Hypertrichosis cubiti.

Hypertrichosis is an unusual but well-recognized genetic condition. Hypertrichosis may be generalized or limited to specific body areas, in which case it is usually not associated with other anomalies. Five previous cases of hypertrichosis cubiti have been reported, with short stature in 2 sibs being the only other associated abnormalities. We report on a child with hairy elbows, developmental delay, facial asymmetry, and delayed speech with normal parents. Our patient may represent severe expression of the hairy elbow syndrome or constitute a previously unrecognized syndrome.

The actomyosin cortex controls t-tubule remodeling in skeletal muscle.

A network of plasma membrane invaginations called t-tubules plays an essential role in controlling calcium release from the endoplasmic reticulum at the triads during muscle contraction. Although the importance of t-tubules for muscle physiology is well established, and abnormalities are found in muscle disorders, the mechanisms that mediate t-tubule growth are unknown. We show that the actomyosin cortex beneath the plasma membrane, regulated by Arp2/3 complexes containing Arpc5, acts as a gatekeeper for the membrane availability during t-tubule growth. Enlarged t-tubules are formed upon disru

Low FODMAP and Gluten-Free Diet Implementation: A Guide for the General Practitioner.

The low fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAP) and gluten-free diets are increasingly used as both therapeutic and diagnostic interventions for pediatric patients with gastrointestinal disorders. The low FODMAP diet is primarily indicated for children with irritable bowel syndrome or disorders of brain-gut interaction when symptoms persist. Implemented in 3 phases (elimination, reintroduction, and personalization), the diet helps identify individual triggers and improve symptoms, such as bloating and abdominal pain. In contrast, the gluten-free diet

Spontaneous megakaryocytic colony formation does not discriminate between essential thrombocythemia and polycythemia vera.

Laboratory detection of spontaneous growth of colony-forming unit-megacaryocytes (CFU-MK), allowing us to distinguish essential thrombocythemia (ET) from reactive thrombocytosis, is therefore useful for the diagnostic of this myeloproliferative disorder. Whether CFU-MK assays allow us to discriminate at least partly between ET and other myeloproliferative disorders such as polycythemia vera (PV) remains, however, to be established. To gain insights about this point, we have performed CFU-MK cultures from bone marrow cells of patients diagnosed with ET (n = 42) or PV (n = 50) using a standardiz

Nutritional Management of Acute and Chronic Pancreatitis.

There is an increased risk of malnutrition in pediatric pancreatic disorders. Acute pancreatitis results in a hypermetabolic and hypercatabolic state and enteral nutrition (EN) may improve patient outcomes. In pediatric pancreatitis, EN should be introduced early and advanced as tolerated. EN is preferred over parental nutrition. Chronic pancreatitis is associated with malnutrition, impaired growth and development, and pancreatic insufficiency. Children with chronic pancreatitis should be routinely screened for exocrine and endocrine pancreatic insufficiency and growth and nutritional deficien

Trends In Reported Outcomes for Growth Hormone Therapy In Children With Growth Hormone Deficiency.

CONTEXT: Although the safety and effectiveness of recombinant human growth hormone therapy (rhGH) has been reported for several years, the level of consensus on the outcomes that should be reported is unclear. OBJECTIVE: This work aims to understand the frequency and trends in reported outcomes of safety and effectiveness for rhGH therapy for growth hormone deficiency (GHD) in childhood. METHODS: A systematic review was performed in 7 English- and Chinese-language databases. Eligibility criteria included all studies published between 2003 and 2022, with participants who started rhGH before a

Impact of Growth Hormone Treatment in Children From an Extended Family With ACAN -Related Short Stature.

Heterozygous variants in the ACAN gene are a recognized cause of short stature. We aimed to evaluate the response to growth hormone (GH) treatment in members of an extended family carrying an ACAN variant. Clinical and laboratory data of five related children were retrospectively collected. GH therapy was initiated at a mean age of 3.2 ± 2.1 years and treated for 6.4 ± 5.0 years. The height standard deviation scores (SDS) increased from -3.3 ± 1.0 to -2.0 ± 1.1 (p = 0.001), with a mean gain of 1.3 ± 0.3 SDS. The most notable increase in growth velocity SDS occurred during the first year of tre

Impact of disease and treatments on growth and puberty of pediatric patients with inflammatory bowel disease.

Growth retardation, associated with delayed puberty, is a frequent feature in pediatric patients with inflammatory bowel disease (IBD), especially with Crohn's disease. It is mainly induced by malnutrition and the effects of the inflammatory process on the growth hormone/insulin-like growth factor-1 axis or on the growth plate. Therefore, control of disease activity and mucosal healing are paramount to promote growth and adequate pubertal onset. Current therapeutic strategies for maintenance in IBD include anti-inflammatory drugs, immunosuppressives, and, more recently, biologic agents. Althou

[The effect of one year therapy with recombinant human growth hormone (rhGH) on growth velocity, calcium-phosphorus metabolism, bone mineral density and changes in body composition in children with growth hormone deficiency (GHD)].

