pathologic changes and hearing loss. Cytokines are glycoproteins produced by macrophages and other cells. Activities of cytokines include fever production, osteoclast, fibroblast, phagocyte and cytotoxic cell activation, regulation of antibody formation, and inhibition of cartilage, bone and endothelial cell growth. Using enzyme-linked immunospecific assays we measured levels of six cytokines in middle ear effusions (MEE)…

[Inner ear damage induced by bacteria (Mycobacterium chelonae)].

Mycobacterium chelonae has a pathogenicity for mouse inner ear, resulting in "spinning disease". The mice were subjected to intravenous injection of 1-1.2 x 10(7) organisms of strain ATCC19977 of M. chelonae subspecies abscessus. Morphological changes in the inner ear were investigated together with changes in behavior and auditory brainstem response (ABR). Three days after injection, the threshold of ABR rose in high frequency and the latency of the fifth wave prolonged. The behaviors of the mice such as "grooming" or "sniffing" were suppressed day by day. Fourteen days after injection, 35% o…

Staged, laminated, costal cartilage framework for ear reconstruction.

Two features of greatest importance in achieving a normal appearance in the reconstruction of an ear with microtia are that a conchal component with sufficient size, shape, and depth is present; and that auricular projection, sufficient in degree to show contours that appear normal from a posterior view, is present. The authors describe the construction of a staged, laminated framework that helps achieve these goals.

[Viral origin of postnatally acquired inner ear damage].

Experimental and clinical data exist which point at a possible role of herpes virus infections in postnatally acquired inner ear disturbances. The presence of viral DNA in the inner ear and vestibular organ of experimentally infected mice and pigs suggests that not only primary infections but also the reactivation of latent herpes viruses can cause severe inner ear damage. Serological examinations are presented and therapeutic considerations discussed.

Major congenital ear malformations: surgical management and results.

Fifteen patients with major congenital aural atresia underwent operations using an anterior (trans-atretic bone) approach. Facial nerve moni … made. First, one of the most important factors in proper patient selection is the degree of middle ear development on computed tomography, both in terms of size and ossicular formation. Second, every attempt should be made to keep the ossicular chain intact (versus use of a prosthesis), as this appears to optimize hearing results. Third, embryologic …

Hearing and middle ear function in rheumatoid arthritis.

The hearing and middle ear function in 68 patients with rheumatoid arthritis were tested. Reduced middle ear compliance, presumably due to stiffening of the tympanossicular chain, was noted in 48 patients (70.50%). Conductive hearing loss and absence of the stapedius reflex, were recorded in only one patient (1.40%). In 20 patients (29.40%) a moderate (20-30 dB) high frequency sensorineural hearing loss was found. The results of loudness balance and tone decay tests and the speech discrimination scores suggested cochlear pathology in some cases. However the multiplicity of factors that might b…

[Significance of computerized tomography in middle ear diagnosis].

Due to the important improvement represented by the latest computed tomography (CT) technology in respect of detail and excellent contrast resolution normal and pathological structures of the inner and middle ear can be visualised. The clinical and diagnostic value of CT in comparison to standard x-rays and tomography is prospectively evaluated on the basis 74 patient examinations. Today CT is unsurpassed in the diagnosis of cholesteatomas and glomus tumours and ranks first among all imaging modalities. Chronic inflammatory diseases and deformities of the middle ear can be evaluated in most ca…

Unilateral Meniere's disease: is the contralateral ear normal?

Certain investigations in patients with unilateral Meniere's disease may on occasion show abnormalities in the completely symptomless contralateral ear. These tests include transtympanic electrocochleography, the acetazolamide cochlear hydration test, vestibular aqueduct tomography, and caloric testing. Eventually these ears may well become symptomatic. Previous studies have shown that otoadmittance changes are a sensitive indicator of glycerol-induced intracochlear pressure alterations in hydropic ears, but do not occur in patients without Meniere's disease. Otoadmittance parameters were eval…

Subarachnoid space: middle ear pathways and recurrent meningitis.

Congenital bony abnormalities of the inner ear may result in a communication between the middle ear and the subarachnoid space. Patients with this anomaly often present with recurrent meningitis associated with acute otitis media or with middle ear fluid. This article presents three cases of recurrent meningitis with open middle ear--subarachnoid space connections. The first two cases involve a cerebrospinal fluid leak into the middle ear via the oval window, both patients having a Mondini-type of inner ear deformity. The pathway in the third case opened into the middle ear along the horizonta…

Hormonal control of hamster ear sebaceous gland lipogenesis.

The sites and hormonal control of lipogenesis in hamster ear sebaceous glands are reported. Sebaceous lipogenesis was determined in ear biopsies by incubation with glucose and tracer concentrations of 14C-acetate in buffer. The 14C-labeled lipids were saponified, extracted, and determined by liquid scintillation counting. Histologically, the ears contained many sebaceous glands. The glands of male animals were much larger and more heavily lipid-stained than glands from females. Lipogenesis was almost entirely confined to the sebaceous glands in the dermal stroma. Lipogenesis was considerably h…

[Minor abnormalities of the internal ear. Radioclinical correlations].

Moderately severe cochleovestibular disorders of often late onset may arise from minor dysplasias of the internal ear. Diagnosis of these bone dysplasias is radiologic. The problem that arises is that of the indications for radiotomography of the internal ear, insofar as on the one hand no certain radioclinical equivalence exists, and on the other hand this radiologic examination is of greater diagnostic than therapeutic value.

The relationship between nasal and middle ear pathology.

The results are presented of a prospective clinical-epidemiological study in 2069 healthy children 2 1/2-6 years of age, to determine the epidemiology of SOM and the role of some predisposing factors. Only the study of rhinological parameters is presented in this report. It was demonstrated that there is a very strong correlation between the annual incidence of common colds and of acute otitis media (p less than 0.0001). The correlation between common colds and the tympanometrical findings was not so high but still obvious (p less than 0.05), because of the higher number of negative middle ear…

Bilateral middle ear carcinomas associated with Waldenström's macroglobulinemia.

A case of bilateral middle ear squamous cell carcinomas with concurrent Waldenström's macroglobulinemia is described. An increased incidence of malignant tumors occurring in patients with this potentially fatal, chronic lymphoproliferative disorder is well recognized, but the association with an otological tumor has not previously been reported. The relationship between the two conditions is discussed.

Twelfth nerve neurilemmoma occurring in the middle ear.

Neurilemmomas of the 12th cranial nerve are rare tumors. We present the first reported case of one occurring in the middle ear. This report details the diagnosis and treatment of a large 12th nerve neurilemmoma in a patient whose initial symptoms included vertigo, otalgia, and left-sided tongue fasciculations with atrophy. Selective carotid angiography and computed tomography scanning defined the extent of the lesion. It involved the skull base and parapharyngeal space with extension into the middle ear and mastoid to the level of the middle fossa and posterior fossa dura. Treatment consisted …

Hearing and middle ear function in osteogenesis imperfecta.

Fifty-five patients with osteogenesis imperfecta (OI) were studied to determine the extent to which the peripheral auditory mechanisms share in the connective tissue lesion. Ninety-two unaffected relatives and 43 control subjects were also studied. Subjects were divided into age groups younger than and older than 30 years. Hearing loss, most frequently sensorineural, occurred in 49% (younger than 30 years) and 94% (older than 30 years) of patients with OI. A sensorineural pattern of hearing loss, here considered characteristic of OI, was observed in 47% of OI subjects irrespective of age, in 4…
