Structural and functional alterations in MRI-negative drug-resistant epilepsy and associated gene expression features.

Neuroimaging techniques have been widely used in the study of epilepsy. However, structural and functional changes in the MRI-negative drug-resistant epilepsy (DRE) and the genetic mechanisms behind the structural alterations remain poorly understood. Using structural and functional MRI, we analyzed gray matter volume (GMV) and regional homogeneity (ReHo) in DRE, drug-sensitive epilepsy (DSE) and healthy controls. Gene expression data from Allen human brain atlas and GMV/ReHo were evaluated to obtain drug resistance-related and epilepsy-associated gene expression and compared with real transcr

Graph-based deep learning models in the prediction of early-stage Alzheimers.

Alzheimer's disease is the most common age-related problem and progresses in different stages, from cognitively normal to early mild cognitive impairment, and severe dementia. This study investigates the predictive potential of resting-state functional magnetic resonance imaging (rs-fMRI) and its derived functional connectivity (FC) in understanding Alzheimer's progression. Leveraging deep learning and graph-based models, we introduce two key contributions: 1) a comparative analysis of rs-fMRI time points and FC for Alzheimer's prediction. 2) an innovative graph transformer variant incorporati

The posterior middle temporal gyrus serves as a hub in syntactic comprehension: A model on the syntactic neural network.

Neuroimaging studies have revealed a distributed neural network involving multiple fronto-temporal regions that are active during syntactic processing. Here, we investigated how these regions work collaboratively to support syntactic comprehension by examining the behavioral relevance of the global functional integration of the syntax network (SN). We found that individuals with a stronger resting-state within-network integration in the left posterior middle temporal gyrus (lpMTG) were better at syntactic comprehension. Furthermore, the pair-wise functional connectivity between the lpMTG and t

Enhanced functional connectivity of the default mode network (DMN) in patients with spleen deficiency syndrome: A resting-state fMRI study.

Numerous studies had investigated the biological basis of spleen deficiency syndrome on gastrointestinal dysfunctions. However, little was known about neuropsychological mechanism of spleen deficiency syndrome. The default model network (DMN) plays an important role in cognitive processing. Our aim is to investigate the change of neuropsychological tests and DMN in patients with spleen deficiency syndrome.Sixteen patients and 12 healthy subjects underwent functional magnetic resonance imaging examination, and 15 patients with spleen deficiency syndrome and 6 healthy subjects take part in the t

Functional hierarchy underlies preferential connectivity disturbances in schizophrenia.

Schizophrenia may involve an elevated excitation/inhibition (E/I) ratio in cortical microcircuits. It remains unknown how this regulatory disturbance maps onto neuroimaging findings. To address this issue, we implemented E/I perturbations within a neural model of large-scale functional connectivity, which predicted hyperconnectivity following E/I elevation. To test predictions, we examined resting-state functional MRI in 161 schizophrenia patients and 164 healthy subjects. As predicted, patients exhibited elevated functional connectivity that correlated with symptom levels, and was most promin

Neuroimaging of tic genesis: Present status and future perspectives.

Tics are hyperkinetic movements that are distinctive by their variety in semiology and duration and by their ability to be modulated by cognitive control. They are the hallmark of Gilles de la Tourette syndrome. Despite the variety of clinical presentations in this syndrome, dysfunction of cortico-striato-pallido-thalamo-cortical networks is suggested as a core pathophysiological mechanism. We review recent structural and functional neuroimaging studies that focused on the anatomical substrate of tics and their possible genesis. These studies showed a consistent relationship between structural…

[Dystonia: contributions of functional imaging and magnetoencephalography].

Functional neuroimaging using positron emission tomography (PET), and more recently functional MRI (fMRI) and magnetoencephalography (MEG), is a valuable tool to study functional anomalies in dystonia. Activation studies have contributed to a better understanding of cerebral dysfunction in dystonia showing two main types of abnormalities: changes in activation levels during performance of sensory or motor tasks and disorganization of the selectivity of neuronal representations. In primary dystonia, most PET and fMRI studies have shown overactivity in premotor and prefrontal areas and underacti…

A Rare Presentation of Functional Movement Disorder Mimicking Normal Pressure Hydrocephalus.

Herein, we describe a 55-year-old female patient with a functional movement disorder (FMD) who presented with normal pressure hydrocephalus (NPH)-like clinic. The neuroimaging data and positive response to the tap test initially suggested NPH. However, a detailed investigation of the clinic features yielded a final diagnosis of FMD. Via the presentation of this patient, we expand the phenomenology of FMD. To our knowledge, this is the first presentation of a patient with FMD mimicking NPH. Therefore, we think this rare illustration is interesting and may provide valuable perspectives for clini

'Where there's smoke': longitudinal cognitive disintegration after postoperative delirium?

Postoperative delirium is an important complication of surgery and is associated with poor long-term cognitive outcomes, although the neural basis underlying this relationship is poorly understood. Neuroimaging studies and network-based approaches play an important role in our understanding of the mechanism by which delirium relates to longitudinal cognitive decline. A recent resting state functional MRI study is reviewed, which shows reduced global connectivity up to 3 months after delirium, supporting recent models of delirium and opening the door for applying this approach to understanding

Evaluation, Treatment, and Outcomes of Viral and Autoimmune Encephalitis in Children.

