Worth a Second Look: Report of a Rare Traumatic Pisiform Fracture Following a Bicycle Fall.

Fractures of the pisiform bone are difficult to diagnose due to the often unsuspicious conventional X-ray images and nonspecific clinical presentation. The following case presents a 29-year-old patient who suffered a bicycle fall onto their right hand and experienced prolonged symptoms despite the initial lack of evidence of a fracture. The fractured pisiform bone, which was eventually detected in a CT scan, was successfully treated with a 4-week period of cast immobilization. The existing conservative and surgical treatment options of a pisiform bone fracture are discussed.

Bizarre parosteal osteocondromatous proliferation (BPOP) of the acromion with soft tissue recurrence.

Bizarre parosteal osteochondromatous proliferation (BPOP) is a benign but rare periosteal-originating chondrogenic tumor. It commonly arises from the hands and feet. It is slow-growing and often presents as a painless lump. On imaging, the mass is well-marginated and almost always remains contiguous with the cortical bone. Histologically, the lesion is composed of a disorganized admixture of fibrous tissue, bone, and cartilage with bizarre features. Treatment is surgical and local recurrence is common contiguous with bone. This case report demonstrates an uncommon acromial BPOP with the first

Does Anyone Need a Vascularized Graft?

Many hand surgeons argue that vascularized bone grafting is indicated in proximal pole avascular necrosis, prior failed surgery, or long-standing scaphoid nonunion. However, the available evidence does not support improved treatment outcomes for vascularized bone grafting rather than traditional nonvascularized techniques. This article addresses the available evidence and examines the role of vascularized bone grafting in scaphoid nonunion treatment. It also identifies important factors that influence healing, clarifies the decision-making algorithm, and proposes areas for further research.

Healing of a scaphoid nonunion using human bone morphogenetic protein.

A chronic nonunion of a proximal pole fracture of the scaphoid was treated by curettage of the nonunion, single K-wire fixation, and implantation of 50 mg of human bone morphogenetic protein followed by 12 weeks of cast immobilization without any conventional corticocancellous bone grafting or rigid screw fixation. Radiographs showed signs of bony healing by 12 weeks and a magnetic resonance imaging scan 6 years after surgery showed no signs of avascular necrosis. The potential future applications of human bone morphogenetic protein in hand surgery are discussed.

Bone and joint surgery--looking ahead.

Looking ahead, one envisions an era of remarkable progress in the study of the healing of bone and cartilage. The physical, biochemical, and biomechanical factors governing bone and cartilage healing will be defined and manipulated to accelerate the healing process. Joint stiffness, the undesirable sequela of many simple fractures of the hand as a result of prolonged immobilization, will be obviated. For cases of limited joint damage, techniques for joint reconstruction will be refined. For non-salvageable bone and joint destruction, microvascular allogenic transplantation of skeletal units wi

Tissue atrophy and subcutaneous edema of the rheumatoid hand. A roentgenographic study of soft tissues.

Soft tissue radiography of the hands using a mammographic immersion technique was performed on 119 patients having definite or classical rheumatoid arthritis and on 123 controls of matching age, sex and professional distribution. The grade of soft tissue atrophy in the hands was visually estimated from the radiographs and the combined cortical thickness of the second metacarpal shaft and skin thickness of the lateral side of the hand were measured. The stage of chronic edema in the absence of synovitis was estimated. Local bone cortical thickness and skin thickness have a highly significant co

Osteomyelitis and septic arthritis complicating hand injuries and infections: pathogenesis of roentgenographic abnormalities.

Osteomyelitis and pyarthrosis may complicate hand injuries and soft-tissue infections, resulting in characteristic radiographic findings. Evaluation of 78 patients revealed bone or joint abnormalities in 10% of individuals. Distinct pathways are available for spread of bacteria which become lodged in the soft tissues of the hand. Such routes include tendon sheaths, fascial planes and lymphatics. An understanding or pertinent anatomy allows the radiologist to predict the site and pattern of soft tissue, articular and osseous alterations.

Hand and wrist tuberculosis in paediatric patients - our experience in 44 patients.

