4 cases of photocontact dermatitis due to ketoprofen.

We report 4 cases of photocontact dermatitis due to ketoprofen, a non-steroidal anti-inflammatory drug derived from propionic acid. We carried out a skin biopsy to examine the amount of ketoprofen in the eruptive skin. We investigated the cross-reactions between tiaprofenic acid, suprofen and ketoprofen by patch and photopatch testing. In case no. 1, 17 days after the discontinuance of Mohrus poultice (containing ketoprofen as an active ingredient), we detected ketoprofen 312.5 ng/g in the area of skin where the poultice was applied. All 4 cases reacted positively to the causative medicaments

Oral terbinafine (Lamisil) in the treatment of fungal infections of the skin and nails.

The efficacy and safety of antifungal drugs depend upon their mode of action, the minimal inhibitory concentration (MIC) and its relationship to the minimal fungicidal concentration (MFC), the spectrum of activity and drug kinetics at the involved site. Terbinafine acts at the fungal cell wall. Its MIC against dermatophytes is the lowest of all currently available systemic antifungal agents. It is the only one with an MIC:MFC ratio of 1:1 so that terbinafine should be effective over very short treatment durations in dermatophyte infections of the scalp, palms and soles, and nail, providing tha

Treatment of chronic palmoplantar eczema with local bath-PUVA therapy.

BACKGROUND: Systemic PUVA therapy may be useful in the treatment of chronic palmoplantar eczema. Topical PUVA-paint avoids some of the unwanted side effects of systemic psoralens and has been used successfully in the treatment of palmoplantar eczema and psoriasis. However, few data are available on the effectiveness of local bath-PUVA therapy in palmoplantar eczema. OBJECTIVE: Our purpose was to assess the effectiveness of local bath-PUVA therapy in 28 patients with chronic palmar or plantar eczema or both who were resistant to conventional topical treatment. METHODS: After fungal or bacteri

Chronic recurrent multifocal osteomyelitis and pustulosis palmoplantaris.

Based on nine patients 4 to 26 years of age, we describe the clinical characteristics of chronic recurrent multifocal osteomyelitis. This disorder was characterized by an insidious onset of fever, local swelling and pain in affected bones, and radiologic findings suggesting osteomyelitis. The lesions were mainly localized to the clavicles and the metaphyses of tubular bones. The clinical course was characterized by intermittent periods of exacerbation and improvement over several years. Six of the patients had recurrent pustulosis palmaris and plantaris, which closely parallelled the exacerbat

[Psychological aspects in occupational dermatology].

The prevalence of psychological disorders in the general population and, therefore, in dermatological and allergological patients continues to increase. Psychodermatology as a branch of dermatology is also becoming ever more relevant in occupational dermatology. Psychological comorbidities and cofactors like depression and anxiety disorders or stress are increasingly important, which must be considered regarding diagnostics and therapy selection. Especially stress is becoming recognized as a possible aggravating factor. There have been numerous studies on selected clinical diseases like atopic

A Case of the Co-Existence of Subcorneal Pustular Dermatosis and Pyoderma Gangrenosum and a Review of the Literature.

BACKGROUND: Subcorneal pustular dermatosis, also known as Sneddon-Wilkinson disease, can be classified as one of the neutrophilic dermatoses together with pyoderma gangrenosum. The development of both SPD and PG in the same patient has rarely been reported and may be a strong indicator of IgA dysglobulinemia. CASE REPORT: We report the case of a 34-year-old woman with a 2-year history of relapsing pustular eruptions mainly affecting the abdomen, gluteus region, elbows, and the extremities. Four years after the onset of subcorneal pustular dermatosis (SPD), she developed pyoderma gangrenosum (

The borderline syndrome in psychosomatic dermatology. Overview and case report.

The borderline syndrome is one of the most severe disturbances of psychosomatic dermatology. Patients with borderline syndrome are situated 'on the border' of psychosis, neurosis and personality disorders. The skin as a borderline organ carries a symbolic role. The clinical picture includes artefactual skin diseases due to self-mutilation by conscious or unconscious cutting, and rubbing, scratching or para-artefactual manipulations of pre-existing dermatoses. Leading symptoms of the borderline syndrome are poor impulse control, emotional instability and poor ego strength with low frustration t

[Antipruritic effects of pimecrolimus and tacrolimus].

The development of topical calcineurin inhibitors resulted in a significant improvement in the treatment of inflammatory skin diseases such as atopic dermatitis. In addition, an excellent amelioration of pruritus could be observed. Other itchy dermatoses such as chronic irritative hand dermatitis, rosacea, graft-versus-host-disease, renal pruritus, lichen sclerosus, prurigo simplex, prurigo nodularis, scrotal eczema, and inverse psoriasis also have been treated successfully with pimecrolimus and tacrolimus. The antipruritic effect currently is believed to be related to the inhibition of inflam

Contact dermatitis in the textile industry: a review of 72 patients.

