Benign lymphoid polyp (localized lymphoid hyperplasia) at the tip of the cecum associated with lymphoid hyperplasia of the appendix.

A case of benign lymphoid polyp (localized lymphoid hyperplasia) manifesting as a sessile polypoid lesion in the tip of the cecum is described. Solitary lymphoid polyps are extremely rare in the proximal part of the large bowel. The polyp was located at the base of the appendix, and microscopically both the polypoid lesion and the appendix revealed lymphoid hyperplasia.

[Unusual tumours of the digestive tract (author's transl)].

The authors summarize the difficulties in diagnosis some tumour-like lesions or digestive tumours. They rely upon thirty years experience in digestive pathology and upon a survey of relevant literature. They recall the difficulties arising from the inflammatory tumour-like lesions (infiltrating sclerosis or granulomas, colitis cystica profunda, inflammatory polyps). They mention the circumstances which bring about errors or incomplete diagnosis among these very tumours. Some difficulties are due to the circumstances in which these tumours appear : the digestive cancers of the child, the assoca

Evidenced-Based Screening Strategies for a Positive Family History.

The most commonly recognized high-risk group for colorectal cancer (CRC) is individuals with a positive family history. It is generally recognized that those with a first-degree relative (FDR) with CRC are at a 2-fold or higher risk of CRC or advanced neoplasia. FDRs of patients with advanced adenomas have a similarly increased risk. Accordingly, all major US guidelines recommend starting CRC screening by age 40 in these groups. Barriers to screening this group include patient lack of knowledge on family and polyp history, provider limitations in collecting family history, and insufficient app

A case of ectopic pancreas in the ileum presenting as obscure gastrointestinal bleeding and abdominal pain.

BACKGROUND: Ectopic pancreas is an infrequent submucosal tumor in the gastrointestinal tract defined as a pancreatic tissue lacking vascular or anatomic continuity with the main body of the pancreas. Ectopic pancreas in the ileum is a rare and often an incidental finding. We report a case of ectopic pancreas in the ileum causing obscure gastrointestinal bleeding and episodes of abdominal pain. CASE PRESENTATION: 59-year-old man with 3 months history of intermittent melena, accompanied by the episodes of abdominal pain in the left upper quadrant and generalized fatigue was admitted to our depa

Management of Non-neoplastic Gastric Lesions.

Benign gastric lesions represent various pathologic entities and management considerations. Upper endoscopy serves as the primary diagnostic modality for gastric lesions. Persistent or giant gastric ulcers represent unique subtypes of ulcers, requiring investigation of the underlying cause. Medical management remains the mainstay of treatment; however, indications for surgical intervention remain. Gastric polyps also represent diverse etiologies, and accurate diagnosis requires pertinent information and tissue samples. Neoplastic lesions often present as polypoid lesions; a high index of suspi

[SERRATED COLORECTAL ADENOCARCINOMA].

THE AIM: To present a clinical case of serrated colon adenocarcinoma. To demonstrate morphological and genetic features of these tumors and to determine their clinical management. KEY POINTS: Described case shows diagnostics of serrated colon adenocarcinoma. A 67-year-old woman presented for colonoscopy because of long constipation besides abdominal pain and distention have appeared recently. Colonoscopy was performed and she was found to have a 3.0 x 3.0-cm tumor next to hepatic flexure and pedunculated 1.2x0.7 cm polyp in sigmoid colon (type lp according to Paris classification). Endoscopic

Mantle cell lymphoma: a rare cause of a solitary duodenal mass.

Mantle cell lymphoma is a very aggressive lymphoma with a very poor prognosis. It commonly involves the gastrointestinal tract but rarely presents as primary gastrointestinal lymphoma. The most notable cases of primary gastrointestinal mantle cell lymphomas have been described as multiple lymphomatous polyposis and have a very poor prognosis. We report a case of primary gastrointestinal mantle cell lymphoma that was discovered by endoscopic biopsy of a single duodenal polyp in a 70-year-old woman who was previously treated for Helicobacter pylori gastritis. She presented with a 6-month history

[Association of brownish polyposis and diverticulosis in the sigmoid colon -- a case of mucosal prolapse syndrome].

The authors present a case of a 48 year-old man, who was diagnosed with several brownish sigmoid polyps of 1-2 cm size and diverticulosis on colonoscopy. Subsequently, laparoscopic sigmoid resection was carried out due to lower gastrointestinal bleeding. Histological examination revealed diverticulosis associated with polyposis. This rare entity is known in the literature as prolapse-type inflammatory polyp, which is a type of mucosal prolapse syndrome. The brownish discolouration was caused by hemosiderin deposition.

Radio-guided resection of non-palpable rectal carcinoma following polypectomy.

Due to widespread use of screening procedures for colorectal cancer, an increasing number of colorectal malignancies is diagnosed at an early stage. A special subgroup of these patients has polyp carcinoma, which can be diagnosed with histological examination of the removed specimen. In such cases the exact intraoperative localization of the non-palpable lesion is a greater challenge than resection itself. In our case report, radio-labeled colloid was used to identify non-palpable rectal carcinoma after polypectomy in a high risk patient. Using a navigator device, the submucosal injected collo

Endoscopic mucosal resection for advanced sessile adenoma and early-stage colorectal carcinoma.

BACKGROUND AND STUDY AIMS: The aim of this study was to evaluate the efficacy and outcomes of treatment by endoscopic mucosal resection (EMR) of patients with high-grade dysplasia (HGD) or carcinoma. PATIENTS AND METHODS: Between January 1995 and January 2002, 50 patients (35 men, 15 women) were treated by EMR for 52 sessile polyps. The median size of the polyps was 27.5 mm (range 10-60). The "lift and cut" EMR technique was used. If the lesion was poorly differentiated or infiltrated the muscularis mucosae to more than 1000 microm, the patient was referred for colectomy. In the other cases,

Inflammatory fibroid polyp of the ileum causing intussusception: report of two cases with emphasis on cytologic diagnosis.

Inflammatory fibroid polyp (IFP) of the gastrointestinal tract is a type of inflammatory pseudotumor or inflammatory myofibroblastic tumor that occurs most commonly in the stomach but also in the small and large bowel. Small-bowel IFP usually presents with intussusception. The purpose of the current study is to describe cytological features of this lesion with differential diagnoses since pathologists may be called on to render a diagnosis on fine-needle aspiration. Two cases of IFP are described with diagnostic features on imprint smears. Both were middle-aged obese women with a history of pr

Flexible sigmoidoscopy.

Flexible sigmoidoscopy remains a common tool used for the periodic screening of colorectal cancer. Most organizations recommend screening at three- to five-year intervals beginning at age 50 for persons with average risk. Extensive training in endoscopic maneuvering, colorectal anatomy and pathologic recognition is required. Most physicians report comfort performing the procedure unsupervised after 10 to 25 precepted sessions. The procedure involves the insertion of the sigmoidoscope through the anus and distal rectum and advancement of the scope tip to an average depth of 48 to 55 cm in the s

Filiform polyposis of the colon in chronic inflammatory bowel disease (so-called giant inflammatory polyps).

On the basis of 3 of our own cases, we describe unusually intense forms of filiform polyposis and local giant polyposis as a consequence of chronic inflammatory bowel disease. The patients are: A 52-year-old woman who for 7 years has been known to have Crohn's disease (CD); a 55-year-old man who for 14 years has been known to have chronic inflammatory bowel disease, which was first thought to have been ulcerative colitis, but, as a result of the findings on the subtotal colectomy specimen, had to be classified as Crohn's disease or colitis indeterminate; and a 53-year-old woman known to have h
