BioGlue and E-Vita Open NEO graft oozing: Long-term solution or band aid?

BACKGROUND: The introduction of the single-step total arch replacement (TAR) with frozen elephant trunk (FET) has revolutionised the field of aortic surgery. TAR is indicated when the aortic arch is involved in aortic pathologies such as Type A aortic dissection and thoracic aortic aneurysms. Several FET devices are available commercially for global use, and example is the E-Vita Open NEO hybrid prosthesis (HP) developed by CryoLife-JOTEC. Unlike other FET devices available, this HP in particular features a design that puts it at a disadvantage as it does not incorporate gelatine or collagen i

Repair of retrograde ascending dissection after descending stent grafting.

OBJECTIVE: Retrograde dissection is now recognized as an important complication after thoracic endovascular aortic repair (TEVAR), but its treatment is poorly understood. Our objectives were to investigate the risks, describe the repair methods, and assess the outcomes of this complication. METHODS: From 2000 to 2012, 766 patients underwent TEVAR. Of these patients, 14 (1.8%), plus 1 who had undergone TEVAR elsewhere (n = 15), developed retrograde dissection after stent grafting. They had undergone TEVAR for distal aortic dissection in 7, intramural hematoma in 5, aneurysm in 2, and transecti

Should we operate on patients with acute type A aortic dissection who present with cardiopulmonary resuscitation?

OBJECTIVES: Acute aortic dissection type A (AADA) is a life-threatening medical emergency. Emergent surgical repair is the gold standard but mortality remains high. Mortality is even higher in patients who arrive at the hospital in poor condition, especially after cardiopulmonary resuscitation (CPR). This study was designed to analyse the outcome of patients who underwent surgery for AADA and who require preoperative CPR. METHODS: Between 2000 and 2023, 810 patients underwent emergent surgery for AADA at our centre. Of these, 63 had preoperative CPR. We performed a retrospective analysis with

Urgent Thoracic Endovascular Aortic Repair for Type-B0,D Acute Aortic Dissection.

BACKGROUND: The objective of the present study is to clarify safety and efficacy of thoracic endovascular aortic repair (TEVAR), excluding the primary entry in the descending aorta, for type-B0,D acute aortic dissection (TB0,DAAD) (so-called retrograde type-A acute aortic dissection). METHODS: Forty-six patients with hyperacute-phase (within 2 days after the onset) type-A acute aortic dissection (TAAAD) and TB0,DAAD underwent urgent (on the admission or next day) intervention (TEVAR or conventional surgical aortic repair [CSAR]) for 2 years. Results of TEVAR for TB0,DAAD were compared with th

Size matters in metabolic scaling: Critical role of the thermodynamic efficiency of ATP synthesis and its dependence on mitochondrial H+ leak across mammalian species.

In this last article of the trilogy, the unified biothermokinetic theory of ATP synthesis developed in the previous two papers is applied to a major problem in comparative physiology, biochemistry, and ecology-that of metabolic scaling as a function of body mass across species. A clear distinction is made between intraspecific and interspecific relationships in energy metabolism, clearing up confusion that had existed from the very beginning since Kleiber first proposed his mouse-to-elephant rule almost a century ago. It is shown that the overall mass exponent of basal/standard metabolic rate

Comparable Outcomes in Redo Total Arch Replacement for Previous Aortic Dissection vs. Other Cardiac Surgeries: A Single-Center Pilot Study of the E-Vita Open Hybrid Prosthesis.

Background/Objectives: Total arch replacement (TAR) with frozen elephant trunk (FET) using the E-vita Open hybrid prosthesis represents a complex surgical intervention for extensive aortic pathologies in previously operated patients. The comparative safety profile between patients with previous acute Type A dissection repair versus other cardiac surgical histories remains unclear. This pilot study evaluated early and midterm outcomes to determine whether previous aortic dissection carries additional operative risk compared to other previous cardiac operations. Methods: This retrospective singl

Outcomes of great vessel debranching to facilitate thoracic endovascular aortic repair.

