OBJECTIVE: Management of recurrent Dupuytren's disease of the little finger is challenging. Various treatment modalities have been proposed: external fixation, local skin flap, dermofasciectomy, or even amputation. An alternative surgical technique was introduced by Honecker et al. in 2016 and refined by Raimbeau et al. in 2019, consisting in resection of the middle phalanx and shortening arthrodesis. We modified the technique by combining arthrodesis with a limited fasciectomy of the abductor and/or pretendinous cord in the fifth ray to improve cosmetic and functional outcomes.

Closed and open injuries of the extensor mechanism at the proximal interphalangeal (PIP) joint can involve the central slip, the lateral slips or both. They are classified as zone III injuries. All open injuries on the dorsal side of the PIP joint should raise suspicion of an extensor tendon injury that is frequently overlooked. The operative strategy consists of wound revision with extensor tendon suture or refixation of the central slip. Acute closed central slip injuries are clinically diagnosed (Elson test) after ruling out bony injuries to the joint. Nondisplaced avulsions of the central

The sport of swimming has been affected by the innovations of technology with the evolution of the swimsuit. The health benefits of swimming are numerous and are well documented in the scientific literature. As swimming is a low-impact sport, injuries are relatively uncommon. As a direct result of the new swimsuits, team physicians have identified the emergence of a new trend in injury in the aquatic athlete. Extensive blistering and ulceration of the finger tips and distal interphalangeal joints in addition to ecchymoses of the lower limb are now common in the aquatic athlete wearing the new

A 35-year-old woman presented with a pathological fracture of a solitary cystic enchondroma in the proximal phalanx of her right index finger (extending to the articular surface with expansion of the cortex). The affected segment was excised and reconstructed with an autograft from the proximal phalanx of her right third toe. At the 2-year follow-up, the graft was well taken, the articular cartilage was intact, and the range of motion was good. There was no evidence of osteoarthrosis.

Stable fractures of the metacarpal and proximal phalanges can be treated satisfactorily by closed methods. The problem in those unstable fractures is whether to treat with open or closed reduction. Stiffness of the proximal interphalangeal joint (PIP), malunion, rotation deformities, shortening, and nonunion are known complications of fractures of the proximal phalanx. Fractures at the base of the proximal phalanx. Fractures at the base of the proximal phalanx have low incidence of complications if they are treated closed. Fractures at the shaft carry a much larger risk of complications with e

Of 359 hands treated surgically for Dupuytren's contracture, 135 were available for study after two years or more, often because of recurrence. Of the sixty-five hands treated by excision of the involved fascia (subtotal fasciectomy), 63 per cent had recurrences in the area operated on but only 15 per cent had sufficient deformity to warrant another operation. Of the forty-one hands treated by palmar fasciotomy, 43 per cent had recurrent deformities severe enough to warrant further surgical treatment. After palmar fasciotomy, improvement at the metacarpohalangeal joint persisted; but, as expec

RATIONALE: When surgical treatment is indicated for primary trigger finger, open A1 pulley release has traditionally been recommended with generally good results. However, ganglion cysts of the flexor tendon sheath as a complication after an open A1 pulley release were rarely reported. Therefore, the purpose of this study is to report a case of multiple ganglion cysts arising from the flexor tendon sheath in a patient undergoing an open A1 pulley release for trigger finger disorder with a review of the relevant literature.
PATIENT CONCERNS AND DIAGNOSIS: A 65-year-old right-handed farmer was

BACKGROUND: Extensor pollicis longus (EPL) tendon ruptures have been treated succesfully with the transfer of the extensor indicis proprius (EIP) tendon. Situations exist in which, due to intraoperative observations, another tendon transfer may be considered preferable to the standard EIP transfer method.
OBJECTIVES: To determine whether transfer of the extensor digitorum communis II (EDC II) tendon from the index finger to the EPL tendon, leaving the EIP tendon to the index finger intact, would serve as an equally efficient transfer and not adversely affect the function of the hand.

Text entry remains a bottleneck for productivity-oriented Virtual Reality (VR), especially in scenarios where optical hand tracking is unstable because of self-occlusion, poor lighting, or out-of-view interaction. We present SlideRing, a dual-thumb wearable text-entry method that senses thumb-to-finger micro-gestures with two miniature Inertial Measurement Units (IMUs). SlideRing defines a 30-command interaction space from two hands, three target fingers, and five gesture types, then maps these commands to a full alphabetic keyboard through two complementary strategies: an ergonomic layout opt

INTRODUCTION: Pathologic phalangeal fracture is a common initial presentation of solitary enchondroma of the finger. Enchondromas of the middle phalanx are most frequently associated with post-operative range of motion deficits. This report describes a case in which the use of Kirschner wires (K-wires) and allograft bone were used to salvage finger motion following surgical fixation of pathologic fracture of a middle phalanx enchondroma with extensor tendon injury.
CASE REPORT: A 37-year-old right hand dominant woman presents with pathologic fracture of the left index finger middle phalanx. T

Extensor tendons in the dorsal hand especially at or close to the metacarpophalangeal joint area and those of the thumb are solid and round in cross-sections. A survey shows that most surgeons are using less strong surgical repair methods, often with over-protection after surgery. In this article, strong surgical repairs (6-strand repairs) are recommended for these areas based on author's experience. These tendons are treated with surgical methods and principles of current flexor tendon surgery. The finger is protected with short splinting with early active motion or hand use. After the extens

No ligamentous laxity was demonstrated in the finger joints. A fracture involving one-third of the phalanx may be caused by forced hyperextension of that joint. Background and Clinical Significance: Heat-press injuries of the hand can cause deceptively deep, progressive tissue damage, and dorsal multi-digit involvement carries a high risk of joint stiffness and scar contracture. Case Presentation: A 58-year-old left-hand-dominant woman sustained a dorsal heat-press injury affecting the left index to small fingers; we performed staged reconstruction with tangential debridement and artificial dermis placement (Day 9) followed by full-thickness skin grafting (FTSG) from the right infraclavicular region (Day 23), with supervised rehabilitation from Day 15

Arachnodactyly literally means spidery fingers, and describes the long, slender fingers typical of patients with Marfan syndrome (MFS). Many clinicians regard arachnodactyly as pathognomonic of MFS; however, this view is misleading as arachnodactyly is a key element of the marfanoid habitus, which is present in several heritable disorders of connective tissue (HDCTs). Other features of the marfanoid habitus include long hands and feet, increased skin stretch, joint hypermobility and characteristic changes in the physiology of the pectum. Here, we focus on the differential diagnosis of diseases

The authors report the results of 30 Swanson implants, inserted immediately, or secondarily in complex trauma of the proximal interphalangeal joint of long fingers in 24 patients (19 men, 5 women). All patients were reviewed by the same examiner with a mean follow-up of 7 years (range: 17 months-17.2 years). All implants were inserted secondarily after the accident, except in 7 cases in which the local conditions led the operator to insert the implant immediately. At review, 20 implants were still in place, 6 patients had requested amputation, and arthrodesis had been performed in 4 cases. The