BACKGROUND: Occupational skin disease can cause significant morbidity in textile industry workers. Both allergic contact dermatitis (ACD) and irritant contact dermatitis (ICD) have been reported. OBJECTIVE: The purpose of this study was to characterize and determine the relative frequency of work-related ACD and ICD in textile workers. METHODS: Seventy-two textile industry workers seen during a 5-year period were evaluated for possible work-related contact dermatitis. All patients underwent patch testing with a screening series. Many patients (70%), depending on occupational exposures, also

Chromoblastomycosis caused by a meristematic mutant of Fonsecaea monophora.

We report the first case of chromoblastomycosis caused by a meristematic mutant of Fonsecaea monophora in an 81-year-old Cantonese male. The patient had a seven-month history of a red plaque with thick yellow crust and purulent exudates on the back of his right hand. Microscopic examination of direct smears revealed brown sclerotic cells and black colonies were recovered in culture from samples of the purulent exudantes. The microscopic appearance of this isolate was quite different from that of other Fonsecaea species, exhibiting mere meristematic growth. The rDNA ITS sequence data confirmed

Evaluating interventions aimed at reducing occupational exposure to latex and rubber glove allergens.

OBJECTIVE: Concerns over occupational exposures to blood-borne viruses resulted in increased protective glove use; consequentially latex allergy became a hazard for some occupational groups. Interventions aimed at managing this problem included substitution measures (eg, non-powdered/non-latex gloves), but such changes may not occur simultaneously across occupational sectors. This study evaluated whether the incidence of occupational dermatoses fell after interventions aiming to reduce exposure to 'latex and rubber glove allergens' ('latex') were introduced, and whether these interventions wer

Workplace screening for hand dermatitis: a pilot study.

BACKGROUND: Health care workers (HCWs) are at increased risk for developing occupational skin disease (OSD) such as dermatitis primarily due to exposure to wet work. Identification of risk factors and workplace screening can help early detection of OSD to avoid the condition becoming chronic. AIMS: To determine risk factors and clinical findings for hand dermatitis using a workplace screening tool. METHODS: Employees at a large teaching hospital in Toronto, Canada, were invited to complete a two-part hand dermatitis screening tool. Part 1 inquired about hand hygiene practices and Part 2 comp

Successful Treatment of a Bullous Pemphigoid Patient with Rituximab Who Was Refractory to Corticosteroid and Omalizumab Treatments.

Omalizumab is a humanized monoclonal antibody which is an FDA-approved treatment of severe allergic asthma and inhibits IgE binding to FcεRI. According to increasing evidence of IgE inhibition, omalizumab was suggested as a therapeutic approach for bullous pemphigoid (BP). Rituximab has been reported to be effective in various autoimmune diseases, including autoimmune bullous dermatoses. A specific protocol for the use of rituximab to treat BP patients is not yet available. There are only small case series and case reports about the efficacy and safety of rituximab in BP. Here we present a you

Pustular skin disorders: diagnosis and treatment.

The differential diagnosis for pustular skin disorders is extensive. The distribution of the lesions and the age of the patient are characteristics that may provide strong clues to the etiology of cutaneous pustular eruptions. In adults, generalized pustular dermatoses include pustular psoriasis, Reiter's disease and subcorneal pustular dermatosis. Medications can cause generalized pustular eruptions, such as in the case of acute generalized exanthematous pustulosis; or more localized reactions, such as acneiform drug eruptions, which usually involve the face, chest and back. Localized pustula

Cinnamic Acid Derivatives as Potential Multifunctional Agents in Cosmetic Formulations Used for Supporting the Treatment of Selected Dermatoses.

Cinnamic acid and its natural derivatives were primarily used in cosmetics as fragrance materials as well as skin and hair conditioners. Nowadays, not only natural but also synthetic cinnamic acid derivatives are used as active ingredients of cosmetic formulations. They still serve as fragrance ingredients but also as active ingredients supporting the treatment of selected dermatoses such as acne vulgaris, atopic dermatitis, and hyperpigmentation. They are also commonly used in anti-aging cosmetic formulations. On the other hand, several cinnamic acid derivatives used as fragrances in cosmetic

Skin diseases in children in Singapore.

Singapore has developed over the past 3 decades with improvement in housing, nutrition and general health. The pattern of skin diseases has changed from conditions like pyodermas and malnutrition to predominantly eczemas. The paediatric cases seen at Middle Road Hospital consist of 13% of general dermatology cases (509 of 37,964 total new cases in 1986). About 2.7% (138) of the children were below 1 year, 32.5% (1630) were below 1-4 years and 64.5% (3251) were below 14 years of age. The ten most common dermatoses (1986) follow a similar trend in most of the developed towns of Asia. This includ

Allergic Contact Dermatitis to 1,3-Diphenylguanidine Over a 23-Year Period: Recent Cases Involve Sterile Polyisoprene Gloves.

BACKGROUND: 1,3-Diphenylguanidine (DPG) is a known contact sensitiser and rubber vulcaniser. METHODS: A retrospective audit was performed including 2675 patients tested to 1% DPG in petrolatum between 2002 and 2024. The MOAHLFA index of those with allergic contact dermatitis (ACD) was compared to the remainder of the patch test clinic population. Temporal changes in the prevalence of contact allergy and ACD to DPG were assessed. RESULTS: 50 (1.9%) patients had positive reactions to DPG on patch testing, with 31 (62% of positive reactions) diagnosed with ACD. In those with ACD, there was a si