INTRODUCTION: The most significant effect of growth hormone treatment is growth promotion. For adults the metabolic effect is the most important. This treatment has an influence on the forming of correct composition of body mass, on metabolism of osseous tissue and bone mineral density. AIM OF THE STUDY: Comparison between the rate of growth and estimation of calcium-phosphorus metabolism, bone mineral density and change of body composition in children with growth hormone deficiency during the first year of growth hormone treatment. MATERIAL AND METHODS: The treatment included 120 children a

Assessment and treatment of short stature in pediatric patients with chronic kidney disease: a consensus statement.

Growth failure is a clinically important issue in children with chronic kidney disease (CKD) and is associated with significant morbidity and mortality. Many factors contribute to impaired growth in these children, including abnormalities in the growth hormone (GH)-insulin-like growth factor-I (IGF-I) axis, malnutrition, acidosis, and renal bone disease. The management of growth failure in children with CKD is complicated by the presence of other disease-related complications requiring medical intervention. Despite evidence of GH efficacy and safety in this population, some practitioners and f

Growth hormone therapy.

Growth hormone (GH) therapy has revolutionized treatment of children with growth hormone deficiency (GHD). Improved height outcome with final height in the target height range has been achieved in these children. Identification of Creutzfeldt-Jakob disease, a deadly prion mediated disorder, in recipients of pituitary GH accelerated the transition from pituitary derived GH to recombinant GH. Once daily subcutaneous administration of the freeze-dried preparation at evening is the recommended mode of GH therapy. Studies have led to use of higher dose of GH for improving height outcome (0.33 mg/kg

Growth, growth factors and diabetes.

Type 1 diabetes mellitus (T1DM) and other chronic diseases in children are well known to adversely affect linear growth and pubertal development. In the years immediately following the introduction of insulin therapy, short stature was consistently reported in children with T1DM. However, over the past 50 years significant improvement in the prognosis for growth and final height in children with diabetes has been achieved. Although pre-pubertal and post-pubertal growth are important phases in growth, puberty and its related hormonal changes represent a critical phase for growth gain and final

Spontaneous and provoked growth hormone (GH) secretion and insulin-like growth factor I (IGF-I) concentration in patients with beta thalassaemia and delayed growth.

Growth retardation in children with thalassaemia major is multifactorial. We studied the growth hormone (GH) response to provocation by clonidine and glucagon, measured the circulating concentrations of insulin, insulin-like growth factor-I (IGF-I), IGF-binding protein-3 (IGFBP3), and ferritin, and evaluated the spontaneous nocturnal (12 h) GH secretion in prepubertal patients with thalassaemia and age-matched children with constitutional short stature (CSS) (height SDS < -2, but normal GH response to provocation). The anatomy of the hypothalamic pituitary area was studied in patients with abn

Clinical manifestations of zinc deficiency.

The essentiality of zinc for humans was recognized in the early 1960s. The causes of zinc deficiency include malnutrition, alcoholism, malabsorption, extensive burns, chronic debilitating disorders, chronic renal diseases, following uses of certain drugs such as penicillamine for Wilson's disease and diuretics in some cases, and genetic disorders such as acrodermatitis enteropathica and sickle cell disease. In pregnancy and during periods of growth the requirement of zinc is increased. The clinical manifestations in severe cases of zinc deficiency include bullous-pustular dermatitis, alopecia,

Dictyostelium discoideum: A Model System for Neurological Disorders.

BACKGROUND: The incidence of neurological disorders is increasing due to population growth and extended life expectancy. Despite advances in the understanding of these disorders, curative strategies for treatment have not yet eventuated. In part, this is due to the complexities of the disorders and a lack of identification of their specific underlying pathologies. Dictyostelium discoideum has provided a useful, simple model to aid in unraveling the complex pathological characteristics of neurological disorders including Alzheimer's disease, Parkinson's disease, Huntington's disease, neuronal c

Infants and Young Children with Children's Interstitial Lung Disease.

Though interstitial lung disease (ILD) can occur at any age in children, disorders more common in infancy and young children have received increased attention as an important group that is disproportionally affected, linked to lung development and lung injury, and represents disorders not seen in adult ILD. Identifying those children with potential children's ILD (chILD) and establishing a specific chILD diagnosis has evolved and is critical for pediatric pulmonologists, neonatologists, radiologists, and pathologists to recognize. Specific disorders more common in infancy include diffuse devel

Early fetal growth retardation: obstetric background and recurrence rate.

The obstetric background of early fetal growth retardation, leading to intrauterine death between 25 and 34 weeks or to delivery before 34 weeks' gestation, was investigated in a group of 100 women. Hypertensive disorders were the most common causative factor (59%). Other causes included antepartum hemorrhage and congenital anomalies. In 20% of the cases no obvious explanation for the fetal growth retardation could be found. The recurrence rate of fetal growth retardation in 49 women who had a subsequent pregnancy was found to be nearly 50%. In one-third the severity of growth compromise was c