OBJECTIVE: With the expanding application of endovascular technology, the need to deploy into zone 0 has been encountered on occasion. In the present study, we evaluated the outcomes of great vessel debranching (GVD) as a method of extending the proximal landing zone to facilitate thoracic endovascular aortic repair (TEVAR). METHODS: We performed a single-center retrospective review of all patients who had undergone GVD followed by TEVAR between May 2013 and December 2020. The primary outcome was primary patency of all targeted vessels, with all-cause perioperative mortality as a secondary ou

Comparison of Single Axillary vs. Dual Arterial Cannulation for Acute Type a Aortic Dissection: A Propensity Score Matching Analysis.

BACKGROUND: The optimal arterial cannulation site for acute aortic dissection repair is unclear, especially for complex arch surgery. Axillary artery cannulation is widely accepted but adding femoral artery cannulation to it was considered to potentially improve perfusion and early outcomes. To clarify this point, a comparison of perioperative outcomes for these two different cannulation strategies was conducted regarding the pathological features of dissection. METHODS: From January 2010 to December 2019, 927 consecutive patients underwent a total arch replacement combined with frozen elepha

A Tale of 20 Alphaviruses; Inter-species Diversity and Conserved Interactions Between Viral Non-structural Protein 3 and Stress Granule Proteins.

Alphaviruses infect a diverse range of host organisms including mosquitoes, mammals, and birds. The enigmatic alphavirus non-structural protein 3 (nsP3) has an intrinsically disordered, C-terminal hypervariable domain (HVD) that can interact with a variety of host proteins associated with stress granules (SGs). The HVD displays the highest variability across the more than 30 known alphaviruses, yet it also contains several motifs that are conserved amongst different subgroups of alphaviruses. For some alphaviruses, specific nsP3-SG protein interactions are essential for virus replication. Howe

Impact of the society for vascular surgery/society of thoracic surgeons classification on prognosis and therapeutic strategies in acute Stanford type A aortic dissection.

OBJECTIVE: The Stanford classification does not differentiate between antegrade and retrograde primary entry tears in acute Stanford type A aortic dissection (ATAAD). The 2020 Society for Vascular Surgery/Society of Thoracic Surgeons (SVS/STS) classification categorizes ATAAD into SVS/STS-A and SVS/STS-B0 based on the location of the primary entry tear; however, its clinical utility requires further validation. This study aims to evaluate the impact of the SVS/STS-A and SVS/STS-B0 on the prognosis and strategies for ATAAD. METHODS: We retrospectively reviewed 931 consecutive ATAAD patients at

Efficacy and Safety of preoperative Controlling Heart Rate in Patients with Acute Type A Aortic Dissection (ESCORT): protocol for a randomised controlled trial.

INTRODUCTION: Aortic dissection (AD) is a life-threatening cardiovascular emergency with a high mortality rate, and acute Stanford Type A AD is the most dangerous form, often requiring surgical intervention. Factors contributing to the progression of AD include increased heart rate, blood pressure and the rate of ventricular contraction (dP/dt). While heart rate control is a cornerstone of preoperative management for AD per existing guidelines, the recommended target (eg, ≤60 bpm in American Heart Association 2010) is consensus-driven (Level C) and thus not grounded in high-level evidence. Blood Product Transfusion and Antifibrinolytic Choice Are Not Associated with Higher Rates of Intraluminal Thrombus Following Non-Emergency Aortic Arch Replacement Surgery.

Background: Intraluminal thrombus (ILT) is a recognized complication of aortic arch replacement surgery using a frozen elephant trunk (FET) prosthesis, with a postoperative incidence of around 10%. Risk factors for ILT include aneurysmal aortic disease, central graft positioning, and prothrombotic medical conditions. Perioperative blood product transfusion is frequently required in these patients to correct postoperative coagulopathy following cardiopulmonary bypass (CPB). We hypothesized that there may be an association between (i) intra- and postoperative blood product transfusion volumes an