Viral encephalitis and autoimmune encephalitis are currently the most common causes of encephalitis. Determining the causative agent is helpful in initiating medical treatment that may help reduce long-term sequelae. Cerebrospinal fluid, neuroimaging, serologic, and electroencephalogram in combination with clinical manifestations play a role in determining the cause of the encephalitis. Although motor dysfunction tends to improve, there is a significant risk of long-term neurologic and cognitive sequelae. These persistent deficits that occur in childhood indicate the importance for ongoing reh

Embouchure Dystonia as a Network Disease.

While the pathophysiology of embouchure dystonia, a sub-entity of musician's dystonia, is still not fully understood, recent research has shown that it involves alterations of several brain functions and networks. Maladaptive plasticity in sensorimotor integration, sensory perception, and deficient inhibitory mechanisms at cortical, subcortical, and spinal level seem to contribute to its pathophysiology. Furthermore, functional systems of the basal ganglia and the cerebellum are involved, clearly pointing toward a network disorder. We therefore propose a novel network model, based on electroph

What does the hippocampus do during working-memory tasks? A cognitive-neuropsychological perspective.

In this commentary, we highlight the role of the hippocampus as a binding device that may explain its recruitment during associative working-memory paradigms. Furthermore, we argue that both functional neuroimaging research, as presented in Slotnick (this issue), and carefully designed lesion studies in patients with selective bilateral hippocampal damage are crucial for advancing our understanding of the neural structures and processing involved in human memory in general and disentangling the role of the hippocampus proper and other medial temporal lobe structures in working-memory function …

Psychogenic movement disorders: aetiology, phenomenology, neuroanatomical correlates and therapeutic approaches.

Paralysis and movement disorders unexplained by identifiable neurological disease are common clinical presentations in neurological outpatient clinics. Despite their frequency and potential importance from a neuroscientific viewpoint these disorders have received surprisingly little attention in clinically oriented neuroscientific research. In this review we summarize the available literature that specifically relates to the functional neuroanatomy of psychogenic movement disorders. We discuss the aetiology, clinical phenomenology, the current concepts of the dynamic neuroanatomical networks u

The neuropsychology of mood disorders.

Cognitive dysfunction is central to our understanding of mood disorders in terms of patient experiences, Diagnostic and Statistical Manual of Mental Disorders criteria, and psychological models. In this article, we highlight key findings from studies that have used neuropsychological tests and functional neuroimaging techniques to explore cognitive dysfunction in patients with depression and mania. In particular, we focus on affective processing bias, abnormal response to negative feedback, and decision making. Results are discussed in the context of current conceptualizations of dysfunctional…

Recurrent artery of Heubner infarction in infancy.

Classically, acquired occlusion of the recurrent artery of Heubner (RAH) results in hemiparesis with faciobrachial predominance. Infarction in the territory of the RAH represents a specific stroke syndrome not yet described in infancy with a range of motor and functional manifestations. An infant is described with apparent congenital infarction of the recurrent artery of Heubner. The child had prominent involvement of the contralateral upper extremity with athetosis. Neuroimaging changes were evident in the vascular territory classically attributed to the RAH. The clinician should suspect cong

Neuropsychiatric factors in the illusion of visitors among geriatric patients: a case series.

The 'illusion of visitors' is a common phenomenon among geriatric patients presenting for psychiatric or neurologic evaluation and treatment. Although these illusory beliefs are etiologically diverse, patients may commonly have visual impairment and functional and/or structural disruption of frontal and right-hemisphere-mediated cognitive functioning. This article outlines eight cases of illusory beliefs among elderly patients, presenting psychiatric, neurologic, neuroimaging, and neuropsychological findings among these patients. Commonalities and differences among these cases are discussed, a

What traditional neuropsychological assessment got wrong about mild traumatic brain injury. I: historical perspective, contemporary neuroimaging overview and neuropathology update.

PRIMARY OBJECTIVE: This is Part I of a four-part review that examines traditional neuropsychological assessment methods and techniques in mild traumatic brain injury (mTBI). Absence of neuropsychological findings has been used to argue no residual neuropathological effects from mTBI. However, given the current potential that advanced multimodality and quantitative neuroimaging can now demonstrate about underlying neurobiology of brain-behavior relations, this review shows that traditional neuropsychological test as standalone findings cannot directly address the underlying complexities of dete

Neuroimaging study designs, computational analyses and data provenance using the LONI pipeline.

Modern computational neuroscience employs diverse software tools and multidisciplinary expertise to analyze heterogeneous brain data. The classical problems of gathering meaningful data, fitting specific models, and discovering appropriate analysis and visualization tools give way to a new class of computational challenges--management of large and incongruous data, integration and interoperability of computational resources, and data provenance. We designed, implemented and validated a new paradigm for addressing these challenges in the neuroimaging field. Our solution is based on the LONI Pip

Desynchronise, disconnect, reorganise, collapse (DDRC): A theoretical framework linking white matter pathology and cognitive dysfunction in vascular disease.

White matter health is closely associated with cognitive function, particularly across the spectrum of cerebrovascular health. However, inconsistencies in the cognitive domains reported to be affected by white matter pathology have complicated efforts to define clear mechanisms of dysfunction. This review synthesises neuroimaging and neuropathological evidence to reveal the cascade of cognitive and neural consequences that follow white matter injury, from microstructural alterations to macroscale white matter hyperintensities. We discuss the contributions of grey matter atrophy and disrupted f