Skeletal tuberculosis (TB) of the hand and wrist is rare, accounting for less than 1% of all osteoarticular TB. Although rare, TB of the hand and wrist is a cause of major morbidity. A common feature among all available reports on TB of the hand and wrist was a delay in diagnosis, causing residual stiffness and pain after treatment. Minimal initial symptoms, rarity of the lesion and ability of wrist TB to mimic more common pathologies account for the delay. Skeletal TB may behave differently in this age compared with the adult population. Further, the disease may affect the growing bone, causi

Costo-Osteochondral Autograft for the Non-salvageable Proximal Scaphoid: A Systematic Review.

Background: Scaphoid non-union is a challenging condition that can lead to chronic wrist pain, decreased range of motion (ROM) and functional impairment. One promising approach involves the use of costo-osteochondral rib autografts, offering a non-vascularised and structurally supportive environment for bone regeneration. This systematic review aims to evaluate the current evidence regarding the management of non-salvageable scaphoid proximal poles and non-union using rib costo-osteochondral autografts. Methods: A comprehensive search of electronic databases was conducted. Data extraction was

Can a Deep Learning Algorithm Improve Detection of Occult Scaphoid Fractures in Plain Radiographs? A Clinical Validation Study.

BACKGROUND: Occult scaphoid fractures on initial radiographs of an injury are a diagnostic challenge to physicians. Although artificial intelligence models based on the principles of deep convolutional neural networks (CNN) offer a potential method of detection, it is unknown how such models perform in the clinical setting. QUESTIONS/PURPOSES: (1) Does CNN-assisted image interpretation improve interobserver agreement for scaphoid fractures? (2) What is the sensitivity and specificity of image interpretation performed with and without CNN assistance (as stratified by type: normal scaphoid, occ

The current management of scaphoid fractures in the emergency department across an Australian metropolitan public health service: A retrospective cohort study.

BACKGROUND: Scaphoid fractures are commonly present to emergency departments (EDs), challenging medical practitioners to achieve accurate diagnosis and management. This is because of the prevalence of radiographically occult scaphoid fractures and complications associated with missed diagnoses. Clinical Guidelines are limited for treatment of suspected scaphoid fractures, and heterogeneity in the literature further complicates management. This study aimed to explore the differences in management between practitioners in the ED and determine if immobilizing clinically suspected scaphoid fractur

Coronal Fracture of the Lunate in Advanced Kienböck Disease: Reestablishing Midcarpal Congruency to Enable Osteochondral Reconstruction: A Case Report.

CASE: We describe a patient with Bain grade-2b Kienböck disease with a coronal fracture of the distal lunate articular surface with proximal structure collapse. To reestablish midcarpal congruity, we used a bone-anchored suture to repair the coronal split in the lunate in preparation for lunate preservation with osteochondral medial femoral trochlea (MFT) reconstruction. CONCLUSION: In Bain grade-2b Kienböck disease, lunate reconstruction of both the proximal and distal joint surfaces can be performed. Successful reconstruction of the distal articular surface can be achieved using a bone-anch

Surgical Considerations for Massive Tarsal Coalitions in Multiple Synostosis Syndrome: A Case Report.

Tarsal-carpal coalition syndrome is an autosomal dominant inherited condition characterized by fusion of the carpal and tarsal bones and foot deformity. Associated pain and/or gait disturbance are the main complaints. The deformity usually consists of varying degrees of hindfoot varus and forefoot supination. The treatment of these patients is mainly aimed at symptomatic relief. We performed a published data review of this condition and discuss our findings in the context of the case of a 10-year-old female with congenital varus deformity of both feet. The tarsal-carpal coalition syndrome has

The biomechanics of fixation techniques for hand fractures.

Multiple options are available for the fixation of metacarpal and phalangeal fractures; treatment options depend largely on the fracture characteristics, concomitant injuries, and surgeon preference. With an understanding of the options available and the biomechanics involved, appropriate treatment options may be chosen. Although plate and screw constructs provide the strongest fixation for these fractures, many other techniques provide adequate fixation and may be favored for reasons other than biomechanics.

Unstable scaphoid nonunion treatment technique: use of a volar distal radius corticocancellous autograft.

Scaphoid nonunions are challenging cases that first must be identified and then treated based on stability, collapse, and vascularity. Unstable nonunions are identified on the basis of collapse of the scaphoid into the "humpback" deformity with an increased intrascaphoid and scapholunate angles. The importance of restoring normal scaphoid alignment has been stressed, because failure to do so leads to worse clinical and radiographic results with predictable patterns of radiocarpal arthrosis. We present a technique to correct unstable scaphoid nonunions using a single volar-based incision with h
